My video is about the treatment of alcoholism with niacin. This is the first video in a series I'm doing called "Nutrition & Alcoholism" where I focus on different nutritional steps that helped me cure my addiction and hopefully it can help others. I was an alcoholic for years, but I changed when I began seeing alcoholism from a nutritional viewpoint. It took years of research--of trial and error--but I cured myself of this addiction through taking the right supplements and focusing on healing my entire body through nutrition. Alcoholism is a poison on every single system of the human body. It effects the liver, the kidneys, the pancreas, the skin, the brain--so since it's creates such an onslaught on the body it only makes sense that the only way you can cure it is by healing the entire body. One of the first things that gave me hope that I could change was niacin. Niacin exerts a number of powerful benefits on the body and mind all of which can be beneficial to alcoholics (and anyone). It's an anxyolitc (meaning that it relieves anxiety). It works on the benzodiazepine receptors (the same ones that Valium and Xanax work on). It also helps relieve depression and it aids in detox just to name a few. So many people are on some sort of medication for depression these days and I started thinking about it--what's with the explosion of depression in this country? Mental health issues didn't seem to be so ubiquitous a hundred years ago. And I think I came up with an answer (it's not a shock that it all comes down to money). Niacin is added to food (bread, grains, etc) because as a population we don't readily get niacin from our modern American diet. However, there's two forms of niacin--nicotinic acid and niacinamide--and niacinamide is the form that's added to food because it's cheaper. Niacin is critical for the formation of serotonin, the feel good neurotransmitter. It goes like this--the amino acid tryptophan is used to make niacin and serotonin and niacin production gets priority over serotonin. When the body senses that there isn't enough niacin it will take the raw materials it would use to make serotonin to keep making niacin. Here's the rub: niacinamide doesn't shut off the niacin pathway like the more expensive nicotinic acid, so the body constantly thinks that there isn't enough niacin and it redirects its supply of tryptophan from serotonin production to keep making niacin. The less expensive niacinamide keeps the population from developing the more obvious diseases associated with niacin deficiency like pellagra, but at a cost to mental health--leading to a whole host of problems ranging from depression to schizophrenia to alcohol dependency. Hi, I'm Dr. Jared Nielsen. I'd like to talk today about the importance of a vitamin called thiamine or B1. B1 is very, very important for liver function, especially for people who have a history of alcoholism. It's quickly depleted in the system and is essential for that person to be repleted, if you will, their B1 vitamin. There are a lot of people that don't think, "I am really an alcoholic or I don't really have a drinking problem." What's interesting is they'll start to manifest some of the symptomatology of B1 deficiency, peripheral neuropathies, sensitivity or hyper‑sensitivity in the legs and feet. These are very commonly overlapped with diabetics. Diabetics also would get that type of symptomatology as the liver becomes congested or a fatty infiltrated liver. When we look at blood labs, a lot patients will actually say, when we see cholesterol levels going up and triglyceride levels matching or even exceeding that they don't drink alcohol. What we see a lot of times is that they're chronic soda pop abusers. Without naming specifically different brands or types of soda drinks, what we find is people who have a daily soda pop actually will start to also have fatty infiltrated livers and their thiamine levels can start to go down. 12 Ways Your Body Tries To Tell You That Your Liver Is Being Damaged The liver is the largest solid organ in your body. It is located in the right upper quadrant of your abdomen and partly protected by the ribs. You need your liver for many important bodily functions and cannot live without your liver. For this reason, you should know the early signs of liver damage and take care of your liver as much as you possibly can. There are more than a hundred different kinds of diseases of the liver. Each has its own set of symptoms. Some of these diseases include alcoholic liver disease, hepatitis, fatty liver disease, and cirrhosis of the liver. The early signs of liver disease tend to be easily overlooked and are non-specific, related to any number of different conditions. Here are some common early signs. 