Stages of Labor Nursing OB for Nursing Students | Stages of Labour NCLEX Explained Video Lecture

18:53
Stages of labor nursing OB review for nursing students who are studying for the NCLEX exam and maternity nursing lecture exams. This lecture explains the various stages of labour. There are four stages of labor and they include Stage 1, Stage 2, Stage 3, and Stage 4. In nursing school, you must know that the Stage 1 of Labor has three phases. These phases are the Latent (early labor), Active, and Transition phases. Stage 1 of labor will be where the cervix will dilate (open) from 0 to 10 cm and will have hundred percent effacement (thinning). During the Latent phase of Stage 1, the cervix will dilate 1-4 cm, contractions will occur every 5 to 30 minutes and be 30-45 seconds in length. During the active phase of stage 1, the cervix will dilate to 4 to 7 cm and thin (dilation will be 1 cm per hour), and contractions will be noticeably stronger and longer (45 to 60 seconds) every 3 to 5 minutes. The next phase, which is the transition phase, will be the shortest but most intense in regards to the strength and duration of contractions. During the transition phase, the cervix dilates from 8 to 10 cm and thins completely. This phase will lead to Stage 2 of Labor. Stage 2 of labor begins when the cervix has fully dilated and ends when the baby is fully delivered. Stage 3 of labor begins when the baby is fully delivered and ends when the placenta is delivered. Stage 4 of Labor is the first 1-4 hours after the delivery of the placenta and is the time when the mother is at most risk for hemorrhage, infection, and other problems. Eye Assessment Nursing | How to Assess Eyes for Head-to-Toe Assessment

04:54
The eye assessment in nursing is a way to gauge the patient's eye health. You'll generally perform a nursing eye assessment as a part of your general head-to-toe nursing assessment. Note: Our full Head-to-Toe assessment video is here: https://www.youtube.com/watch?v=gG8kh8MfnGY&t=25s While checking the eye, you'll want to note several things. How does the eye look? Do the eyelids look swollen or red? Does the sclera of the eye look nice and white, or does it have a yellow appearance, which is common in patients with jaundice? You'll want to check for strabismus, which is a misalignment of the eye. Anisocoria is another thing you'll want to check, which is unequal pupil dilation. You'll want to assess the pupil accommodation, reflex, dilation/constriction as well. Are the pupils an appropriate size (3-5 mm)? Are they both equal? Do both pupils constrict when exposed to light and dilate in its absence? This eye assessment will also assess the cranial nerve III (oculomotor), cranial nerve IV ( trocklear), and cranial nerve VI (abducens). To perform the eye exam, you'll want to have your penlight handy. Remember the acronym PERRLA, which stands for Pupils Equal, Round, Reactive to Light, and Accommodation. The head-to-toe assessment in nursing is an important physical health assessment that you'll be performing as a nursing student and nurse. Head-to-toe assessments allow nurses to assess the health status of patients by following a checklist of criteria. On the job, your head-to-toe nursing assessment will be performed much faster, and it may be different or more specialized to accommodate the patients' needs within your nursing specialty. This assessment represents a general assessment checklist (or cheat sheet) that you might encounter in nursing school. (Note: Always follow your instructor's requirements or your employer's assessment protocols). This nursing head-to-toe examination video guide will focus on the following areas/skills: -Vital Signs (pulse rate, respiration rate, temperature, oxygen saturation, blood pressure, pain assessment) https://www.youtube.com/watch?v=JpGuSxDQ8js&t=103s -Cranial Nerve examination -Head assessment (hair, cranium, eyes, nose, mouth, ears, sinuses) -Neck assessment (jugular vein, thyroid, trachea, carotid) -Heart sounds assessment: https://www.youtube.com/watch?v=H48WsyIjFs0&t=73s -Lung sounds assessment: https://www.youtube.com/watch?v=KNrcG077brQ -Abdominal assessment -Assessing extremities (arms, hands, legs, feet) -Back assessment -and more While performing your comprehensive head-to-toe assessment, you'll want to record your findings in the documentation. Colostomy and Ileostomy Nursing Care | Types of Ostomies NCLEX | Ileostomy vs Colostomy

