There’s car insurance, home insurance, life insurance, even pet insurance . . . Most of these insurance policies work well and are fairly priced. But there is one glaring exception: health insurance. Only health insurance becomes more complicated and more expensive at the same time. So, the obvious question is: why? To answer this question, we have to start at the beginning. What is insurance? It’s pretty straight-forward: You pay a monthly fee which provides financial protection against unforeseen, sometimes catastrophic, events. People buy homeowners insurance, for example, to protect themselves from the financial loss incurred in the event of a fire, a flood or theft. Because millions of people are paying into the insurance pool, the pool has enough money to cover the unlucky person whose house does burn down. And since insurance is meant to share risk, it only stands to reason that higher-risk individuals have to pay more to be insured. Someone who has had two accidents is going to pay more for car insurance than someone who has never had an accident. Why? Because their track record indicates they are more likely to have another accident. But while insurance provides a bulwark against unforeseen loss, it does not protect against routine expenses. Car insurance protects you in the event that you wind up in a car wreck or your vehicle is stolen, but it doesn’t cover routine maintenance like oil changes, replacing brake pads or tire erosion. Why? Because everyone needs routine oil changes, new brake pads, and new tires. So, there is no risk to protect against. Health insurance in America works very differently. Many of us have health insurance plans that aren’t insurance at all. They’re really pre-paid health care plans. They cover routine check-ups, less serious illnesses, and recurring expenses like prescription medications in addition to protecting you from a health disaster. All of this has made healthcare much more expensive and complex than any other form of insurance. That is true whether you get your insurance through your employer, through the government, or if you pay for your own plan. The Affordable Care Act, known as Obamacare, was passed on the promise that it would fix these issues and bring down healthcare costs. But it has actually made the problem much worse. First, it limited the variety of health insurance plans private companies could offer. It did this by mandating that every plan had to cover the same set of ten health benefits, including preventive care, maternity care, mental health care, and contraception. Second, Obamacare prevented insurers from charging premiums based on the risk they were assuming. A person with a much higher risk of getting sick couldn’t be charged more than a person with a much lower chance. These two aspects of Obamacare – requiring all policies to have certain coverages and not allowing insurance companies to charge more for riskier clients – caused the price of insurance to rise dramatically. When I consider purchasing an individual health insurance plan for myself or my family, do I have any financial obligations beyond the monthly premium and annual deductible? Answers: It depends on the plan, but some plans have the following cost-sharing elements that you should be aware of. Co-Payments: Some plans include a co-payment, which is typically a specific flat fee you pay for each medical service, such as $30 for an office visit. After the co-payment is made, the insurance company typically pays the remainder of the covered medical charges. Deductibles: Some plans include a deductible, which typically refers to the amount of money you must pay each year before your health insurance plan starts to pay for covered medical expenses. Coinsurance: Some plans include coinsurance. Coinsurance is a cost sharing requirement that makes you responsible for paying a certain percentage of any costs. The insurance company pays the remaining percentage of the covered medical expenses after your insurance deductible is met. Out-of-pocket limit: Some plans include an out-of-pocket limit. Typically, the out-of-pocket limit is the maximum amount you will pay out of your own pocket for covered medical expenses in a given year. The out-of-pocket limit typically includes deductibles and coinsurance. But, out-of-pocket limits don't typically apply to co-payments. Lifetime maximum: Most plans include a lifetime maximum. Typically the lifetime maximum is the amount your insurance plan will pay for covered medical expenses in the course of your lifetime. Exclusions & Limitations: Most health insurance carriers disclose exclusions & limitations of their plans. It is always a good idea to know what benefits are limited and which services are excluded on your plan. You will be obligated to pay for 100% of services that are excluded on your policy. Beginning September 23, 2010, the Patient Protection and Affordable Care Act (health care reform) begins to phase out annual dollar limits. Starting on September 23, 2012, annual limits on health insurance plans must be at least $2 million. By 2014 no new health insurance plan will be permitted to have an annual dollar limit on most covered benefits. Some health insurance plans purchased before March 23, 2010 have what is called "grandfathered status." Health