1. Upset Stomach You are likely to get nausea and vomiting. They can mimic other diseases, such as migraines, food poisoning, vertigo, motion sickness, early pregnancy, depression and vomiting. It can mimic kidney problems as well. Those who suffer from early liver damage often feel persistently nauseous, because the liver has a decreased ability to rid the body of toxins. The nausea can also be due to ongoing metabolism and changes in digestion. If you have these symptoms for several days and don’t know why, seek the attention of your doctor. 2. Fatigue and Overall Weakness One of the early and obvious signs of liver damage is constantly feeling tired and weak. When the liver is not in great condition, the body must work harder and you will need more rest. This is due to an increase in toxins in the bloodstream that can’t be cleared out because the liver can’t get rid of the toxins properly. If the symptoms persist, especially with other early signs of liver damage, seek medical advice as soon as you can. 3. Lack of Appetite This happens due to a lack of production of bile that helps digest fat in the diet. When food isn’t properly digested, it can lead to a lack of appetite and a severe loss of weight. See your doctor if you experience any of these symptoms for a long period of time. 4. Digestion Problems The liver is important in digestion because it makes bile that helps the nutrients be better processed and absorbed by the small intestines. If the liver is failing, you can develop an increase in indigestion and diarrhea. If the bile is not produced properly, you can also experience irritable bowel syndrome, fatty food intolerance, alcohol intolerance, abdominal bloating, constipation and gallstones. If this becomes severe, you need to seek the medical attention of a doctor. 5. Alterations in the Color of Urine If you think you are drinking enough but still have a dark color of your urine, this may be one of the early signs of liver damage. The urine may look brown, amber, or orange in color because of elevated levels of bilirubin in the bloodstream. The bilirubin is elevated because the liver cannot get rid of the bilirubin and it is excreted through the kidneys. Dark urine may also be due to dehydration, side effects of taking antibiotics, taking B vitamin supplements, urinary tract infections, enzyme deficiencies or kidney abnormalities. The doctor can help sort out the cause of dark urine. 6. Changes in Stool Coloration Because the bile is not made enough by a malfunctioning liver, the color of the stool changes. When it occurs, it is often three days into the withdrawal symptoms and lasts for two to three days. People may also see or hear things other people do not.[1] Physical effects may include shaking, shivering, irregular heart rate, and sweating.Occasionally, a very high body temperature or seizures may result in death. Alcohol is one of the most dangerous drugs to experience withdrawal from. Delirium tremens typically only occurs in people with a high intake of alcohol for more than a month.[4] A similar syndrome may occur with benzodiazepine and barbiturate withdrawal.[5] Withdrawal from stimulants such as cocaine does not have major medical complications. In a person with delirium tremens it is important to rule out other associated problems such as electrolyte abnormalities, pancreatitis, and alcoholic hepatitis. Prevention is by treating withdrawal symptoms. If delirium tremens occurs, aggressive treatment improves outcomes. Treatment in a quiet intensive care unit with sufficient light is often recommended. Benzodiazepines are the medication of choice with diazepam, lorazepam, chlordiazepoxide, and oxazepam all commonly used. They should be given until a person is lightly sleeping. The antipsychotic haloperidol may also be used. The vitamin thiamine is recommended.Mortality without treatment is between and 40%. Currently death occurs in about 1% to 4% of cases. Of these, three to five percent develop DTs or have seizures.The name delirium tremens was first used in 1813; however, the symptoms were well described since the 1700s.The word "delirium" is Latin for "going off the furrow," a plowing metaphor. Then you recall the past two occasions when you met friends for “a” drink, and ended up having ten. If drinking has become an unhealthy habit, causing you to resemble Mike Tyson after his ear-chewing moment, rather than a giggly drunk just passing time with her friends, then it’s time to cut yourself off. Here’s how: 1. Drink water and juice instead of booze when you go out. Everybody wants to fit in socially, and drinking is a big part of that scene. You can still “drink” with your friends. Offer to be the designated driver, or tell them you need to get up really early. Lie if you have to, but drink something besides an alcoholic beverage when you go out. 2. Alcohol consumption inhibits your absorption of B-vitamins. These vitamins are responsible for just about every function in your body, but also combat anxiety and depression. Ironically, the more you drink, the more you’ll