24:34
Colostomy and ileostomy nursing care video for the NCLEX exam. Learn about the types of gastrointestinal ostomies, such as the colostomy and ileostomy. The types of colostomies include the ascending colostomy, transverse colostomy, double-barrel colostomy, descending colostomy, and sigmoid colostomy. An ostomy is a surgical opening created on the surface of the abdomen to allow stool (waste) to exit the body rather than through the rectum. Gastrointestinal ostomies will be formed from the small or large intestine. A colostomy is a surgical opening created to bring the LARGE intestine (also called colon) to the surface of the abdomen. An ileostomy is an opening created to bring the SMALL intestine to the surface of the abdomen (specifically the ileum). As a nurse providing care to a patient with a GI ostomy, it is important to know the pre-opt/post-opt care along with diet, medication restrictions (no enteric-coated and sustained-released medications) for the ileostomy, the different types of ostomies, stoma care, and the stool consistency expected from the ostomy. Depending on the type of ostomy and its location, the patient should have the following stool types: Ascending: liquid stool, Transverse: lose to partly formed stool, and Descending/Sigmoid: similar to normal consistency, and ileostomy: liquid stool. Colostomy Irrigation Procedure Care | Ostomy Enema Nursing | Stoma Cleaning

13:12
Colostomy irrigation procedure nursing teaching video for ostomy care. The purpose of performing colostomy irrigation is to remove stool from the colon by instilling an enema of warm water through the stoma. This stimulates the colon to empty it contents. A physician may order ostomy irrigation when stoma output is low or the patient wants to train the colon for bowel continence. Ostomy irrigation is used for patients who have a descending or sigmoid colostomy. Why? Because these patients still have majority of the large intestine in place and is able to produce formed stool when compared with other types of ostomies (hence these patients will have the most success with ostomy irrigation for bowel training). Patients with an ileostomy are not candidates for this procedure because the stool is always a liquid consistency and it is rich in fluid and electrolytes. It takes about 6-8 weeks for ostomy irrigation to be successful when training the colon. The result will be that the patient will not have bowel movements between irrigations and will not have to wear a continuous pouching system. Instead, the patient will wear a stoma cap which is a very small closed pouch. It is best to perform colostomy irrigation at the same time every day and one hour after meals. Ostomy irrigation can take (from start to finish) 1 hour. Supplies you will need: Irrigation bag with tubing and cone, warm water of 500 cc to 1000 cc (depending on physician's order), water-soluble lubricant, irrigation sleeve with clip (if patient has a two-piece ostomy system: irrigation sleeve will attach to the flange/base plate or if patient has a one-piece pouching system: irrigation sleeve will attach to the face-plate via ostomy belt), towels/wash cloths, gloves, PPE (gown). Congestive Heart Failure (CHF) Pathophysiology, Nursing, Treatment, Symptoms | Heart Failure Part 1

23:11
Congestive heart failure (congestive heart failure CHF) pathophysiology, treatments, and symptoms nursing lecture on heart failure for nursing students. This congestive heart failure NCLEX review with give you mnemonics on how to remember the causes of heart failure and the different types of heart failure, such as left-sided or right-side heart failure. I also discuss the pathophysiology of both left ventricular systolic and diastolic dysfunction along with right-sided dysfunction. Signs and symptoms of heart failure affecting the left-side are pulmonary in nature with the patient experience, dyspnea, crackles, orthopnea, and paroxysmal nocturnal dyspnea. Whereas, the signs and symptoms of right-sided heart failure are peripheral in nature with the patient experiencing swelling of the liver, hands, feet, abdomen nocturia, and jugular venous distention. Tests used to diagnose heart failure include a blood test called BNP, echo-cardiogram, heart catheterization, chest x-ray, or nuclear stress test. Left-sided heart failure is the most common type of heart failure and can lead to right-sided heart failure. Patients with left ventricular systolic dysfunction have low ejection fractions (EFs) compared to patients with left ventricular diastolic dysfunction who may have a normal ejection fraction. Cirrhosis of the Liver Nursing Care Management Symptoms NCLEX

34:23
Cirrhosis of the liver nursing NCLEX review over the symptoms, nursing care/management, pathophysiology, complications, and treatments. Cirrhosis is a liver disease where liver cells become extremely damaged due to long-term/severe damage. This damage can be from a viral infection (Hepatitis B or C), heavy alcohol consumption, obesity, hyperlipidemia, bile duct problem, autoimmune etc. These damaged liver cells are replaced with fibrous tissue, and this leads to scarring of the liver. The role of the liver is to filter the blood and perform the following functions to it: metabolizing, detoxifying, storing, producing etc. When liver cells are damaged these processes do not happen. Complications of cirrhosis include: portal hypertension, splenomegaly, ascites, jaundice, hepatic encephalopathy, esophageal varices etc. All of these conditions are discussed in depth in the video. During the early stages of cirrhosis the patient may be asymptomatic, however, in the late stages the patient will have signs and symptoms that include: asterixis, fetor hepaticus, jaundice, reduced platelets and white blood cells, spider angiomas, ascites, gynecomastia, increase ammonia level etc. Nursing interventions for cirrhosis include monitoring for bleeding, abnormal glucose levels, implementing a proper diet: if patient has confusion (hepatic encephalopathy) they need a low protein diet, however, if patient isn't confused they need a high lean protein diet, low sodium, fluid restriction, assessing for rupture of varices, administering lactulose etc. Treatment for cirrhosis includes a liver transplant and medications such as diuretics, blood products, vitamin K, beta blocker or nitrates. In addition to a paracentesis. which helps remove fluid from the abdomen, if ascites is present. Appendicitis Symptoms, Examination, Nursing Assessment | NCLEX Review Appendectomy and Peritonitis

