Understanding half life is important to inform how and when we administer any medication. These are not casual viewing, but are for serious students who want to understand the essential underpinning concepts of the care we give our patients. Notes Half life t 1/2 Pharmacokinetics and Pharmacodynamics Bioavailability – the proportion of an administered drug that reaches the systemic circulation in unchanged form Half life is the time from peak plasma concentration until half is eliminated Clearance T ½ First order kinetics First order elimination kinetics : "Elimination of a constant fraction per time unit of the drug quantity present. The elimination is proportional to the drug concentration." Zero-order elimination kinetics : The plasma concentration – time profile during the elimination phase is linear (Fig. 1). For example 20 mg are eliminated every hour, independently of the drug concentration in the body. Order 0 elimination is rather less common, mostly occurring when the elimination system is saturated. Eg. Alcohol Phenytoin Warfarin (Coumadin) Heparin Paracetamol (acetaminophen) Over dose of aspirin (normally t ½ = 3-4 hours) Steady state reached after 5 – 6 half lives (3.3 half lives for 90% concentrations), therefore possible loading dose. Drug eliminated after 5 half lives (95% eliminated after 4.5 half lives) Half life t ½ of common drugs Noradrenaline 2 minutes Salbutamol 1.6 hours Morphine 2 – 3 hours Methadone 24 hours Gentamycin 2 – 3 hours Penicillin 6 – 8 hours Diazepam 24 hours Fluoxetine 6 days Warfarin 4 days

Applied Pharmacology 5, Theraputic Index
Therapeutic index is a measure of how safe a drug is, comparing the effective dose with the toxic dose. You can support Campbell Teaching for free if you use the link below to access Amazon! USA http://goo.gl/mDMfj5 UK http://goo.gl/j0htQ5 Thank you! Notes The therapeutic index (TI) (also called therapeutic ratio) is a comparison of the dose of a drug that causes the therapeutic effect to the amount that causes toxicity. This describes the margin of safety, accounting for the effective dose, toxic dose and even potential lethal dose. The related terms therapeutic window or safety window refer to a range of doses which optimize between efficacy and toxicity. This is always where we aim to be. High (wide) TI is good for good safety profile, drug has low toxicity Low (narrow) TI is more risky (NTI drugs, narrow therapeutic index), drug more toxic TD (toxic dose) TI = ----- ED (effective dose) TD50 (toxic dose) TI = ----- ED50 (effective dose) Lethal dose Toxicity effects Adverse response threshold Therapeutic window / range Duration of action Sub therapeutic range Desired response threshold Eg, anticoagulants, hypotensives, hypoglycaemics, analgesics, diuretics, Low (narrow) Therapeutic Index drugs Digoxin 2:1 Paracetamol (acetaminophen) 7 gram fatal dose Lithium – nephrotoxicity, ataxia, discordination Warfarin - Gentamycin – nephrotoxic, auditory nerve damage - ototoxicity Vancomycin - nephro Phenytoin, fast uncontrolled eye movements, dizzy confused, poor coordination, slured speech Antidepressants, tricyclics Insulin, AZT, High (wide) TI drugs NSAIDs Benzodiazepines Most antibiotics Furosemide SSRIs Does not include any synergistic effects, e.g. diazepam and alcohol or diazepam and morphine, aspirin and warfarin, pharmacokinetic, pharmacodynamics effects, eg. Stop Rat Poison: Dying Rat
Stop Rat Poison!!! One night while walking in the kitchen I found a rat that did not move even by my presence. I thought that it was sleeping so I started filming it. But my presence made no impact on it and this made me worried. I tried to disturb him but he was very slow. Then what I saw was a dying rat. I was its mouth bleeding. It tried to scare it and then it moved. I followed it and later found the blood stains on the floor. I realized that it was suffering after eating rat poison that was put by my mother to get rid of many such rats. Next day I told this incident to my mother. And we decided that we won't use rat poison and rather only use cage. Since then we are using cages and tried not to hurt rats. Please stop using rat poison as it gives immense suffering to rats. - Nikhil Sablania sablanian@gmail.com

Aspirin - mechanism of action, acetylsalicylic acid action mechanism
1. Mechanism of action: Stimulation of platelets by thrombin, collagen, and ADP results in activation of platelet membrane phospholipases that liberate arachidonic acid from membrane phospholipids. Arachidonic acid is frst converted to prostaglandin H2 by COX-1 (Figure 22.6). Prostaglandin H2 is further metabolized to thromboxane A 2, which is released into plasma. Thromboxane A2 promotes the aggregation process that is essential for the rapid formation of a hemostatic plug. Aspirin [AS-pir-in] inhibits thromboxane A2 synthesis by acetylation of a serine residue on the active site of COX-1, thereby irreversibly inactivating the enzyme (Figure 22.7). This shifts the balance of chemical mediators to favor the antiaggregatory effects of prostacyclin, thereby preventing platelet aggregation. The inhibitory effect is rapid, and aspirin-induced suppression of thromboxane A 2 and the resulting suppression of platelet aggrega tion last for the life of the platelet, which is approximately 7 to 10 days. Repeated administration of aspirin has a cumulative effect on the function of platelets. Aspirin is the only antiplatelet agent that irre versibly inhibits platelet function.

