Dental insurance is confusing. With all the political changes in healthcare, dental insurance benefits are changing too. When a dentist goes out of network on a PPO, it's usually because they don't get paid enough by the insurance to make a profit at their business. When you have a dentist that uses quality materials and the best technology, they actually lose money. You shouldn't just go online and "throw a dart at the board" from among the dentists that take your insurances in-network. A lot of dentists that accept "in-network" insurances are driven on a model where they have to upsell you things in order to make up for the lost income on insurances. That means most often that there's a lot of hidden charges in your bill. It's like buying an airline ticket, and then they charge you for baggage fees and snacks. In order to continue giving people the best care, we're going out of network with some PPO insurances. To save the most money, when you come in, talk with our financial coordinator prior to any major treatment. This way, you can get an estimate of what your fees will be, and get options. You'll be able to get an idea of what your insurance will pay for and what your out of pocket expenses will be. We can also show you any hidden downgrade fees. For example, instead of a white filling - some insurances will only pay for a mercury filling (which we don't do). Sometimes instead of white crown, they'll only pay for a metal one. In the end our top priority is patient care. This means actual time with patients, quality materials and the latest tech that allows us to do the best work possible.

UnitedHealthcare dental plans can offer a great way to meet your needs. More dentists in our network means you are more likely to keep the same dentist you use today and also have coverage available when traveling. You can add vision benefits, which are available in most states, to your dental plan. Our dental plans are family-friendly. And since there’s no maximum age, these plans can also benefit those over age 65. Also, our Dental Plans do not have any medical underwriting. The minimum issue age for these new dental plans is 18. As long as the primary insured is at least 18 years of age, then younger dependents can be on the plan. There are four dental plans available for you to choose from. Dental Primary is the lowest priced of the four plans, and doesn’t include coverage for major services. Dental Primary Preferred offers coverage for major services for a slightly higher premium. Dental Premier Choice is a benefit-rich plan. UnitedHealthcare Dental Premier Elite is a benefit-rich plan with more out-of-network coverage. Dental Primary and Dental Primary Preferred have a deductible for basic services of $50, and Dental Primary Preferred has a deductible for major services of $50. Neither plan has a waiting period for preventive services, but both have a six-month waiting period for basic services. Also, there is a 12-month waiting period for major services on Dental Primary Preferred. Both plans have a $25 copay for preventive care, and the same annual maximum of $1,000 per covered person. Basic services are covered at 70% coinsurance after the deductible, and major services are covered at 50% on Dental Primary Preferred. Major services are not covered on Dental Primary. Both plans cover up to 4 bitewing x-rays once per calendar year and provide out-of-network care based on the network rates. Dental Premier Choice and Dental Premier Elite have a deductible of $50 combined for basic and major services. Neither plan has a waiting period for preventive services, but both have a six-month waiting period for basic services, and a 12-month waiting period for major services. Preventive care is covered at 100%, no copays or deductibles. Both plans start with an annual maximum of $1,200 per covered person. Over time, your annual maximum can increase to $1,500 per covered person. Basic services are covered at 80% coinsurance after the deductible. Major services are covered at 50%. Also, both plans cover up to 4 bitewing x-rays once per calendar year. Dental Premier Choice provides out-of-network care based on the network rate. Dental Premier Elite offers more out-of-network coverage. These dental plans also utilize the UnitedHealthcare Dental network, with access to providers across the United States. Also, if you have an existing UnitedHealthcare dental plan, you can switch to one of the plans! This change requires a new application, and new waiting periods will apply on the new coverage. For an application sent online, the effective date will be the later of the requested effective date or the day after receipt by Golden Rule Insurance Company. For a mailed paper application, the effective date will be the later of the requested effective date or the day after the postmark date. The vision coverage you can add to a dental plan features an extremely simple plan design. There are no waiting periods, and covered services have a simple copay. Utilizing a network provider is very important. After the copay of $10 or $25, depending on the service, the vision plan pays 100% of your covered expenses. If you should choose to utilize an out-of-network provider, UnitedHealthcare still provides coverage, just on a more limited basis. This type of plan offers a higher level of reimbursements when you receive treatment from a “preferred” doctor or hospital. In less fancy insurance terms, you pay less. What Does HMO Mean? HMO stands for Health Maintenance Organization. This plan type works a little different from a PPO. The goal of this plan structure is to have a primary physician manage your care. The most famous example of an HMO structure is the company HealthSpan, formerly known as Kaiser Permanente. Let’s Take a Closer Look So I’ll ask you again, how many doctors do you want to be able to see if something really bad happens? The answer to that question is the answer to your internal PPO vs. HMO debate. But if you want to go even deeper, here’s what you need to think about. When does a HMO plan Make Sense? This is a hard question to answer since it will always come down to personal preference. But, this is how I like to explain it to clients. If there is a HMO plan that has doctors close to your home that you like and trust and also has an adequate number of hospital facilities, that’s a decent first step. You also need to be able to save a considerable amount of money on your health insurance payment every month. Finding both of those together is the tricky part. I have yet to see a HMO plan that offers enough savings to consider recommending one to a client. When you only save a couple dollars every month, you have to ask yourself if it is really worth it to limit yourself that severely when it matters the most. I've never been able to answer yes to that question for a client.

