Thyroid Cancer Symptoms & Early Warning Signs
This video is all about thyroid cancer symptoms and early warning signs you may have cancer. This is a topic worth discussing because many people who believe they have thyroid cancer probably don't. The good news is that even though thyroid cancer is fairly common, it has a great survival rate with a 5-year survival rate of around 98%. So, even if you do have thyroid cancer, it's one of the better types of cancers to have. An interesting fact about thyroid cancer is that it typically does NOT cause any symptoms. In fact, most people who are diagnosed with thyroid cancer present to their doctor with a normal exam and without any symptoms. In some cases, however, if your cancer is sufficiently large then it may cause problems to nearby structures in your neck. Your neck is a tight place which means that if anything grows in that area it may push on or bump into other structures which may result in symptoms. With that in mind, if your cancer is large enough it may present with the following symptoms: - Compression of the esophagus or trachea - Difficulty swallowing - - Swelling or enlargement of the neck (bulge in the neck) - Changes to speech including hoarseness - Enlargement of lymph nodes in the neck - Symptoms of thyroid goiter - Pain in the neck, especially around the thyroid gland - Constant non-productive cough It's important to note that in the majority of cases thyroid cancer will not cause problems with the production of thyroid hormone (but you should still have it tested to be sure). Some other conditions may mimic thyroid cancer and you want to watch out for these as well. They include: - Thyroid Nodules - Thyroid Cysts - Goiter - Hashimoto's thyroiditis These can be differentiated from thyroid cancer with an ultrasound test and certain blood markers. If you suspect that you have thyroid cancer then your next steps should be: - Complete history and physical exam - Thyroid function lab tests (if you have symptoms such as fatigue, etc.) - Thyroid ultrasound - Biopsy (if necessary) - Iodine testing - Other blood tests such as inflammatory markers These steps will help differentiate between thyroid cancer and the other conditions listed above. Thyroid Nodule Mass - Later Found To Be Cancer
11/2/15 UPDATE - Biopsy results revealed "atypical" cells, doctor didn't use the "C" word but insisted that the entire thyroid be removed ASAP. I believe these types of cells are considered pre-cancerous. I was expecting this, so I am not upset. 11/20/15 UPdate - Finally scheduled surgery for Jan 7th. My thyroid is completely whacked out, mostly hypo but sometimes hyper, some days it is hard to breathe...among other symptoms, life is absolute hell. I am literally counting down the days until surgery. They want to start by only taking out the left side, which is where this huge mass is, if it comes back clear from pathology, I can keep the right side. But I still need to get on hormones due to being hypo for so many years. Uh yeah, fun video today. I recently discovered I have a pretty large mass on the left side of my thyroid. I've had an enlarged thyroid for years, much longer than before the doctor noticed, because I used to cut slits in my hoodies so they wouldn't sit so far up on my neck, sometimes it felt so uncomfortable! Anyway, a year passed between the doctor's exams and he thought it was bigger, so I got an ultrasound, which revealed a mass. I got a fine needle biopsy done last week, I don't know the results yet, but will update. From what I understand, this is actually fairly common and 95% of the time it is benign. This runs in my family, but I'm not exactly sure what to do about it. My bloodwork is always normal, and I think for the most part, I don't experience symptoms of hyper- or hypo-thyroidism. Maybe every once in a while. I am hypoglycemic and I have alopecia, but I don't know if any of that is related. My mother has a really terrible autoimmune disease but no thyroid problems. Every once in a while, I do feel like my face is puffy and red, I deal with anxiety occasionally, I am normally a very mellow, easy-going person so it is different. I have always been thin with an athletic build, I am 30 now and still look like I am 10 years younger. I do honestly work out excessively, so I maintain that build, however I do not know how to gauge if my progress is on a normal scale. I have a feeling if I did not work out so much, and just depended on my diet (which is good!), I might not stay so thin. I don't know. You can see the mass when I swallow. If it isn't super evident at first, I pointed to it. It is about 3.18cm, so it's pretty big. I woke up feeling that slight discomfort all through my neck and it has been bugging me all day, I kept having to pull my hoodie neckline down. Now that I am aware of my condition, I am noticing some things more often, like the fact that I can SEE the mass! Wow. 10/30/15 Update: Doctor has not commented on biopsy yet, but I called and the woman from the office read me the results from the lab. Said "carcinoma cannot be excluded at this time". Going back in a few days to show/tell the doc how it's been growing and pushing on my windpipe and talk about well, probably getting it removed.

Trevor E. Angell, MD, MHS, Division of Endocrinology, Diabetes, and Hypertension at Brigham and Women’s Hospital, discusses thyroid nodules. What are thyroid nodules? Thyroid nodules are discrete lesions within the thyroid that can be thought of as a mass or a lump. They can represent many different things, as opposed to just thyroid enlargement, which is sometimes called a goiter. Thyroid nodules are more common in older individuals and women are more likely to develop thyroid nodules than men. What are the risk factors for developing thyroid nodules? There are a number of things that may increase the risk of having a thyroid nodule, one of which is iodine deficiency, although that’s rare in the United States. People who have had radiation exposure to the area in the neck where the thyroid is located, particularly if that happened during childhood, are more likely to have thyroid nodules. Also, people with an underlying autoimmune thyroid disease, which is sometimes called Hashimoto’s, or lymphocytic thyroiditis, may develop thyroid nodules. What are the symptoms of thyroid nodules? Most thyroid nodules are asymptomatic and they're often found incidentally on different kinds of testing but a number of them can be either felt by a physician as a small firm lump where the thyroid is located in the neck. How are thyroid nodules evaluated? Almost all thyroid nodules are benign. The primary way to evaluate a thyroid nodule without removing it surgically is what's called a fine-needle aspiration biopsy. In this procedure a very small needle is put into the neck where the thyroid nodule is located and cells are collected for evaluation. It's a short well-tolerated procedure that's usually done under local anesthetic. Patients are able to go home afterwards with very few issues. When is surgery needed? Surgery can be required for the treatment of thyroid nodules in a variety of situations. If the thyroid nodule is found to be a malignancy, typically a thyroid cancer, the standard treatment at this point is a surgery to remove that nodule along with the thyroid in its entirety. There are other cases in which surgery will be necessary. Some nodules, even after biopsy can't be definitively described as benign or malignant. In this case the nodule may need to be removed surgically to determine what it is. There are also cases in which benign thyroid nodules need to be removed surgically because they are compressing in a place that's causing symptoms that aren't well tolerated. On Tuesday, the U.S. Preventive Services Task Force issued an update continuing to recommend against thyroid cancer screening in adults without symptoms. Christian Nasr, M.D., of Cleveland Clinic was not involved in creating the recommendations, but said the harms of screening the population to find a small thyroid cancer likely outweigh the benefits. “They found that there was no benefit and there’s probably slight harm from finding disease that was not supposed to be found and treating that disease,” said Dr. Nasr. Over the past ten years, the incidence of thyroid cancer has risen 4.5 percent per year. Doctors aren’t sure why cases are rising, but research shows that despite finding the disease early, survival hasn’t changed substantially. Most cases of thyroid cancer have a good outcome with about 98 percent survival at five years. More screening would likely find more early stage cancer and lead to potentially unnecessary treatments, which may carry more harm than the cancer itself. “When you talk about treatment, you know you’re removing the thyroid, you’re committing the patient to a long term thyroid hormone, so we have to make sure the patient really needs this treatment,” said Dr. Nasr.
