My video is about the treatment of alcoholism with niacin. This is the first video in a series I'm doing called "Nutrition & Alcoholism" where I focus on different nutritional steps that helped me cure my addiction and hopefully it can help others. I was an alcoholic for years, but I changed when I began seeing alcoholism from a nutritional viewpoint. It took years of research--of trial and error--but I cured myself of this addiction through taking the right supplements and focusing on healing my entire body through nutrition. Alcoholism is a poison on every single system of the human body. It effects the liver, the kidneys, the pancreas, the skin, the brain--so since it's creates such an onslaught on the body it only makes sense that the only way you can cure it is by healing the entire body. One of the first things that gave me hope that I could change was niacin. Niacin exerts a number of powerful benefits on the body and mind all of which can be beneficial to alcoholics (and anyone). It's an anxyolitc (meaning that it relieves anxiety). It works on the benzodiazepine receptors (the same ones that Valium and Xanax work on). It also helps relieve depression and it aids in detox just to name a few. So many people are on some sort of medication for depression these days and I started thinking about it--what's with the explosion of depression in this country? Mental health issues didn't seem to be so ubiquitous a hundred years ago. And I think I came up with an answer (it's not a shock that it all comes down to money). Niacin is added to food (bread, grains, etc) because as a population we don't readily get niacin from our modern American diet. However, there's two forms of niacin--nicotinic acid and niacinamide--and niacinamide is the form that's added to food because it's cheaper. Niacin is critical for the formation of serotonin, the feel good neurotransmitter. It goes like this--the amino acid tryptophan is used to make niacin and serotonin and niacin production gets priority over serotonin. When the body senses that there isn't enough niacin it will take the raw materials it would use to make serotonin to keep making niacin. Here's the rub: niacinamide doesn't shut off the niacin pathway like the more expensive nicotinic acid, so the body constantly thinks that there isn't enough niacin and it redirects its supply of tryptophan from serotonin production to keep making niacin. The less expensive niacinamide keeps the population from developing the more obvious diseases associated with niacin deficiency like pellagra, but at a cost to mental health--leading to a whole host of problems ranging from depression to schizophrenia to alcohol dependency. Now, I feel like it is my mission to share what I've learned with the world and hopefully help others suffering. Plus, I love all things nutrition. 12 Ways Your Body Tries To Tell You That Your Liver Is Being Damaged The liver is the largest solid organ in your body. It is located in the right upper quadrant of your abdomen and partly protected by the ribs. You need your liver for many important bodily functions and cannot live without your liver. For this reason, you should know the early signs of liver damage and take care of your liver as much as you possibly can. There are more than a hundred different kinds of diseases of the liver. Each has its own set of symptoms. Some of these diseases include alcoholic liver disease, hepatitis, fatty liver disease, and cirrhosis of the liver. The early signs of liver disease tend to be easily overlooked and are non-specific, related to any number of different conditions. Here are some common early signs. 1. Upset Stomach You are likely to get nausea and vomiting. They can mimic other diseases, such as migraines, food poisoning, vertigo, motion sickness, early pregnancy, depression and vomiting. It can mimic kidney problems as well. Those who suffer from early liver damage often feel persistently nauseous, because the liver has a decreased ability to rid the body of toxins. The nausea can also be due to ongoing metabolism and changes in digestion. If you have these symptoms for several days and don’t know why, seek the attention of your doctor. 2. Fatigue and Overall Weakness One of the early and obvious signs of liver damage is constantly feeling tired and weak. When the liver is not in great condition, the body must work harder and you will need more rest. This is due to an increase in toxins in the bloodstream that can’t be cleared out because the liver can’t get rid of the toxins properly. If the symptoms persist, especially with other early signs of liver damage, seek medical advice as soon as you can. 3. Lack of Appetite This happens due to a lack of production of bile that helps digest fat in the diet. When food isn’t properly digested, it can lead to a lack of appetite and a severe loss of weight. See your doctor if you experience any of these symptoms for a long period of time. 4. Digestion Problems The liver is important in digestion because it makes bile that helps the nutrients be better processed and absorbed by the small intestines. If the liver is failing, you can develop an increase in indigestion and diarrhea. If the bile is not produced properly, you can also experience irritable bowel syndrome, fatty food intolerance, alcohol intolerance, abdominal bloating, constipation and gallstones. If this becomes severe, you need to seek the medical attention of a doctor. 