17:11
Appendicitis review of the causes, symptoms, complications (perforation, abscess, peritonitis), pathophysiology, and nursing care for the NCLEX exam. Appendicitis is the inflammation of the appendix. Where is the appendix? It is found on the right lower side of the abdomen and connects to the cecum of the large intestine (it looks like a protruding worm or finger-like structure coming out of the large intestine (specifically the ascending colon). The role of the appendix is to store the "good" bacteria in your GI tract while the tract is recovering from a diarrhea illness (so it helps maintain healthy GI flora). Appendicitis can be caused from an obstruction (fecalith...hard piece of feces, parasites...worms, enlarged lymph nodes) or trauma/injury. When appendicitis occurs, the lumen of the appendix becomes blocked and this increases pressure in the appendix. As the pressure increases, venous obstruction occurs which can cause the formation of a blood clot. Clot formation leads to ischemia to the appendix which leads to perforation (rupture of the appendix) and eventually peritonitis. Signs and symptoms of appendicitis include: abdominal pain that starts in or around umbilicus and radiates to the right lower quadrant, pain will be most intense at McBurney's Point, abdominal rebound tenderness/rigidity, nausea, vomiting, fever, and relief of pain when placed in the fetal position. Treatment for appendicitis is an appendectomy. Peptic Ulcer Disease Pharmacology and Nursing Care | Gastric and Duodenal Ulcer NCLEX Lecture Part 2

18:59
Peptic ulcer disease pharmacology and nursing care NCLEX lecture review. Peptic ulcer disease is the formation of ulcers in the lining of the upper GI tract that affects mainly the mucosal lining of the stomach, duodenum or esophagus. There are various types of peptic ulcers such as gastric ulcers which are located in the stomach and duodenal ulcers which are located in the duodenum. Peptic ulcer disease is treated with various medications such as proton-pump inhibitors, histamine receptor blockers, antacids, antibiotics, muscosal healing agents, and bismuth subsalicylates. Treatment of ulcers due to h. pylori infection includes a various drug regime of antibiotics, PPIs, H2 blockers, and bismuth subsaliylates per physician order. Peptic ulcer disease nursing care involves assessing, monitoring, educating, and administering medications to the patient per physician's order. The nurse will want to assess and monitor the patient for any signs and symptoms of complications from a peptic ulcer, such as gastrointestinal bleeding. GI bleeding could present as vomiting "coffee ground" emesis or bright blood or dark, tarry stools along with increased heart, low blood pressure, abdominal mass or tenderness. The patient should be monitored and educated on dumping syndrome if a gastric resection is performed to cure the ulcer. In addition, the patient should follow a soft, bland until the ulcer is healed. It is also important the patient avoids alcohol usage, smoking, or consuming caffeine products because these products can irritate the ulcer. Signs and symptoms of PUD include main indigestion and epigastric pain that can be felt from the breastbone to the belly button. It is typically described as a gnawing, aching, or dull pain. Complications of peptic ulcer disease include: gastrointestinal bleeding, perforation which can lead to peritonitis, and obstruction of the pylorus which is a muscular section that connects the stomach and duodenum together and allows food to flow between these parts of the GI tract. 5 Common New Nurse Grad Mistakes to Avoid

06:20
When you're a new nurse, it's easy to make rookie mistakes that can end up costing you a lot of problems down the line. Here are 5 common mistakes that new nurses make: 1. Failing to Think Critically: When you're a new nurse, it's easy to focus solely on tasks that need to be completed. Instead, you have to force yourself to think critically, as I describe in this video. 2. Social Media Problems: A common mistake that new nurses make is posting sensitive patient information on social media. Remember, we have HIPAA and other privacy laws that we must obey. In addition, many nurses post too often on social media during a shift, which can affect patient care. 3. Time Management Problems: It can be a real challenge to complete all of your tasks as a new nurse, and if you don't take time to learn how to perform your tasks more efficiently, things will only get worse. Time Management TIps: https://www.youtube.com/watch?v=8vjrlriT-SE 4. Getting Caught Up In Gossip, Cliques, or Drama: It's only a matter of time before people try to win you over to their clique, or a coworker may begin to gossip about another coworker. It's best to avoid politics on your nursing unit and treat everyone with respect. 5. Putting Too Much on Your Plate: Even though you're so excited to begin working as a nurse and to start earning a paycheck, you have to be careful to avoid burnout. I'd recommend that you pace yourself and avoid piling on too many extra shifts, especially during your first 6-12 months. Ostomy Bag Pouch Change | Ostomy Care Nursing | Colostomy, Ileostomy Bag Change