Anyway, The Drugs Song has no official video so here's my attempt to bring this brilliant song into the visual medium! Enjoy! Antidepressants Increase Adverse Reactions in Surgical Patients
According to an article published in the April 2013 issue of the journal Internal Medicine, patients on SSRI antidepressants at the time of surgery have a 20% increased mortality, 9% increased risk for bleeding, and 22% increased risk for readmission within a month of discharge from the hospital! This was based on a review of 530,000 surgical patients from 2006-8. SSRIs such as Celexa, Lexapro, Prozac, Luvox, Paxil, and Zoloft, interfere with platelet function that can interfere with blood coagulation. Common side effects of SSRIs include nausea, vomiting, diarrhea, insomnia, impotence, suicidal ideation, mania and much more. They interfere with the toxicity of Coumadin, digitalis, antiarrhythmic drugs, tricyclic antidepressants, triptans, benzodiazepines, and dilantin. SSRIs toxicity is increased by alcohol, diuretics, Sudafed, lithium, Meridia, Ambien, Tramadol, and St. Johns Wort. For a family of drugs that is no more effective for placebo for mild to moderate depression it should make you wonder why they are used most of the time! Therapeutic window & Therapeutic Index by K M Istieake I Life Line
Therapeutic window by K M Istieake Therapeutic window is a range of doses that produces therapeutic response without causing any significant adverse effect in patients. Generally therapeutic window is a ratio between minimum effective concentrations (MEC) to the minimum toxic concentration (MTC). The levels of drug should always be between MEC and MTC in order to provide risk free therapeutic effects. If any drug crosses MTC then it will surely elicit toxic effects and if drug is unable to surpass MEC then it will cause therapeutic failure. MEC is also called as minimum inhibitory concentration (MIC). NSAIDs have wider therapeutic index and warfarin has narrow therapeutic index as it has therapeutic index less than two. Examples of Drugs: Here is a list of some common potent drugs having low therapeutic index; Digoxin Lithium Warfarin Theophylline Phenytoin Gentamicin Amphotericin B 5-fluorouracil Presented by K. M. Immunisation: Getting to the Point
Immunisation is the safest and most effective way of giving protection against disease. Protection through immunisation has both an individual and population aspect. If enough people in the community are immunised, the infection can no longer be spread from person to person and the disease dies out altogether - 'herd immunity'. Consequently smallpox was eradicated from the world and polio has been eliminated from many countries. Since the first mass immunisations were introduced in 1932, immunisation has been successful in saving the lives of millions of people world wide. Today Australian researchers are at the forefront of developing new vaccines. In the past two years there have been a large number of discoveries of vaccines for diseases such as H1N1 and seasonal flu. Currently there is work on dengue fever, H5N1flu, a generic flu vaccine, asthma, and Golden Staph, as well as work on vaccines being administered via nasal drop/spray. This new program covers the updates to the National Immunisation Program Schedule, new vaccines, vaccine safety and best practice. It discusses recent outbreaks and pandemics as well as the benefits and impacts of immunisation across age groups. Issues such as: developing awareness and protocols for assisting Indigenous and culturally and linguistically diverse communities (CALD); ensuring informed consent; common misconceptions and risk factors, are all covered. NCLEX Nursing Interventions for Amlodipine -- Top NCLEX Meds
Learn the Top Meds on NCLEX RN exam http://www.amazon.com/dp/B00EMLFF0S/ref=rdr_kindle_ext_tmb Get NCLEX and Nursing tips @ https://www.facebook.com/nclex.preceptor and http://www.pinterest.com/nclexp/ Author is Registered Nurse Jonathan Small Drug information provided by: Micromedex US Brand Name Norvasc Descriptions Amlodipine is used alone or together with other medicines to treat angina (chest pain) and high blood pressure (hypertension). High blood pressure adds to the workload of the heart and arteries. If it continues for a long time, the heart and arteries may not function properly. This can damage the blood vessels of the brain, heart, and kidneys, resulting in