Vitamin D role and use in prediabetes. Barengolts E. Department of Medicine, Section of Endocrinology, Jesse Brown VA Medical Center and University of Illinois, Chicago, Illinois, USA. eibareng@uic.edu Abstract OBJECTIVE: To review the role of vitamin D in prediabetes on the basis of evidence from human studies. METHODS: English-language literature in MEDLINE (January 1969-July 2009) was searched for observational studies and randomized controlled trials of vitamin D deficiency and treatment in prediabetes, including impaired fasting glucose, impaired glucose tolerance, and metabolic syndrome. Search terms included hyperglycemia, glucose, glycohemoglobin, insulin resistance, diabetes, homeostasis model assessment, insulin secretion, vitamin D, and related terms. Publications were also identified from review articles and references in the found articles. Abstracts, conference proceedings, case reports, and letters were excluded. Articles concerning only type 1 and type 2 diabetes, hemodialysis, or hyperparathyroidism and studies in children were also excluded. RESULTS: Vitamin D insufficiency is defined by a circulating 25-hydroxyvitamin D concentration less than 30 ng/mL, and it is prevalent in the United States (77% of the population). Most cross-sectional and prospective studies in various populations show inverse association between circulating 25-hydroxyvitamin D and fasting plasma glucose, impaired glucose tolerance, hemoglobin A1c, metabolic syndrome, and incidence of prediabetes. A few clinical trials suggest beneficial effect of vitamin D supplementation in prediabetes, including improved insulin secretion, basal fasting insulin sensitivity, and postprandial peripheral insulin resistance. The limitations of the studies are small sample size, short duration of follow-up, lack of control groups, and inability to achieve vitamin D sufficiency with treatment. CONCLUSION: Available data suggest that achieving vitamin D sufficiency may be beneficial in patients with prediabetes, although clinical trials are needed to provide evidence-based recommendations.

For example; when elevated blood pressure was tightly controlled, diabetic related deaths were reduced by 32% compared to those with less controlled blood pressure. Many observational and clinical studies had been conducted to investigate the role of several vitamins on diabetic complications, the results of these studies elevated a suggested beneficial role of vitamins on diabetic complications. In the First National Health and Nutrition Examination Survey (NHANES I) Epidemiologic Follow-up Study, vitamin supplementations were observed to be associated with 24% reduction on the risk of diabetes was observed during 20 years of follow-up. Many observational studies and clinical trials have linked several vitamins with the pathological process of diabetes; these vitamin include folate, thiamine, β-carotene, and vitamin E, C, B 12, and D. However, numerous researches had been shown inconsistent results about the roles of vitamins on diabetic risk and complications. Most of these researches performed to investigate effect of individual vitamin without looking to status of other vitamins. Even though, it is expected that vitamin supplementation might be more effective and might induce a beneficial role on diabetic process when deficiency exists. Despite the current contradictory in the association between the discussed vitamins and diabetic complications, and regardless of the lack of strong and inclusive evidence about their mechanism of action, the discussed effects of these vitamins seem promising for preventing and reducing the severity of diabetic complications. Since optimal blood level of all vitamins is important for normal metabolic process, it is essential to encourage diabetic patients and high risk population to try to achieve and maintain this level. And until more inclusive evidence is established about vitamins supplementations, the awareness of diabetic patients should be elevated toward the importance of consuming adequate amounts of all vitamins. Thiamine: Thiamine acts as an essential cofactor in glucose metabolism, therefore, it may modulate diabetic complications by controlling glycemic status in diabetic patients. Additionally, deficiency of thiamine was observed to be associated with dysfunction of β-cells and impaired glucose tolerance. Different studies indicated possible role of thiamin supplementation on the prevention or reversal of early stage diabetic nephropathy, as well as significant improvement on lipid profile. Today I'm going to answer the question, is turmeric good for diabetes. Turmeric is an herbaceous perennial plant of the ginger family. It has been used in the Middle East and East India for years and has a peppery flavor. It is a very popular spice now worldwide. You probably recognize the taste as the main spice in curry. But turmeric is much more than a flavoring aid in cooking. I have found in my research that has been used for medicinal purposes for everything from arthritis to flatulence. It is used as an enema and skin cream. But what about the uses for turmeric in regards to diabetes? That is the question posed. Recent claims concerning turmeric have many people taking notice of this spice. Can turmeric prevent or actually reverse Type 1 and Type 2 diabetes? An article published in the International Journal of Endocrinology and Metabolism in October, 2014 makes a pretty remarkable statement concerning turmeric and diabetes. The article focused on studies from 1998 to 2013. These studies looked at the key component in turmeric, curcumin, and its effects on insulin resistance. The conclusion of the article, after reviewing all the studies, was that curcumin can reduce blood glucose levels. The article also concludes that “more studies evaluating the effects of curcumin on hyperglycemic state and insulin resistance in related disorders such as diabetes are recommended.” Another study, published in the American Diabetes publication, “Diabetes Care” found that after a 9 month curcumin intervention in pre-diabetic subjects, a significant population of those studied did not develop Type 2 diabetes. Now, this is not to say that using turmeric will cure diabetes, as some have claimed. It does suggest that curcumin has properties to help prevent and aid in the control of diabetes. A major plus also is that turmeric is a readily available, relatively inexpensive spice. So, is turmeric good for diabetes? Yes it is. Is it a cure? Well, more research has to be done to determine that. But in my research I have found that turmeric has few, if any, side effects and can help lower blood glucose levels. I am not ready to throw away my diabetes medication yet, but I will consider adding turmeric to my diet. As always, I suggest you consult your health care provider before taking any supplements that contain curcumin and monitor your blood sugar regularly if you do decide to add turmeric to your diet. So when the insulin is spiked too much or too fast in the body, it drops the sugar too low. The symptoms of low blood sugar include headache, stomach noise, brain dead, weepiness, craving sweets, anxiety, depression, moody, paranoid, fatigue, fear. So we should avoid having sugar in case of any hypoglycemia symptoms. A person with diabetes can have both forms of blood sugar levels, i.e. hyperglycemia (high blood sugar) or hypoglycemia (low glucose levels). Calcium and vitamin D probably are not enough, however, and treatment with bisphosphonates such as alendronate (Fosamax) and risedronate (Actonel) may be necessary. Calcitonin (Miacalcin) also is effective. The development of osteoporosis and the need for treatment can be monitored using bone density scans. Adrenal insufficiency and weaning off prednisone: Prolonged use of prednisone and other corticosteroids causes the adrenal glands to atrophy (shrink) and stop producing the body's natural corticosteroid, cortisol. tapering off prednisone after short term use Necrosis of hips and joints: A serious complication of long-term use of allergic reaction Prednisone is aseptic necrosis of the hip joints.