Insurance Plans with Grandfathered status are exempt from several changes required by health care reform including this phase out of annual limits on health coverage. If you purchased your health insurance policy after March 23, 2010 and you're due for a routine preventive care screening like a mammogram or colonoscopy, you may be able to receive that preventive care screening without making a co-payment. You can talk to your insurer or your licensed eHealthInsurance agent if you need help determining whether or not you qualify for a screening without a co-payment. There are five important changes that occurred with individual and family health insurance policies on September 23, 2010. Those changes are: 1. Added protection from rate increases: Insurance companies will need to publically disclose any rate increases and provide justification before raising your monthly premiums. 2. Added protection from having insurance canceled: An insurance company cannot cancel your policy except in cases of intentional misrepresentations or fraud. 3. Coverage for preventive care: Certain recommended preventive services, immunizations, and screenings will be covered with no cost sharing requirement. 4. No lifetime maximums on health coverage: No lifetime limits on the dollar value of those health benefits deemed to be essential by the Department of Health and Human Services. 5. No pre-existing condition exclusions for children: If you have children under the age of 19 with pre-existing medical conditions, their application for health insurance cannot be declined due to a pre-existing medical condition. In some states a child may need to wait for the state's open-enrollment period before their application will be approved.

That's because of bargaining power. Government programs, like Medicare and Medicaid, can ask for a lower price from health service providers because they have the numbers: the hospital has to comply or else risk losing the business of millions of Americans. There are dozens of private health insurance providers in the United States and they each need to bargain for prices with hospitals and doctors. The numbers of people private insurances represent are much less than the government programs. That means a higher price when you go to the doctor or fill a prescription. Uninsured individuals have the least bargaining power. Without any insurance, you will pay the highest price. They're more worried about packing every other court in this country with right-wing political corporate hack judges. That is exactly what's been going on ever since the day that Donald Trump nominated Brett Kavanaugh to that vacant seat on the US Supreme Court. In the two months since Kavanaugh was nominated, 26 judges have been confirmed by the Senate to federal appeals court and district court judgeships. Every single one of these is a lifetime appointment. All of that happened, again, 26 of them, in the two months since Kavanaugh was nominated. Now, if you're watching this, I want you to ask yourself this question. How many times did you hear anybody in the corporate media talk about that? Do you know the names? Can you name one of those 26 judges that Republicans rammed through the Senate? Can you name the Democrats who struck a deal with Republicans two weeks ago to fast track 11 of these corporate political hack judges for lifetime appointments on our courts? That we probably can. Chuck Schumer led the charge on that one, working with Republicans to try to get as many of their judicial picks on the bench as possible before the midterms. Donald Trump inherited 108 judicial vacancies throughout the United States that have not been appointed during the Obama era. Now, we've talked at length as to why these were left vacant. Some were Obama's fault, some was obstruction fault. Overall, they were vacant. He has appointed close to 100 at this point, most of which have been confirmed, and yet now due to retirement and other factors he still has 126 left to confirm here in the United States. He has even more than he inherited. He has appointed more judges so far this year, the second year of his presidency, than Obama did in the second year of his presidency. By the end of this year, depending on how things go, possibly even by the end of next year, he'll have appointed more judges than Barack Obama, completely reshaping the judicial landscape here in the United States, and people don't care. They think the only thing that matters is the Supreme Court. You have to understand. I know it's the court of the land, the top of the top, but very few, a tiny fraction, of all cases in this country ever go there. Most judicial issues are figured out and determined and settled in federal appeals courts and district courts across the country. Republicans know that. Democrats and their voters don't seem to understand that at all. That's why Republicans are so obsessed with packing these courts with pro-corporate, anti-consumer judges. They know they can dangle a horrible Supreme Court justice in front of your face and you're gonna eat it up and follow it wherever it goes. While you're doing that, they're packing the courts with people who are gonna strip you of your civil rights, of your right to choose, of workplace protections, and of environmental protections.