11:11
Ostomy bag change (ostomy care) nursing procedure for nurses and nursing students. This video will demonstrate how to change a colostomy or ileostomy pouching system, specifically the one-piece pouch system. In this ostomy teaching video, you will learn the supplies you will need to change an ostomy bag (pouch, skin barrier which is also called a wafer or flange, barrier ring if needed, a clip to close the pouch, stoma measuring card, ostomy scissors, washcloths, towels, and gloves). First, you will want to perform hand hygiene and don gloves. Then, remove the pouching system (you may need adhesive remover). Properly discard the system. Then cleanse the skin around the stoma (stoma care) and the stoma with warm water. You may use mild soap but be sure it contains no alcohol, perfumes, or lotions. Assess the surrounding skin for breakdown. Then, pat the area dry and trim any hair if needed (this will help the new pouching system stick and prevent pain when the system is removed in 3-5 days). Then, measure the stoma with a measuring card. Be sure to pick a measurement that measures 1/8 inch larger than the stoma. The opening you create needs to be a good fit rather than too big (may leak stool onto the skin) or small (risk of constricting the stoma). Remember new stomas will shrink overtime and should be measured regularly to ensure proper fit of the new ostomy system. After measuring the stoma, cut the measurement on the skin barrier/wafer. You can trace it using the measuring card or most skin barriers have it outlined for you. Then cut the opening with the ostomy scissors. Ostomy scissors are specially designed to achieve a round cut without leaving jagged edges which could cause trauma to the stoma. Next, seal the opening to the bag (if not a closed system) and apply the pouch. Hard to Find Veins Blood Draw, IVs, Venipuncture - Nursing, Phlebotomy

05:56
Do you have trouble finding veins when drawing blood, starting IVs, or other venipuncture procedures? If so, you're not alone. Many professionals (working in phlebotomy or nursing) struggle with finding veins in many patients or can't find veins at all. Veins can be especially hard to find when patients are obese, dehydrated, or suffering from advanced cardiac or renal disease. Luckily, there are some solutions that can help you find those difficult veins. First, you have to learn to palpate the arm to learn what a vein feels like. Oftentimes, I will find a vein by touch, not sight. I'd recommend practicing on your own arm (or someone with great veins) to learn how this feels. Second, you have to take advantage of tourniquets and hand pumping. Apply the tourniquet and instruct the patient to hold their arm to the side while pumping their fist. This helps those hidden veins become engorged. Sometimes you can't use this technique, though, especially in cases where blood is being drawn for sensitive lab work, but it can help in other cases. Those two tips above have helped me find veins in most patients who don't have visible veins. Nevertheless, even those tips don't work in all cases. If you still can't find a vein, you have a few other solutions. You might be able to use a vein finder or vein light tool. Some use bright lights to illuminate the skin, whereas others will use infrared to display vein locations. In addition, I've consulted with the ultrasound techs, and I've been able to find veins that way. And finally, you always have a central line option if nothing else works. Personalized Cancer Care Plans in the Age of "Dr. Google"

00:52
Ann Culkin, R.N., O.C.N, an oncology nurse at Memorial Sloan Kettering Cancer Center, discusses the importance of patient-centered care in the age of the internet. Culkin generally asks worried patients and their loved ones to stay off the internet, because "Dr. Google," a term referring to diagnoses and information patients get from search engines, can often create worry and fear. To mitigate this, it is imperative to revert back to personalized medicine and helping patients understand their individual diagnosis.