likely feel like drinking to ease your anxiousness because you’ve depleted this vital nutrient. Go to a play, comedy show, concert, or suggest doing something really crazy like going for a hike or volunteering at a local food shelter. This process is mostly done for those who have been drinking alcohol for a long time, those who have been heavy drinkers in their final days of drinking, and those who are stopping after a dependence has formed who have health concerns. The most common medication for alcohol detoxification is chlordiazepoxide. Detoxification can be done in a number of ways. One way of detoxing from alcohol is by a doctor prescribing medication during the first days of quitting drinking, which are the days when the symptoms of withdrawal will be very intense. This medication will act as means to wean off the alcohol. The dosage will be reduced gradually until the individual's body has adapted to the changes and the signs of withdrawal have faded away. You must obviously avoid taking alcohol during the period you will be going through this process. This is a difficult period in the life of the alcoholic, more so due to the effects of withdrawal, and also because one may be easily tempted to go back to their drinking habit. Support from friends and relatives will be very important during this time. In some cases, it is a friend or a member of the family who will be administering the medication to the alcoholic. GSH is produced by every cell in our bodies. It is an antioxidant that works within cells. Aerobic cells in our body require oxygen. During the cellular processes that occur, ROS (reactive oxygen species) occur. When cells are injured even more ROS will be produced. ROS are not always bad. They are necessary for cell survival, but when there are too many produced it can lead to cell damage and death. When the levels of ROS raise, our bodies work to balance this out and regain homeostasis through the production of antioxidants, such as GSH. GSH: -Protects against oxidative damage and environmental toxins -Disposes peroxides (natural bleaching agents that cause harm in the body) -Participates in metabolic processes -Maintains intracellular health -Boosts immune system Due to its properties, GSH: -Protects brain health -Protects liver and other organs from damage -May protect against many other diseases due to cell damage, including Alzheimer’s, cancers and AIDs It has been found that most patients in the hospitals for conditions such as cancer and AIDs have depleted GSH levels. So what are the ways that we understand presently that GSH benefits our health? Brain Health: The cells of the brain make up 2% of our body weight while consuming 20% of the oxygen used in the body. The makes brain cells particularly at risk to oxidative damage. GSH is important in the detoxification processes of reactive oxygen species in the brain. Under normal conditions GSH synthesis is determined by cysteine availability and the activity of an enzyme called GCL (glutamate-cysteine ligase). Cysteine comes from the diet, protein breakdown and the liver. The amount of GSH present in the brain is dependent upon how much of the precursors are present. Different kinds of brain cells require different precursors. Liver: Helps the liver remove chemicals, such as drugs and environmental toxins. Glutathione is produced by the liver in response to alcohol consumption. When alcohol comes into contact with enzymes in your mouth, Acetaldehyde forms. Acetaldehyde is extremely toxic. When the remainder of the alcohol reaches your liver, it is processed into Acetaldehyde and your liver produces glutathione, which attacks the acetaldehyde to form a non-toxic substance similar to vinegar. Cancer: Oxidative stress can lead to cancer, and GSH can help protect against oxidative stress. This is how GSH can help to protect your body from cancer. There is an instance where medical professionals may desire to reduce the GSH in your cells, and that is when fighting cancer cells with chemotherapy. High GSH levels in tumor cells can make them difficult to fight. There is other evidence that GSH supplementation may help to reduce chemotherapy side effects and thus make it more effective, so this is currently under debate. How to Get Enough GSH: While studies have shown that elevated GSH levels can benefit health, exactly how to boost these levels is still debated. Supplements have not been proven to be an effective way to boost cellular GSH. Intravenous GSH was shown in one study to boost the survival rates and decrease the side effects from chemotherapy in women with ovarian cancer. When it occurs, it is often three days into the withdrawal symptoms and lasts for two to three days. People may also see or hear things other people do not.