5. Alterations in the Color of Urine If you think you are drinking enough but still have a dark color of your urine, this may be one of the early signs of liver damage. The urine may look brown, amber, or orange in color because of elevated levels of bilirubin in the bloodstream. The bilirubin is elevated because the liver cannot get rid of the bilirubin and it is excreted through the kidneys. Dark urine may also be due to dehydration, side effects of taking antibiotics, taking B vitamin supplements, urinary tract infections, enzyme deficiencies or kidney abnormalities. The doctor can help sort out the cause of dark urine. 6. Changes in Stool Coloration Because the bile is not made enough by a malfunctioning liver, the color of the stool changes. Instead of being brown, it can become pale yellow, clay colored, or gray in appearance. Hi, I'm Dr. Jared Nielsen. I'd like to talk today about the importance of a vitamin called thiamine or B1. B1 is very, very important for liver function, especially for people who have a history of alcoholism. It's quickly depleted in the system and is essential for that person to be repleted, if you will, their B1 vitamin. There are a lot of people that don't think, "I am really an alcoholic or I don't really have a drinking problem." What's interesting is they'll start to manifest some of the symptomatology of B1 deficiency, peripheral neuropathies, sensitivity or hyper‑sensitivity in the legs and feet. These are very commonly overlapped with diabetics. Diabetics also would get that type of symptomatology as the liver becomes congested or a fatty infiltrated liver. When we look at blood labs, a lot patients will actually say, when we see cholesterol levels going up and triglyceride levels matching or even exceeding that they don't drink alcohol. What we see a lot of times is that they're chronic soda pop abusers. Without naming specifically different brands or types of soda drinks, what we find is people who have a daily soda pop actually will start to also have fatty infiltrated livers and their thiamine levels can start to go down. Read Chris' full story below: "My name is Chris, I’m 39, and I’m an engineer from Fort Lauderdale, Florida. My substance of choice is alcohol. My post traumatic stress disorder in the last couple years blew up and I wasn’t managing those symptoms, and I was self-medicating with alcohol. I did a traditional, inpatient detox for ten days, and then a month and a half in a rehab facility, and it was all about AA meetings and I was with a population of people that I didn’t understand. I won’t say it was a waste of time, it just wasn’t for me… so when I left that program, I relapsed within two week pretty hard, and then for the next about 9 months, I was trying to wean myself off of alcohol and found myself about every weekend in the hospital for alcohol withdrawal symptoms. I was a mess. I was defeated, I was depressed, I was on 11 psychotropics and it was starting to take a toll on my job, take a toll on my relationship, I was so beat down. I wouldn’t say I was completely hopeless, because I knew there had to be something out there that would work. I searched the internet for alcohol detox programs that was outpatient based so I that could maintain my professional life and my life with my partner, my friendships, my hobbies, all of that, and not have to put everything on hold, not be in a bubble. New Beginnings’ approach is very different than other programs I tried, it’s very personalized in my opinion. They didn’t look at me as a template or as a default approach; they really listened to what didn’t work in the past, what my struggles were, what all of my symptoms were, not just the alcohol.. What were all of the symptoms that I was going through? And they developed a plan that worked for me. It was discussed that it would be best if not only detox from the alcohol, but also detox from the benzodiazepines that I was taking, but to also add the Restore program. But my neurotransmitters were so screwed up from being on so many psychotropics and just my general symptoms, that the idea of feeding the building blocks to my neurotransmitters so they could start operating a little bit more healthy… I was going to give it a shot. As I progressed through the Restore, I noticed I was feeling hyper. Really, really hyper. So we started to decrease my antidepressants, and then we decreased it again, and then we decreased it again. And then we started tapering me off of all my other medications that I was taking. So I went from 11 psychotropics down to one. One antidepressant, anti-anxiety medication… Out of 11. That’s amazing! I feel