Urinary Tract Infection Nursing NCLEX | UTI Symptoms Treatment Cystitis, Pyelonephritis, Urethritis

29:03
Urinary tract infection (UTI) nursing management of the treatment, signs and symptoms and causes for the following conditions: cystitis, pyelonephritis, urethritis, ureteritis. A urinary tract infection is an infection found within the urinary tract which is mainly caused by a bacteria (usually E.coli from the GI tract). It can also be caused by a virus or fungus as well. A UTI can be found anywhere through the urinary system such as:, Lower urinary system: Urethra (Urethritis) and Bladder (Cystitis) or the upper urinary system: Ureters (ureteritis) which is usually associated with bladder or kidney infection and Kidneys (pyelonephritis). There are many defense systems in the urinary tract that help prevent a urinary tract infection. However, if these defense systems are broken down a urinary tract infection can develop. These defense system include: physiology of how urine drains out of the kidneys (no back flowing), urine itself (sterile and has antiseptic properties), lining of the urinary tract (immune cells to fight infection), prostate gland (secrete antimicrobial fluid), and normal flora of the vagina (good bacteria to fight bad bacteria). What are the causes of a urinary tract infection (UTI)? The following can cause a UTI: Hormone changes: pregnancy, menopause, birth control (changes the normal flora in the vagina), Antibiotics: changes the normal flora of the body, Renal Stones: cause blockage..urine stays in the kidneys and can’t drain, Diabetes: compromised circulation, the immune system is not able to work as strong as it should to fight the infection, high glucose in the urine (breeding ground for bacteria to flourish), bladder doesn’t empty as it should (urinary retention from nerve damage), Toiletries: excessive bubble baths, powders, perfumes, especially scented tampons and sanitary napkins, Obstructive prostatic hypertrophy…seen in male with BPH: The urethra which is surrounded by the prostate gland becomes squeezed shut from the large prostate gland which causes urine to stay in the bladder longer and the prostate gland isn’t able to properly secrete fluid that has antimicrobial properties, Vesicoureteral reflux: most common in pediatric patients and it causes urine from the bladder to back flow into the kidneys (usually congenital defect) due to a ureter valve problem, Overextended bladder, Indwelling catheters or invasive procedures, Intercourse, especially with the usage of spermicides, incontinence of the bowel, and Decreased immune system. Symptoms of urinary tract infection include: Pain when voiding (burning), Persistent need to void but not a lot is voided or can’t void, Odor to urine that is dark and cloudy (can have blood), Cramping in abdomen or pain at the costovertebral angle, Spasms of the bladder or urethra, Fever, Increased WBC on U/A Nursing management of UTI includes: assessing for signs and symptoms of UTI (catch it before it becomes complicated), Maintain fluid status (intake and output), control pain, monitor for complications, administer medications per MD order, EDUCATE the patient about how to prevent and follow MD treatment plan for UTI etc. Quiz on Urinary Tract Infection: http://www.registerednursern.com/urinary-tract-infection-nclex-questions/ Lecture Notes: http://www.registerednursern.com/urinary-tract-infection-nclex-review/ More Renal Lectures: https://www.youtube.com/playlist?list=PLQrdx7rRsKfXkfh1nWrtxLcRTrohj41su Subscribe: http://www.youtube.com/subscription_center?add_user=registerednursern Nursing School Supplies: http://www.registerednursern.com/the-ultimate-list-of-nursing-medical-supplies-and-items-a-new-nurse-student-nurse-needs-to-buy/ Visit our website RegisteredNurseRN.com for free quizzes, nursing care plans, salary information, job search, and much more: http://www.registerednursern.com Check out other Videos: https://www.youtube.com/user/RegisteredNurseRN/videos All of our videos in a playlist: https://www.youtube.com/watch?v=pAhHxt663pU&list=PLQrdx7rRsKfXMveRcN4df0bad3ugEaQnk Popular Playlists: NCLEX Reviews: https://www.youtube.com/playlist?list=PLQrdx7rRsKfWtwCDmLHyX2UeHofCIcgo0 Fluid & Electrolytes: https://www.youtube.com/playlist?list=PLQrdx7rRsKfWJSZ9pL8L3Q1dzdlxUzeKv Nursing Skills: https://www.youtube.com/playlist?list=PLQrdx7rRsKfUhd_qQYEbp0Eab3uUKhgKb Nursing School Study Tips: https://www.youtube.com/playlist?list=PLQrdx7rRsKfWBO40qeDmmaMwMHJEWc9Ms Nursing School Tips & Questions" https://www.youtube.com/playlist?list=PLQrdx7rRsKfVQok-t1X5ZMGgQr3IMBY9M Teaching Tutorials: https://www.youtube.com/playlist?list=PLQrdx7rRsKfUkW_DpJekN_Y0lFkVNFyVF Types of Nursing Specialties: https://www.youtube.com/playlist?list=PLQrdx7rRsKfW8dRD72gUFa5W7XdfoxArp Healthcare Salary Information: https://www.youtube.com/playlist?list=PLQrdx7rRsKfVN0vmEP59Tx2bIaB_3Qhdh New Nurse Tips: https://www.youtube.com/playlist?list=PLQrdx7rRsKfVTqH6LIoAD2zROuzX9GXZy