[1] Physical effects may include shaking, shivering, irregular heart rate, and sweating.Occasionally, a very high body temperature or seizures may result in death. Alcohol is one of the most dangerous drugs to experience withdrawal from. Delirium tremens typically only occurs in people with a high intake of alcohol for more than a month.[4] A similar syndrome may occur with benzodiazepine and barbiturate withdrawal.[5] Withdrawal from stimulants such as cocaine does not have major medical complications. In a person with delirium tremens it is important to rule out other associated problems such as electrolyte abnormalities, pancreatitis, and alcoholic hepatitis. Prevention is by treating withdrawal symptoms. If delirium tremens occurs, aggressive treatment improves outcomes. Treatment in a quiet intensive care unit with sufficient light is often recommended. Benzodiazepines are the medication of choice with diazepam, lorazepam, chlordiazepoxide, and oxazepam all commonly used. They should be given until a person is lightly sleeping. The antipsychotic haloperidol may also be used. The vitamin thiamine is recommended.Mortality without treatment is between and 40%. Currently death occurs in about 1% to 4% of cases. About half of people with alcoholism will develop withdrawal symptoms upon reducing their use. Of these, three to five percent develop DTs or have seizures.The name delirium tremens was first used in 1813; however, the symptoms were well described since the 1700s.The word "delirium" is Latin for "going off the furrow," a plowing metaphor. It is also called shaking frenzy and Saunders-Sutton syndrome. Nicknames include barrel-fever, blue horrors, bottleache, bats, drunken horrors, elephants, gallon distemper, quart mania, pink spiders, among others. The main symptoms of delirium tremens are nightmares, agitation, global confusion, disorientation, visual and[9] auditory hallucinations, tactile hallucinations, fever, high blood pressure, heavy sweating, and other signs of autonomic hyperactivity (fast heart rate and high blood pressure). These symptoms may appear suddenly, but typically develop two to three days after the stopping of heavy drinking, being worst on the fourth or fifth day.[10] Also, these "symptoms are characteristically worse at night".[11] In general, DT is considered the most severe manifestation of alcohol withdrawal and occurs 3–10 days following the last drink.[9] Other common symptoms include intense perceptual disturbance such as visions of insects, snakes, or rats. These may be hallucinations, or illusions related to the environment, e.g., patterns on the wallpaper or in the peripheral vision that the patient falsely perceives as a resemblance to the morphology of an insect, and are also associated with tactile hallucinations such as sensations of something crawling on the subject—a phenomenon known as formication. Delirium tremens usually includes extremely intense feelings of "impending doom". Severe anxiety and feelings of imminent death are common DT symptoms. We often hear this being talked about in the forums and wherever there are people looking for information about how to quit drinking, or about alcoholism in general. They want to know information about this so I’m going to do this video. What is alcoholic wet brain? They are basically a combination of two different disorders. The first one is Wernicke’s encephalopathy and the second one is Korsakoff’s psychosis. These two can occur together or separately. They result from a deficiency of vitamin B1, Thiamine. A lot of the time, people who drink a lot can be short on this. The more alcohol you drink, the more your body will get to a stage where it can’t absorb the vitamin B1 so you can be deficient in that way. The more alcohol you put into your body, the less likely you are to give a shit about your health, give a shit about what you’re putting into your mouth. I’m speaking from experience here. All the drinkers I’ve ever known who go out and get plastered, the last thing they think about is ‘’will I have to cook a healthy meal?’’ Maybe the next day or the day after when their hangovers wore off but not in the moment and certainly not while they’re drinking. It’s always convenience over quality. That’s where it comes from, basically. The deficiency of vitamin B1. Either you’re not getting it anymore into your diet or your body is not capable of absorbing Thiamine. They call it alcoholic wet brain but in reality, you don’t have to be a drinker to get this. It’s not exclusive for heavy drinkers. People who are just not taking it enough in their diet can get this. It’s a long-term condition but it happens very quickly. This is a brain condition. Once you get wet brain syndrome, regardless of what it’s caused by, it’s irreversible. You can’t do anything about it. It’s part of what’s known as alcoholic dementia. They reckon that between 10 and 24% of all dementias are caused by alcohol. That’s another figure that is staggering, and yet we still don’t call this fucking thing a drug. 