*** THE JACK BOOT IS ONE OF MY TOP 5 FAVORITE BOOTS - SEE THE REST: https://www.youtube.com/watch?v=3Y7_Bu7XGB0 Taft is a direct to consumer footwear brand that’s based in the United States, but all of their boots are made in Europe with European leather, but the Jack boot — indubitably their flagship model — doesn’t contain that much leather. TAFT JACK BOOT OVERVIEW These are very, very distinctive boots. With a woolen upper and oxblood leather toecap and counter, they’re sometimes described as something like what the Great Gatsby would wear. Because it has a Blake stitch and rubber soles the shoe is very lightweight and flexible, with a svelte silhouette and fully calfskin lining. It’s such an unusual shoe that it’s not super versatile — that’s just a necessary byproduct of a memorable shoe — but if you can find the right outfit I think they look great. *** FULL REVIEW + PICTURES: http://stridewise.com/taft-jack-boot-review *** TAFT JACK BOOT UPPER Mostly raw Portuguese wool with oxblood leather on the toecap and counter. The leather is called crust leather, which means it’s dried after it’s been tanned but it hasn’t’ undergone additional finishing. You might call it semi-finished or maybe describe it as an intermediary stage in leather production; I just think they used crust leather so they could paint it themselves. And that’s an interesting aspect here: the leather is hand-painted. It’s also vegetable-tanned and chromium-free, so it’s better for the environment than a lot of chrome-tanned leathers out there which have really screwed up a lot of areas, particularly in India and Vietnam. *** SEE MY REVIEW OF TAFT’S HARDIER DRAGON BOOT HERE: https://www.youtube.com/watch?v=DWpBJLQR7Co *** TAFT JACK BOOT LEATHER CARE This leather is not particularly hardy and it scrapes very easily. You can’t clomp around the city or in rough weather the same way you would something like a Red Wing or a Thursday boot. According to Taft, you should use Kiwi’s Oxblood polish for the leather and just use an all purpose upholstery cleaner on the wool. Does a wool boot hold up well in the rain? Well, it should do OK in a light shower but honestly it’s best to avoid wearing them in a downpour. Taft told me that if the boot gets “super wet it will go through the wool and affect the lining somewhat.” TAFT JACK BOOT SOLE The sole is a very soft rubber that has pretty solid grip, after which there’s memory foam, a wood shank, leather, and cork. All of which sounds very bulky but since it has a Blake stitch the shoe is actually pretty damn flexible and light. There are some downsides to the Blake stitch: it’s not that water resistant, which adds to the problems you might experience in wet weather. The sole is very soft which I quite liked, but it takes nicks and cuts very easily and the heel is relatively high and when you combine that with a soft sole you’ll find that it doesn’t feel super stable. But that’s kind of the trade off you often have with soles: soft but not so sturdy, hard but not so comfy. *** Like cheap, furry boots? It’s fortunate that they run a half size large because I’m typically an 11.5 and this company doesn’t offer half sizes or any other widths, which could be a big disappointment for some consumers. TAFT JACK BOOT PRICE At $250 these are cheaper than a lot of boots on the market — most guys don’t take a boot seriously unless it costs over $300. That said, this is a direct to consumer company and the shoe is mostly wool, so honestly, I was expecting it to be a little cheaper. But a government report later found that: "we are aware of no instance in which the program directly contributed to ... disruption of a terrorist attack." Yes, says Ilya Shaprio, the CATO Institute's senior fellow in constitutional studies, Kavanaugh disagrees with libertarians on national security. But on just about every other issue, Kavanaugh would likely advance liberty. Shapiro notes that Kavanaugh is one of the best judges in the country at opposing government regulation on individuals and companies. As a judge on the U.S. Court of Appeals for the D.C. Circuit, Kavanaugh tried to strike down lots of regulations: net neutrality, EPA admissions rules, and the Consumer Financial Protection Bureau. He didn’t always succeed, but he argued that they were all bureaucratic overreaches. Shapiro tells Stossel that Kavanaugh would also likely overturn the "Kelo" eminent domain decision, which allowed governments to take private property for nearly any reason. Kavanaugh is also good on gun rights. Citing the 2nd Amendment, he tried to overturn a DC ban on all semi-automatic rifles. The left fears Kavanaugh mostly for other reasons. Protestors shout "hell no, Kavanaugh" at rallies. Senator Kristen Gillibrand warns: "What’s at stake is freedom for LGBTQ Americans, for equal rights, for civil rights." Senator Bernie Sanders warns he would "tell women they don’t have the constitutional right to control their own bodies." But their biggest fears are misplaced, Shaprio tells Stossel. Kavanaugh is unlikely to change rulings on Roe v. Wade and gay marriage, because he and Chief Justice John Roberts respect precedent, especially if changing it would disrupt people's lives. The left can breathe easier on those things, says Shapiro, but he notes that Kavanaugh will likely overturn one thing that is dear to the left: affirmative action. "Kavanaugh could provide the 5th vote to overturn that 40-year-old experiment with using racial preferences to promote some kind of nebulous diversity." Should libertarians be happy with Kavanaugh overall, Stossel asks? “Definitely," Shapiro says. "He’s not going to agree with us all the time, certainly. But no judges or justices do."