really focused, I feel really happy, my concentration level is laser-beam perfect, especially at work where I do a lot of detail-oriented tasks, I just feel so much more successful at my job, my outlook is really positive, I feel really hopeful, and I definitely think the Restore is a large part of that. There’s no way that I would have been able to taper off of so many medications so quickly and still feel this fantastic about my life. There’s no way, there’s no way it could be that way. I just, come here, for a little bit of time, a few days a week, I get the help I need, and I’m successful. I’m going on three months of sobriety… That’s the longest period of sobriety I’ve had in two years. When it occurs, it is often three days into the withdrawal symptoms and lasts for two to three days. People may also see or hear things other people do not.[1] Physical effects may include shaking, shivering, irregular heart rate, and sweating.Occasionally, a very high body temperature or seizures may result in death. Alcohol is one of the most dangerous drugs to experience withdrawal from. Delirium tremens typically only occurs in people with a high intake of alcohol for more than a month.[4] A similar syndrome may occur with benzodiazepine and barbiturate withdrawal.[5] Withdrawal from stimulants such as cocaine does not have major medical complications. In a person with delirium tremens it is important to rule out other associated problems such as electrolyte abnormalities, pancreatitis, and alcoholic hepatitis. Nicknames include barrel-fever, blue horrors, bottleache, bats, drunken horrors, elephants, gallon distemper, quart mania, pink spiders, among others. The main symptoms of delirium tremens are nightmares, agitation, global confusion, disorientation, visual and[9] auditory hallucinations, tactile hallucinations, fever, high blood pressure, heavy sweating, and other signs of autonomic hyperactivity (fast heart rate and high blood pressure). We often hear this being talked about in the forums and wherever there are people looking for information about how to quit drinking, or about alcoholism in general. They want to know information about this so I’m going to do this video. What is alcoholic wet brain? They are basically a combination of two different disorders. The first one is Wernicke’s encephalopathy and the second one is Korsakoff’s psychosis. These two can occur together or separately. They result from a deficiency of vitamin B1, Thiamine. A lot of the time, people who drink a lot can be short on this. The more alcohol you drink, the more your body will get to a stage where it can’t absorb the vitamin B1 so you can be deficient in that way. The more alcohol you put into your body, the less likely you are to give a shit about your health, give a shit about what you’re putting into your mouth. I’m speaking from experience here. All the drinkers I’ve ever known who go out and get plastered, the last thing they think about is ‘’will I have to cook a healthy meal?’’ Maybe the next day or the day after when their hangovers wore off but not in the moment and certainly not while they’re drinking. It’s always convenience over quality. That’s where it comes from, basically. The deficiency of vitamin B1. Either you’re not getting it anymore into your diet or your body is not capable of absorbing Thiamine. They call it alcoholic wet brain but in reality, you don’t have to be a drinker to get this. It’s not exclusive for heavy drinkers. People who are just not taking it enough in their diet can get this. It’s a long-term condition but it happens very quickly. This is a brain condition. Once you get wet brain syndrome, regardless of what it’s caused by, it’s irreversible. You can’t do anything about it. It’s part of what’s known as alcoholic dementia. They reckon that between 10 and 24% of all dementias are caused by alcohol. That’s another figure that is staggering, and yet we still don’t call this fucking thing a drug. 'Oh no it’s just a beverage. It’s only if you keep drinking loads of it that you do any damage', that’s what the experts say. The experts are quick to point out that -if I say it’s a poison, it’s a toxin for your system, they’ll say ‘so is war! War is toxic for your system. If you drink too much war, you can kill yourself’. And you can as well. But that’s not the fucking point. I’ve tried this before, I’ve gone out with my friends when I stopped drinking. I stopped drinking and I carried on going to the pub, and decided that all I was going to do all night was drink water. Apart from the boredom factor – I was bored out of my brain by not drinking, by the belch that was coming out of my friends’ gobs when they were drunk-, that wasn’t very sensible to me, but I tried to stick it out. But I couldn’t drink the water. These lads were downing pint after pint, which is exactly what I did when I was still drinking, and I could maybe get through a pint of water for every 4 of their pints. I just couldn’t get it down me. So, to say if you drink a lot of water, if you drink too