'Oh no it’s just a beverage. It’s only if you keep drinking loads of it that you do any damage', that’s what the experts say. The experts are quick to point out that -if I say it’s a poison, it’s a toxin for your system, they’ll say ‘so is war! War is toxic for your system. If you drink too much war, you can kill yourself’. And you can as well. But that’s not the fucking point. I’ve tried this before, I’ve gone out with my friends when I stopped drinking. I stopped drinking and I carried on going to the pub, and decided that all I was going to do all night was drink water. Apart from the boredom factor – I was bored out of my brain by not drinking, by the belch that was coming out of my friends’ gobs when they were drunk-, that wasn’t very sensible to me, but I tried to stick it out. But I couldn’t drink the water. These lads were downing pint after pint, which is exactly what I did when I was still drinking, and I could maybe get through a pint of water for every 4 of their pints. I just couldn’t get it down me. So, to say if you drink a lot of water, if you drink too much water then you can get poisoned by it, that’s true but you have to drink a lot of water and it’s very difficult to do that. Water is a beverage. Beer, wine, spirits, alcohol - that’s a drug. The reason you can drink so much of this stuff is because a) it can act as a diuretic so it makes you want to piss more. It turns off that survival mechanism inside your body to retain water as long as you can; and b) you need more of the drug as you go along. So when you start drinking alcohol, you can only drink 2 or 3 pints, and it makes you vomit. It makes the whole world go spinning around. Drink 3 pints of water and it makes you hydrated. It’s something good for you. Drink 3 pints of even weak beer when you’re not used to it, and it will make you sick, drunk and make you have the same symptoms as if you’re having a stroke. What we call a hangover is not an actual medical condition, because a hangover is not one condition. A hangover is the result of several body processes occurring in response to drinking alcohol. In order to cure or even prevent a hangover, each one of these body processes must be addressed accordingly. Let’s run through each of these body processes: Step One: Dehydration For a very long time, this was thought to be the sole cause of a hangover. Alcohol is much harder for the body to process than regular food and drink, therefore your demand for water greatly increases on a “night out.” To make matters worse, ingesting alcohol has detrimental effects on your hypothalamus, which is the part of your brain responsible for the regulation of the hormone vasopressin. When vasopressin goes off the hinges, you urinate more frequently, diluting your concentration of electrolytes causing even more dehydration. Step Two: Toxic Waste Production Alcohol metabolism has not been heavily studied, but here’s what we do know: Alcohol metabolism in your liver produces toxic by-products called acetaldehydes. Your body has natural acetaldehyde dehydrogenase and glutathione stores that turn these toxic substances into acetate, which is thought to be harmless at low levels. When your glutathione storage runs out, this leaves the toxic substances in your body, which have been shown to cause flushing, sweating and vomiting, much like when any other toxic substance enters your body. Step Three: Immune System Failure The consumption of alcohol also puts a heavy load on your immune system. Studies show that there are significant changes in immune system parameters during a hangover, namely increased concentrations of cytokines, which are released during an inflammatory response. It is likely that these cytokines are the culprit behind many cognitive effects of alcohol, such as memory impairment and mood changes. Cerebral cytokines have also been shown in animal studies to increase sickness behavior such as weakness, inability to concentrate and reduced appetite. (Sound familiar?) Step Four: The Glutamine Rebound The next step happens after you have already gone to sleep. The cause of nausea and vomiting is often attributed only to alcohol quantity, but have you ever slept so poorly you felt sick? Vomiting while drinking is a result of your body rejecting the pseudo-poison that you just ingested, but once it’s all processed, much of your hangover nausea is actually from poor sleep quality. This is due to the Glutamine Rebound. When someone is drinking, alcohol inhibits one of the body’s natural stimulants – glutamine. When the drinking stops, the body will immediately begin producing more glutmine to catch up. This “Glutamine Rebound” almost always happens while you are asleep, stimulating the body and keeping you from reaching your deepest sleep all night. The resulting daytime sleepiness is what accounts for any sensitivity to light and general weakness you may feel in the morning. You must