Pneumonia Symptoms, Pathophysiology, Nursing | Respiratory Disorders NCLEX Lecture Part 1

17:17
Pneumonia symptoms, pathophysiology (pathology) presentation for nursing NCLEX review. In this respiratory nursing NCLEX review, I will discuss the pneumonia pathophysiology, signs and symptoms, types of pneumonia, and how it is diagnosed. This respiratory disorder affects the lower respiratory system and is due to an infection caused by either bacteria, virus, or fungi that causes inflammation of the alevoli sacs. The lung parenchyma are affected which are the alevolar ducts, bronchioles, and aveoli. These three units play an important role in gas exchange in the respiratory system. Patients with pneumonia can experience respiratory acidosis with a PCO2 greater than 45, blood pH less than 7.35, PO2 less than 90, and if the body is trying to compensate the HCO3 will be greater than 26. Types of pneumonia include: community-acquired and hospital-acquired. Pneumonia is diagnosed with a chest x-ray and sputum culture along with abnormal pneumonia lung sounds like coarse crackles, rhonchi (type of wheezing), or bronchial breath sounds in the peripheral lung fields which represent lung consolidation. Pneumonia symptoms include productive cough, pleuritic pain, neuro changes (especially in elderly patients), elevated labs like PCO2 and WBCs, unusual breath sounds, mild to high fever, decreased oxygen saturation, nausea/vomiting, increased respiratory rate and heart rate, and activity intolerance with shortness of breath. Don't forget to watch Part 2 which will include nursing interventions and medications used in pneumonia. COPD (Chronic Obstructive Pulmonary Disease), Chronic Bronchitis, Emphysema-NCLEX Part 1

21:36
Chronic obstructive pulmonary disease (COPD) nursing NCLEX lecture on the pathophysiology, signs and symptoms, types (chronic bronchitis and emphysema), complications, and how it is diagnosed. COPD is a pulmonary disease that causes chronic obstruction of airflow from the lungs. The cause of the limited airflow is due to chronic inflammation and narrowing of the bronchioles which becomes deformed over time and produces excessive mucous (mainly in chronic bronchitis) and the loss of elasticity of the alevoli sacs (mainly found in emphysema). These problems lead to limited gas exchange such as respiratory acidosis due to extreme retention of carbon dioxide (CO2) and low availability of oxygen. COPD is a "catch all" term used to describe diseases that limit airflow which now includes diseases such as chronic bronchitis and emphysema. Patients with chronic bronchitis are sometimes referred to as "blue bloaters". This is because of the cyanosis and edema (hence the bloating) experienced with this disease. Patients with emphysema are sometimes referred to as "pink puffers". These patients do not experience cyanosis, although they have low oxygen levels but maintain a "pink" appearance due to the compensatory state of hyperventilation keeping the oxygen level sufficient enough. The hyperventilation leads to the increased usage of the accessory muscles which leads to a barrel chest look (increased anteroposterior diameter noted on inspection). Signs and symptoms of COPD include: chronic dry or productive cough, lack of energy, extreme dyspnea, abnormal blood gases, abnormal lung sounds (coarse crackles or wheezing), barrel chest, tripod position for breathing, poor nutrition, recurrent lung infection etc. Complications of COPD include: heart disease such as right-sided heart failure, pulmonary hypertension, pneumothorax, pneumonia, increase risk of lung cancer. Chronic obstructive pulmonary disease is diagnosed with spirometry. Don't forget to watch Part 2 which includes the nursing interventions and medication regime for COPD. Diverticulitis | Diverticular Disease Nursing | Diverticulosis Symptoms, Diet, Treatment NCLEX