much water then you can get poisoned by it, that’s true but you have to drink a lot of water and it’s very difficult to do that. Water is a beverage. Beer, wine, spirits, alcohol - that’s a drug. The reason you can drink so much of this stuff is because a) it can act as a diuretic so it makes you want to piss more. It turns off that survival mechanism inside your body to retain water as long as you can; and b) you need more of the drug as you go along. So when you start drinking alcohol, you can only drink 2 or 3 pints, and it makes you vomit. It makes the whole world go spinning around. Drink 3 pints of water and it makes you hydrated. It’s something good for you. Drink 3 pints of even weak beer when you’re not used to it, and it will make you sick, drunk and make you have the same symptoms as if you’re having a stroke. Glutathione Detox: The Science of Glutathione and Alcohol – Thomas DeLauer… Your body can have its own glutathione detox! Glutathione and Alcohol is a huge question, how does it actually work? More at http://www.thomasdelauer.com What is Glutathione? Glutathione (GSH) is a peptide made up of three amino acids - glutamic acid, cysteine and glycine. GSH is produced by every cell in our bodies. It is an antioxidant that works within cells. Aerobic cells in our body require oxygen. During the cellular processes that occur, ROS (reactive oxygen species) occur. When cells are injured even more ROS will be produced. ROS are not always bad. They are necessary for cell survival, but when there are too many produced it can lead to cell damage and death. When the levels of ROS raise, our bodies work to balance this out and regain homeostasis through the production of antioxidants, such as GSH. GSH: -Protects against oxidative damage and environmental toxins -Disposes peroxides (natural bleaching agents that cause harm in the body) -Participates in metabolic processes -Maintains intracellular health -Boosts immune system Due to its properties, GSH: -Protects brain health -Protects liver and other organs from damage -May protect against many other diseases due to cell damage, including Alzheimer’s, cancers and AIDs It has been found that most patients in the hospitals for conditions such as cancer and AIDs have depleted GSH levels. So what are the ways that we understand presently that GSH benefits our health? Brain Health: The cells of the brain make up 2% of our body weight while consuming 20% of the oxygen used in the body. The makes brain cells particularly at risk to oxidative damage. GSH is important in the detoxification processes of reactive oxygen species in the brain. Under normal conditions GSH synthesis is determined by cysteine availability and the activity of an enzyme called GCL (glutamate-cysteine ligase). Cysteine comes from the diet, protein breakdown and the liver. The amount of GSH present in the brain is dependent upon how much of the precursors are present. Different kinds of brain cells require different precursors. Liver: Helps the liver remove chemicals, such as drugs and environmental toxins. Glutathione is produced by the liver in response to alcohol consumption. When alcohol comes into contact with enzymes in your mouth, Acetaldehyde forms. Acetaldehyde is extremely toxic. When the remainder of the alcohol reaches your liver, it is processed into Acetaldehyde and your liver produces glutathione, which attacks the acetaldehyde to form a non-toxic substance similar to vinegar. Cancer: Oxidative stress can lead to cancer, and GSH can help protect against oxidative stress. This is how GSH can help to protect your body from cancer. There is an instance where medical professionals may desire to reduce the GSH in your cells, and that is when fighting cancer cells with chemotherapy. High GSH levels in tumor cells can make them difficult to fight. There is other evidence that GSH supplementation may help to reduce chemotherapy side effects and thus make it more effective, so this is currently under debate. How to Get Enough GSH: While studies have shown that elevated GSH levels can benefit health, exactly how to boost these levels is still debated. Supplements have not been proven to be an effective way to boost cellular GSH. Intravenous GSH was shown in one study to boost the survival rates and decrease the side effects from chemotherapy in women with ovarian cancer. It is likely that taking precursor supplements of cysteine may boost GSH levels. The presence of this signal leads to increased breakdown of glucose (Glycolysis), decreased creation of glucose (Gluconeogenesis), and decreased fat breakdown (Fat Oxidation). NADH is also used to convert Pyruvate to Lactate, which means high levels of NADH leads to increased Anaerobic Glycolysis and Lactic Acidosis. Mild withdrawal may present as Agitation or aggression a couple days after being brought into the hospital. Severe withdrawal can include Tremors, Seizures, confusion, and psychosis. This collection of symptoms is referred to as Delirium Tremens (DTs). Treatment for DTs is Benzos. The best type