attack each of these pain points individually to prevent or cure a hangover. Many hangover cures try to treat a hangover as one entity, and often lean on dehydration as the root cause. Alcohol Alcohol absorbs into a humans blood, principally from our small intestine. It accumulates in the blood because absorption is more rapid than oxidation and elimination. Depression of the central nervous system is a principle effect of alcohol. The CNS is at first stimulated. This is a defensive response by the body to begin action to eliminate this harmful poison. Due to the enervation that invariably follows, exhaustion and depression follow. People who drink large amounts of alcohol repetitively become somewhat “tolerant” to its effects; later doses seem not to have the same intoxicating effect as earlier ones. This tolerance is not based primarily on changes in drug dispositions or metabolism but is caused by destructive changes in central nervous system cells. Those said to be tolerant to alcohol may have incredibly high blood alcohol concentrations. In this case, the body has become so enervated that it has lost its capacity to react to this drug and eliminate its poisons from the system. The so-called “physical dependence” accompanying tolerance is profound, and withdrawal produces a series of pronounced physical and mental effects. While these symptoms may be severe, the acute period usually only lasts about two days if the alcoholic fasts, but may last as long as ten days under orthodox treatment. Delirium tremens, experienced by many alcoholics, usually ceases after two days on the fast. Since alcohol results in central nervous system stimulation I and its eventual depression, it invariably has similar effects on all bodily parts connected with the CNS and this includes everything. Especially effected is the brain and this is demonstrated in the alcoholic’s lack of memory, disorientation, slurred speech, etc. Also, there is a marked prevalence of vitamin B deficiency, especially thiamine, due partly to the poor diet of the alcoholic and partly due to malabsorption of this vitamin by the impaired gastrointestinal tract of such individuals. Many so-called vitamin deficiencies have been corrected during the fast while no food was being ingested. The body normalizes while on the fast and certain food stored vitamins are utilized. On the Hygienic diet of raw fruits, vegetables and nuts, no thiamine deficiency could be possible because all of these foods contain this vitamin in abundance. Thus, the alcoholic is very greatly benefited by this raw diet since his body will be provided the most ideal conditions for normalcy of function to return. Not only will all nutrients be available for utilization but the proper conditions will be provided for healing. Usual orthodox treatment includes large doses of vitamin C and B-complex vitamins, particularly thiamine. These inorganic vitamins will not provide any benefit to the alcoholic since the body cannot utilize nutrients in this form. The result will be added toxins and additional problems for the body to deal with. In addition, alcoholics are often given fluids if they are thought to be dehydrated. The usual therapy is 1,000 ml of 5% dextrose in a saline solution followed by 1,000 ml of 10% dextrose in distilled water. This practice is totally anti-life and will result in more stimulation if the body is able to respond defensively. In any case, it is harmful practice. Drugs are used frequently to treat the alcohol withdrawal symptoms. But trading one drug for another never results in health. It only results in additional sickness and destruction of tissues and cells. Another treatment given to alcoholics is the administration of Disulfiram. This is a drug that interferes with metabolism of acetaldehyde (an intermediary product in the oxidation of alcohol) so that acetaldehyde accumulates, producing toxic symptoms and great discomfort. Metallothionein is very important to our long term health and survival. Nutrition begins in the kitchen, in the cooking pot. It begins with whole foods that you consume in as whole a form as possible. GGOBE is an acronym he uses for top sulfur-rich foods. It stands for garlic, ginger, onions, brassica sprouts and eggs. 16:55 Whey Protein: Whey protein is high in cysteine. 19:22 Metallothionein and Glutathione: The purpose of both of these is to protect us. Glutathione is synthesized inside of cells. Oral glutathione may help the gut and the mucosal barrier, but it does not go to your body’s cells. Glutathione is an important intermediary in keeping oxidative stress low. If we give the precursor amino acids for glutathione production in cells, GGOBE foods or supplements that contain cysteine, methionine and we have a healthy alkaline cell with mitochondria are healthy, we can make glutathione. 