18:01
Diverticulitis, a diverticular disease, nursing NCLEX review of diverticulosis, diverticulitis symptoms, diet, treatment, and nursing interventions. Diverticulosis is the formation of hollow sac cavities throughout the intestinal wall. These outpouching sacs can form anywhere throughout the intestine but are most commonly found in the sigmoid colon of the large intestine. Diverticulitis is a complication from diverticulosis and occurs when several diverticula or single diverticulum become inflamed, hence why it is known as diverticulITIS. Other complication that can arise from diverticulosis are diverticular bleeding, fistula formation (most commonly of the bladder), and strictures. Diverticular bleeding is due to the rupturing of an artery that surrounds the diverticulum. When diverticulitis occurs it can lead to abscess formation, peritonits, sepsis, bowel obstruction, and fistula formation. Nursing interventions for diverticulitis included monitoring gastrointestinal system for complications such as peritonitis and following physician's orders for diet guidelines. During the initial phase of diverticulitis, when the patient is experiencing signs and symptoms, the patient will be nothing by mouth until the symptoms have decreased. In addition, the nurse will be responsible for administering IV or oral antibiotics along with IV solutions. During the recovery phase, as the signs and symptoms decrease, the patient will be started on clear liquids. If tolerated, the physician may order the patient to be advanced to a low-fiber diet and to consume plenty of fluids (unless the patient has a condition that restricts fluid intake, such as heart or renal failure). This is the only time a patient with diverticulosis should on a low-fiber diet. A low-fiber diet will allow the bowel to rest and heal. Low-fiber foods include white rice, cooked/skinned vegetables and fruits, eggs etc. Once full healing has occurred the patient will need to consume a high-fiber diet which should include raw fruits and vegetables, grains like bran, oatmeal, and beans. High-fiber foods will keep the stool soft and bulky, hence prevent constipation. It is important to prevent constipation in a patient with diverticulosis because theories suggest constipation and straining during a bowel movement increases colon pressure which leads to diverticulitis. In addition, the physician may prescribe the patient to take Psyllium (Metamucil) which is a fiber supplement to keep stool soft. Psyllium works be removing water from the intestine and allows for the stool to be softer. New research now suggests it is okay for patients with diverticulosis to consume seeds or nuts. However, it is always important to follow your GI doctor's recommendations. Crohn's Disease Nursing | Crohns Symptoms, Pathophysiology, Treatment, Diet NCLEX

25:48
Crohn's disease nursing review regarding diet, symptoms, pathophysiology, nursing interventions and treatment for NCLEX exam. Crohn's disease is a type of inflammatory bowel disease that causes inflammation and ulcers formation in the GI tract. Crohn's can be found in both the small and large intestine; however, it seems to be most common in the terminal of the ileum and beginning of the colon. Crohn's disease affects the whole bowel lining which is why its complications are unique when comparing crohn's disease vs ulcerative colitis. Crohn's disease complications include fistulas, fissures, abscesses, malnourishment, strictures which can lead to bowel obstructions, and other changes such as mouth ulcers, skin changes, gall and liver problems etc. Crohn's disease symptoms include abdominal pain (which tends to be located in the right lower abdomen), ulcers in the mouth and GI lining, anal fissures, diarrhea which can contain pus, mucous, and blood, and undernourishment. Crohn's disease tends to present in scattered patches throughout the GI tract where healthy areas will be noted next to diseased areas. This will give a cobble-stone appearance during a scope of the GI lining. This is different than ulcerative colitis which tends to start in the rectum and migrate in a continuous fashion through the large intestine without any skipped areas. Nursing interventions for Crohn's Disease include: educating patient about this condition along with medication and diet regime and smoking cessation, monitoring nutrition status, assessing GI system by monitoring bowel sounds, bowel movements, monitoring administration of TPN (total parenteral nutrition) if ordered, providing ostomy education (if the patient has one). Medications used to treat Crohn's includes: 5-Aminosalicylates, corticosteroids, Immunosuppression drugs, etc. Quiz on Crohn's Disease: http://www.registerednursern.com/crohns-disease-nclex-questions/ Lecture Notes: http://www.registerednursern.com/crohns-disease-nclex-review/ Ulcerative Colitis NCLEX Review: https://www.youtube.com/watch?v=ICkNjkg6BHs GI NCLEX Lectures: https://www.youtube.com/playlist?list=PLQrdx7rRsKfVVQ5TeRY7Vy1Tq3pcGTDFO Subscribe: http://www.youtube.com/subscription_center?add_user=registerednursern Nursing School Supplies: http://www.registerednursern.com/the-ultimate-list-of-nursing-medical-supplies-and-items-a-new-nurse-student-nurse-needs-to-buy/ Nursing Job Search: http://www.registerednursern.com/nursing-career-help/ Visit our website RegisteredNurseRN.com for free quizzes, nursing care plans, salary information, job search, and much more: http://www.registerednursern.com Check out other Videos: https://www.youtube.com/user/RegisteredNurseRN/videos All of our videos in a playlist: https://www.youtube.com/watch?v=pAhHxt663pU&list=PLQrdx7rRsKfXMveRcN4df0bad3ugEaQnk Popular Playlists: NCLEX Reviews: https://www.youtube.com/playlist?list=PLQrdx7rRsKfWtwCDmLHyX2UeHofCIcgo0 Fluid & Electrolytes: https://www.youtube.com/playlist?list=PLQrdx7rRsKfWJSZ9pL8L3Q1dzdlxUzeKv Nursing Skills: https://www.youtube.com/playlist?list=PLQrdx7rRsKfUhd_qQYEbp0Eab3uUKhgKb Nursing School Study Tips: https://www.youtube.com/playlist?list=PLQrdx7rRsKfWBO40qeDmmaMwMHJEWc9Ms Nursing School Tips & Questions" https://www.youtube.com/playlist?list=PLQrdx7rRsKfVQok-t1X5ZMGgQr3IMBY9M Teaching Tutorials: https://www.youtube.com/playlist?list=PLQrdx7rRsKfUkW_DpJekN_Y0lFkVNFyVF Types of Nursing Specialties: https://www.youtube.com/playlist?list=PLQrdx7rRsKfW8dRD72gUFa5W7XdfoxArp Healthcare Salary Information: https://www.youtube.com/playlist?list=PLQrdx7rRsKfVN0vmEP59Tx2bIaB_3Qhdh New Nurse Tips: https://www.youtube.com/playlist?list=PLQrdx7rRsKfVTqH6LIoAD2zROuzX9GXZy Nursing Career Help: https://www.youtube.com/playlist?list=PLQrdx7rRsKfVXjptWyvj2sx1k1587B_pj EKG Teaching Tutorials: https://www.youtube.com/playlist?list=PLQrdx7rRsKfU-A9UTclI0tOYrNJ1N5SNt Personality Types: https://www.youtube.com/playlist?list=PLQrdx7rRsKfU0qHnOjj2jf4Hw8aJaxbtm Dosage & Calculations for Nurses: https://www.youtube.com/playlist?list=PLQrdx7rRsKfUYdl0TZQ0Tc2-hLlXlHNXq Diabetes Health Managment: https://www.youtube.com/playlist?list=PLQrdx7rRsKfXtEx17D7zC1efmWIX-iIs9