When I consider purchasing an individual health insurance plan for myself or my family, do I have any financial obligations beyond the monthly premium and annual deductible? Answers: It depends on the plan, but some plans have the following cost-sharing elements that you should be aware of. Co-Payments: Some plans include a co-payment, which is typically a specific flat fee you pay for each medical service, such as $30 for an office visit. After the co-payment is made, the insurance company typically pays the remainder of the covered medical charges. Deductibles: Some plans include a deductible, which typically refers to the amount of money you must pay each year before your health insurance plan starts to pay for covered medical expenses. Coinsurance: Some plans include coinsurance. Coinsurance is a cost sharing requirement that makes you responsible for paying a certain percentage of any costs. The insurance company pays the remaining percentage of the covered medical expenses after your insurance deductible is met. Out-of-pocket limit: Some plans include an out-of-pocket limit. Typically, the out-of-pocket limit is the maximum amount you will pay out of your own pocket for covered medical expenses in a given year. The out-of-pocket limit typically includes deductibles and coinsurance. But, out-of-pocket limits don't typically apply to co-payments. Lifetime maximum: Most plans include a lifetime maximum. Typically the lifetime maximum is the amount your insurance plan will pay for covered medical expenses in the course of your lifetime. Exclusions & Limitations: Most health insurance carriers disclose exclusions & limitations of their plans. It is always a good idea to know what benefits are limited and which services are excluded on your plan. You will be obligated to pay for 100% of services that are excluded on your policy. Beginning September 23, 2010, the Patient Protection and Affordable Care Act (health care reform) begins to phase out annual dollar limits. Starting on September 23, 2012, annual limits on health insurance plans must be at least $2 million. By 2014 no new health insurance plan will be permitted to have an annual dollar limit on most covered benefits. Some health insurance plans purchased before March 23, 2010 have what is called "grandfathered status." Health Insurance Plans with Grandfathered status are exempt from several changes required by health care reform including this phase out of annual limits on health coverage. If you purchased your health insurance policy after March 23, 2010 and you're due for a routine preventive care screening like a mammogram or colonoscopy, you may be able to receive that preventive care screening without making a co-payment. You can talk to your insurer or your licensed eHealthInsurance agent if you need help determining whether or not you qualify for a screening without a co-payment. There are five important changes that occurred with individual and family health insurance policies on September 23, 2010. Those changes are: 1. Added protection from rate increases: Insurance companies will need to publically disclose any rate increases and provide justification before raising your monthly premiums. 2. Added protection from having insurance canceled: An insurance company cannot cancel your policy except in cases of intentional misrepresentations or fraud. 3. Coverage for preventive care: Certain recommended preventive services, immunizations, and screenings will be covered with no cost sharing requirement. 4. No lifetime maximums on health coverage: No lifetime limits on the dollar value of those health benefits deemed to be essential by the Department of Health and Human Services. 5. No pre-existing condition exclusions for children: If you have children under the age of 19 with pre-existing medical conditions, their application for health insurance cannot be declined due to a pre-existing medical condition. In some states a child may need to wait for the state's open-enrollment period before their application will be approved.

Sometimes you may need a little coverage. Sometimes you need a lot. Golden Rule Insurance Company, a United Healthcare Company offers Health ProtectorGuard Fixed Indemnity Insurance Plans. These plans offer a variety of benefit options so you can choose a plan that works best for you. For example, would you prefer a plan with benefits for two doctor visits per year, or a plan that offers benefits for five doctor visits per year? Or, maybe a plan with benefits for 10 visits. You choose. Or maybe a wellness preventive visit is more important to you. Our plans offer options of an annual $100, $200 or a $250 wellness preventive care benefit. Do you need prescription coverage? There are plans available with and without prescription coverage. But every plan does include a prescription discount card where you can save 50 to 75% at participating pharmacies. With Health ProtectorGuard you can find a plan that balances benefits with your budget. And since Health ProtectorGuard is fixed indemnity coverage, a doctor visit benefit of $100 will pay $100 regardless of the actual office visit charge. For example, if you go to the doctor and the visit actually only costs $75, a $100 benefit is still paid. But you get to keep the remaining $25. On the other hand, if the doctor visit actually costs $125 you still get a $100 benefit but you would simply pay the difference. Each specified medical expense pays a fixed amount of cash. Besides doctor visits, specified medical expenses include but are not limited to, hospital stays, emergency room visits, ambulance services, surgeon fees, x-rays and lab tests, chemotherapy and preventive care. Benefits for these individual services depend on the plan level you purchase. Lower cost plans generally pay lower amounts. Higher benefit plans generally cost a little more. Whatever you choose, it's important to know that Health ProtectorGuard plans are not ACA coverage which means it isn't considered minimum essential coverage. We'll have some medical questions on the application because Health ProtectorGuard plans do not provide coverage for preexisting medical conditions. That means, if you are being treated for a medical condition or are experiencing symptoms before coverage begins then the policy won't cover those things for a specific amount of time. The plan is designed to cover you if new medical issues arise during your coverage period. Health ProtectorGuard, whether you need a little coverage or a lot. Choose the plan with benefit levels that work best for you.