of treatment for alcoholism is a 12 step program like Alcoholics Anonymous (AA). In some patients, Disulfiram is also used. This drug Inhibits Acetaldehyde Dehydrogenase and makes patients very sick if they drink any alcohol as Acetaldehyde builds up much faster. You are essentially giving them a really bad hangover on purpose to dissuade them from drinking. However, this it is not always effective as there is relatively low compliance for this drug. Patients considering drinking can think ahead and easily not take their medication to avoid the consequences. Disulfiram intentionally causes these hangover like symptoms after alcohol consumption, but other drugs (such as Metronidazole) inadvertently have this same effect. Drugs with this type of side effect are often described as having a Disulfiram-Like effect. Methanol is in antifreeze, paint thinner and improperly prepared moonshine (illegally produced spirits). It can accidentally be consumed by children or adults mistaking the liquid for ethanol. Methanol itself does not cause problems, but after being metabolized by Alcohol Dehydrogenase Formaldehyde is formed. Formaldehyde can cause blindness and death so treatment focuses on preventing aldehyde formation. Fomepizole Inhibits Alcohol Dehydrogenase decreasing the amount of methanol that is metabolized into Formaldehyde. Alcohol can also be given as it utilizes the same enzyme and will compete for alcohol dehydrogenase. Nonalcoholic fatty liver disease (NAFLD) is a term used for a wide range of liver conditions that can affect people who drink very little to no alcohol. Fatty liver can also occur from excess alcohol consumption, but here we will concentrate on NAFLD. NAFLD occurs when an excess buildup of fat in the liver occurs, known as steatosis. Once fat exceeds 5-10% of the liver’s weight, the liver is considered to be a fatty liver. This occurs when the rate of de novo fatty acid synthesis and hepatic fatty acid uptake from plasma is greater than the rate of fatty acid oxidation and export - ie, too much fat is being created and not enough is being metabolized. Liver inflammation is characteristic is nonalcoholic steatohepatitis, one of the more advanced forms of the disease, that can eventually lead to scarring. This damage to the liver is similar to that caused by heavy alcohol use and can progress to liver cancer and cirrhosis (3). Causes: The exact cause isn’t known, however NAFLD is most common among those in their 40s and 50s who have metabolic syndrome, a cluster of risk factors that raise your chances of a wide range of diseases, including cardiovascular disease and type-2 diabetes. (1) Metabolic syndrome includes the following risk factors (2): Midsection fat accumulation High triglyceride level Low HDL cholesterol level High blood pressure High fasting blood sugar levels The majority of these risk factors are tied to obesity, so helping to lose weight and maintain a healthy weight is crucial. What Can You do to Combat Fatty Liver Disease? Currently there are no definitive treatments for fatty liver disease, so treatment is focused on lifestyle changes (3). 1. Cut out drinking 2. Cut out the fruit! Fructose is absorbed via portal vein and delivered to the liver in much higher concentrations as compared to other tissues. Fructose increases protein levels of all DNL enzymes during its conversion into triglycerides. Additionally, fructose supports lipogenesis in the setting of insulin resistance as fructose does not require insulin for its metabolism, and it directly stimulates SREBP1c, a major transcriptional regulator of DNL De-Novo lipogenesis 3. Boost Glutathione Levels- Glutathione is needed in order to recover from a fatty liver. Eat things like eggs, cabbage, broccoli and garlic as they have a higher sulfur content which can help convert into Acetyl Cysteine and create more glutathione in the body. 4. Turmeric- Turmeric is a spice whose use as medicine date back thousands of years in Ayurvedic medicine. Curcuminoids are the compounds in turmeric that give the distinct yellow color and are largely credited with the impressive therapeutic benefits of turmeric. Turmeric is both a powerful anti-inflammatory and antioxidant spice, and it is likely these aspects of turmeric that make it especially helpful for those with NAFLD. Study (7) A randomized double-blind placebo controlled trial took patients suffering from NAFLD and broke them into two groups. One group took 70-mg curcumin once daily for 8 weeks while the other group did the same with a placebo. Multiple measures were taken at baseline and then following the 8 weeks of treatment. It was found that those in the curcumin group experienced a significant reduction in liver fat content when compared to the placebo group - on average the curcumin participants experienced a 78.9% improvement in liver fat content! Additional significant reductions occurred in body mass index, serum total cholesterol levels, LDL cholesterol, triglycerides, and blood glucose when compared to the placebo group. References: 1.