a stroke, heart failure, or kidney failure. High blood pressure may also increase the risk of heart attacks. These problems may be less likely to occur if blood pressure is controlled. Amlodipine is a calcium channel blocker. It affects the movement of calcium into the cells of the heart and blood vessels. As a result, amlodipine relaxes blood vessels and increases the supply of blood and oxygen to the heart while reducing its workload. This medicine is available only with your doctor's prescription. This product is available in the following dosage forms: Tablet Tablet, Disintegrating In deciding to use a medicine, the risks of taking the medicine must be weighed against the good it will do. This is a decision you and your doctor will make. For this medicine, the following should be considered: Allergies Tell your doctor if you have ever had any unusual or allergic reaction to this medicine or any other medicines. Also tell your health care professional if you have any other types of allergies, such as to foods, dyes, preservatives, or animals. For non-prescription products, read the label or package ingredients carefully. Pediatric Appropriate studies have not been performed on the relationship of age to the effects of amlodipine to treat high blood pressure in children younger than 6 years of age. Safety and efficacy have not been established. Geriatric Appropriate studies performed to date have not demonstrated geriatric-specific problems that would limit the usefulness of amlodipine in the elderly. However, elderly patients are more likely to have age-related kidney, liver, or heart problems, which may require caution and an adjustment in the dose for patients receiving amlodipine. Precautions Drug information provided by: Micromedex It is very important that your doctor check your progress at regular visits to make sure this medicine is working properly. Blood tests may be needed to check for unwanted effects. a Dizziness, lightheadedness, or fainting may also occur if you exercise or if the weather is hot. Heavy sweating can cause loss of too much water and result in low blood pressure. Use extra care during exercise or hot weather. If you have been using this medicine regularly for several weeks, do not suddenly stop using it. Stopping suddenly may cause your chest pain or high blood pressure to come back or get worse. Check with your doctor for the best way to reduce gradually the amount you are taking before stopping completely. Chest pain resulting from exercise or physical exertion is usually reduced or prevented by this medicine. This may tempt you to be too active. Make sure you discuss with your doctor a safe amount of exercise for your medical problem. After taking a dose of this medicine you may get a headache that lasts for a short time. This should become less noticeable after you have taken this medicine for a while. If this effect continues, or if the headaches are severe, check with your doctor. In some patients, tenderness, swelling, or bleeding of the gums may appear soon after treatment with this medicine is started. Brushing and flossing your teeth carefully and regularly and massaging your gums may help prevent this. See your dentist regularly to have your teeth cleaned. Check with your doctor or dentist if you have any questions about how to take care of your teeth and gums, or if you notice any tenderness, swelling, or bleeding of your gums. Do not take other medicines unless they have been discussed with your doctor. This especially includes over-the-counter (nonprescription) medicines for appetite control, asthma, colds, cough, hay fever, or sinus problems, since they may increase your blood pressure.

Dietary Supplement Practicum (19 of 21): Dietary Supplements for Weight Loss
(Bethesda, MD) Carol Haggans, M.S., R.D., scientific and health communications consultant in the National Institutes of Health (NIH) Office of Dietary Supplements (ODS), gives an overview of the weight-loss supplement market and the types of products available. She summarizes the scientific evidence for selected ingredients in these products (including hoodia, chitosan, garcinia cambogia, raspberry ketone, chromium, and green tea) and the quality of that evidence. Ms. Haggans uses ephedra as a case study and discusses potential safety issues for weight-loss supplement ingredients, including possible interactions with drugs and other supplements.