21:58 Mitochondria and Cell Health:The cell protects the mitochondria and the mitochondria give the cell ATP. The mitochondria also detoxify things like pesticides, persisting pollutants and hormone disruptors. These must be detoxified inside the mitochondria. If the mitochondria are overwhelmed, then the energetics of the cell slows and uses up Coenzyme Q10, which is the electron shuttle, impairing things further. About 66% of our detoxification happens in the liver. Maybe 15% of detox happens in the kidneys. A small percentage takes place in the spleen. Some also happens in our skin. Every muscle, every cell of the body does some detoxification and utilizes mitochondria to do that. 24:50 Ascorbate and Raising Glutathione: Unlike vitamins, ascorbate does not activate catalysts. It is used directly. It is a sub straight. It is used up in the course of detoxifying, energizing and protecting the other antioxidants in the body. Ascorbate is the correct name. It is the mother protective molecule for all other protective molecules in the body. The best way to raise glutathione in the cell is to give ascorbate. The amount of glutathione that you generate is proportional to having enough ascorbate. 27:42 C-Cleanse: Quickly saturate the body with mineral ascorbate, so you have calcium, magnesium, potassium and zinc minerals along with the ascorbate. When alcohol is consumed in large quantities Acetaldehyde, an intermediate of alcohol metabolism, builds up faster than it can be metabolized. This excess Acetaldehyde contributes to hangover symptoms. When alcohol is consumed in excess the NADH generated from alcohol metabolism can also cause many health effects. This buildup of NADH can be exacerbated when large quantities of alcohol are consumed without food being consumed. NADH is an electron transporter. Its presence signals liver cells that there is ample energy present. The presence of this signal leads to increased breakdown of glucose (Glycolysis), decreased creation of glucose (Gluconeogenesis), and decreased fat breakdown (Fat Oxidation). NADH is also used to convert Pyruvate to Lactate, which means high levels of NADH leads to increased Anaerobic Glycolysis and Lactic Acidosis. Alcoholism can also cause Traumatic Injuries Due to Falls or Esophageal Tears due to frequent vomiting. Alcoholism is also associated with many health outcomes which I will cover in other sections. Wernicke-Korsakoff, thiamine deficiency, is common among alcoholics, because alcoholics sometimes get a majority of their calories from alcohol. Excessive alcohol intake is also associated with Pancreatitis. Chronic steatosis can lead to liver inflammation ("non-viral" Hepatitis) and eventually Liver Cirrhosis. Unlike viral hepatitis, inflammation due to alcohol usually presents with an AST Elevated More than ALT. I remember this by thinking Alcohol --) Steatosis --) Higher AST. Excessive alcohol consumption by pregnant woman can also lead to Fetal Alcohol Syndrome and mental retardation. Alcohol Withdrawal must be watched carefully in any hospitalized patients with a history of alcoholism as it can be life threatening. It usually occurs a couple days after abrupt cessation of alcohol (usually in the hospital where alcohol is not accessible). Mild withdrawal may present as Agitation or aggression a couple days after being brought into the hospital. Severe withdrawal can include Tremors, Seizures, confusion, and psychosis. This collection of symptoms is referred to as Delirium Tremens (DTs). Treatment for DTs is Benzos. The best type of treatment for alcoholism is a 12 step program like Alcoholics Anonymous (AA). In some patients, Disulfiram is also used. This drug Inhibits Acetaldehyde Dehydrogenase and makes patients very sick if they drink any alcohol as Acetaldehyde builds up much faster. You are essentially giving them a really bad hangover on purpose to dissuade them from drinking. However, this it is not always effective as there is relatively low compliance for this drug. Patients considering drinking can think ahead and easily not take their medication to avoid the consequences. Disulfiram intentionally causes these hangover like symptoms after alcohol consumption, but other drugs (such as Metronidazole) inadvertently have this same effect. Drugs with this type of side effect are often described as having a Disulfiram-Like effect. Methanol is in antifreeze, paint thinner and improperly prepared moonshine (illegally produced spirits). It can accidentally be consumed by children or adults mistaking the liquid for ethanol. Methanol itself does not cause problems, but after being metabolized by Alcohol Dehydrogenase Formaldehyde is formed. Formaldehyde can cause blindness and death so treatment focuses on preventing aldehyde formation. Fomepizole Inhibits Alcohol Dehydrogenase decreasing the amount of methanol that is metabolized into Formaldehyde. Alcohol can also be given as it utilizes the same enzyme and will compete for alcohol dehydrogenase.