Standard & Isolation Precautions Nursing | Infection Control Contact, Droplet, Airborne PPE NCLEX

Nursing standard precautions and isolation precautions (nursing) review for the NCLEX exam. This review will cover standard precautions for infection control and transmission-based precautions (also called isolation precautions) such as contact, droplet, and airborne precautions. In addition, I will give you 3 isolation precaution mnemonics on how to remember droplet, contact, and airborne precautions. Standard precautions are precautions we follow with every patient at all times because we do not know if the person has an infectious disease or not. This is to protect ourselves and other patients. Standard precautions include: hand hygiene, wearing appropriate PPE (personal protective equipment), injection safety, respiratory/cough etiquette, patient room placement, handling of patient laundry and equipment. Isolation precautions are precautions we follow when we know or suspect a patient has an infectious disease. Depending on the type of infectious disease the patient has we will place the patient in the appropriate precautions. For example, if a patient has Pertussis (whooping cough) we will place the patient in droplet precautions. As the nurse, we would continue to follow standard precautions along with always wearing a surgical mask during patient contact. For a patient in airborne precautions, we would wear an N95 mask along with following standard precautions and place the patient in a special AIIR room (which is a negative pressure room). For a patient in contact precautions, we would always wear gloves and a gown during patient contact and follow standard precautions. Please watch the video for details on what diseases are included with each precaution and the mnemonics to help you remember the material. Iron Deficiency Anemia Treatment, Nursing, Pathophysiology, Symptoms w/ Nursing Interventions

12:52
Iron-deficiency anemia treatment, nursing interventions, pathophysiology, and symptoms NCLEX review lecture. Iron deficiency anemia is a type of anemia caused by low iron levels in the body. Anemia is a condition where there is a low amount of red blood cells or hemoglobin in the body. Red blood cells and hemoglobin are responsible for the transport of oxygen throughout the body to the tissues and organs. Iron is responsible for making hemoglobin. When iron levels are low in the body red blood cells and hemoglobin do not work properly which leads the patient to feel symptoms of lethargy, pallor, tachycardia, weird food cravings (ice, clay, dirt), inflammation of the tongue (glossitis), nail changes such as koilonychia (spoon-shape of the nails) etc. Treatment for iron-deficiency anemia includes iron supplementation, increasing iron in the diet with foods rich in iron (such as egg yolks, red meats, poultry, shellfish, fish, legumes, seeds etc), and prevention. The causes of iron-deficiency anemia include poor intake of iron (as with pregnancy, vegetarian diet, or malnourishment), blood loss (GI bleed, menstruation), or absorption issues (as with Celiac disease, GI surgery etc). It is diagnosed with complete blood count (CBC), iron level, or blood smear to assess the RBC which may appear hypochromic (pale) and microcytic (small). Don't forget to take the free NCLEX review quiz.