Medicare Advantage is an insurance program that invites private payors, including health plans and insurance companies, to offer alternatives to traditional Medicare. By providing access to the private insurance market, Medicare Advantage gives beneficiaries the option to choose from a wide range of competing plans, many of which offer richer benefits than Medicare. This video explains how Medicare Advantage works. Video transcript Medicare Advantage is part of the U.S. Medicare system, which is a federal health insurance program that covers 50 million elderly and disabled Americans. Medicare Advantage, also known as Medicare Part C, differs from traditional Medicare in that it doesn't define categories of health coverage in the same way as Medicare parts A, B, and D. Instead, it's a special program that invites private payors, including health plans and insurance companies, to provide alternatives to traditional Medicare insurance. As with traditional Medicare, eligibility for Medicare Advantage includes most U.S. residents over age 65, as well as certain nonelderly people with disabilities. While traditional Medicare is administered directly by the federal government, under Medicare Advantage, the government contracts with private payors. At minimum, these payors are required to provide the same level of coverage outlined in Medicare Parts A and B, and typically they provide Medicare Part D prescription drug coverage, as well. By providing access to the private insurance market, Medicare Advantage gives beneficiaries the option to choose from a wide range of competing plans, many of which offer richer benefits. A growing number of Medicare beneficiaries are opting for Medicare Advantage. Twenty-seven percent are currently covered by Advantage plans, and enrollment is expected to continue to increase in 2013. All Medicare Advantage plans are funded by federal Medicare dollars—but some plans require members to pay an additional premium. The premium is determined by the benefits provided and the federal reimbursements in the area where the plan is offered. Medicare has specific rules for how Advantage plans are priced. Each county has its own payment benchmarks, as well as different benefit and premium combinations. Each spring and summer, Medicare Advantage plans participate in a bid process that establishes the terms of contract between the payor and the federal government for the next calendar year. Before bids are submitted, plans project member health statuses, costs, revenue from the federal government, and member premium. These projections serve as the basis for the bids, which define the benefits to be offered in each plan. Once the bid process is complete, members are told what the premiums and benefits will be for each plan in the coming year. During the eight week open-enrollment period for Medicare Advantage, beneficiaries must consider a number of variables to decide on a plan that best suits their healthcare needs and budget. In addition to monthly premiums, benefit options, and out-of-pocket limits, they must also consider network composition and plan type. If provider choice is important to them, they may opt for a plan that features either a Preferred Provider Organization (PPO) or Private Fee-for-Service (PFFS) structure rather than a more restrictive HMO. Beneficiaries will also want to consider how flexible a plan is with respect to authorizing prescriptions. While most plans provide for prescription drug coverage, not all of them will pay for all pharmaceuticals. Within each class of drugs, plans may cover only a limited number of drugs. Ultimately, those looking for more comprehensive benefits may gravitate toward the Medicare Advantage program.
