Nursing standard precautions and isolation precautions (nursing) review for the NCLEX exam. This review will cover standard precautions for infection control and transmission-based precautions (also called isolation precautions) such as contact, droplet, and airborne precautions. In addition, I will give you 3 isolation precaution mnemonics on how to remember droplet, contact, and airborne precautions. Standard precautions are precautions we follow with every patient at all times because we do not know if the person has an infectious disease or not. This is to protect ourselves and other patients. Standard precautions include: hand hygiene, wearing appropriate PPE (personal protective equipment), injection safety, respiratory/cough etiquette, patient room placement, handling of patient laundry and equipment. Isolation precautions are precautions we follow when we know or suspect a patient has an infectious disease. Depending on the type of infectious disease the patient has we will place the patient in the appropriate precautions. For example, if a patient has Pertussis (whooping cough) we will place the patient in droplet precautions. As the nurse, we would continue to follow standard precautions along with always wearing a surgical mask during patient contact. For a patient in airborne precautions, we would wear an N95 mask along with following standard precautions and place the patient in a special AIIR room (which is a negative pressure room). For a patient in contact precautions, we would always wear gloves and a gown during patient contact and follow standard precautions. Presentation Aims: • Accessing and Navigating the Patient Centric Documents Window • Completing a Document Template: Mental Health Care Plan • Adding the completed document to the Patient Record in the Clinical Software. Closing Summary: • Patient Centric Documents can be created and modified. • When finished completing it, Copy to Clipboard. • This will also save the answers in the DCP for future reloading. • Paste the document into the Clinical Software Letter/Document Editor and amend further if necessary. • This finalised document is kept in Outbound Letters/Correspondence Out. Transcript: Hi, and welcome to another video where I am demonstrating the document feature of the DCP. The DCP enables an advanced templating system to create complex documents based on patient information. I will just go through and demonstrate one particular template: the mental health healthcare plan. Then, you will perhaps understand this better than if I go through a complete instruction on how it came about, and how it works. So we're just going to do a Mental Health Care Plan. So with the patient open, we have a Mental Health Care Plan prompt in the MBS Items section. Now to access the Patient Centric Document feature, go to Functions [button] and the very first one: Patient Centric Documents. When you click on that, it will open the Patient Centric Document form [window]. We have here on the left the list of Patient Centric Documents, we are interested in the Mental Health Care Plans. We double click on that, and it opens up on the right. Now the initial display, is in the Editor [tab], so you don't need to edit the document. But there are instructions on the top of the editor and that's why it opens on editor just so you can read the instructions in red. That will tell you to open Service Providers tab, set the Psychologist, use the Questionnaire tab, switch to the Preview tab and then copy and paste into the clinical software. So if we do that, we go to the Service Providers tab. To select the Service Provider, you have to click on the category, the Psychologist and then click on the drop-down and select one from the list. Click away and we'll set the Psychologist. You can clear it by clicking away out clicking the clear button. So we will select a Psychologist and go to the Questionnaire tab. Now the Questionnaire tab, contains the meat of the project where we're going to create a complex document based on these patient’s documented history. In this film, an on-call psychiatrist is assessing a young man who has been referred urgently by his GP. The psychiatrist takes a history in which she elicits persecutory delusions, third person auditory hallucinations, running commentary, thought insertion, and somatic hallucinations. She then makes a risk assessment, takes a drug history and assesses risk. The patient is clearly suffering from a psychotic disorder and the most likely diagnosis is schizophrenia. Differential diagnoses would include a drug-induced psychosis. Please note that this video has been made by the University of Nottingham for teaching purposes. The psychiatrist is a real psychiatrist but the patient is played by an actor.

How SMART are your goals? How to recognize and write SMART goals for change.

Do you want to make a successful change? Learn how to develop SMART goals. How SMART are your goals? When you are developing goals, whether they are for personal learning or achievement, clients or care plans you need to be sure they are SMART. Writing SMART goals takes some time, but it increases your chances of success because you can clearly see and understand where you are headed. SMART is an acronym that stands for Specific, Measurable, Attainable, Relevant/Realistic and Time-limited. This concept has been used for more than three decades in business, psychology and education literature to promote behavior change. It is an evidence-based approach to promoting practice changes. Each component of SMART goals in promoting performance improvements has been studied and supported by literature. To be specific your goal needs to be clear and detailed enough for everyone to understand it. To be measurable your goal needs to be quantifiable. Make sure you avoid being vague. Ask yourself, How will you measure the results? What changes will you see? To be attainable you need to be able to achieve the goal. It needs to be realistic given your current abilities and resources. To be relevant your goal needs to be linked to your role in the work setting or your personal life if it is a personal goal. Some forms of this acronym replace "relevant" with "realistic." It is also important to consider if each component of your goal is realistic. Placing a time limit on the goal is important because deadlines motivate you and help you evaluate your progress. Again, think about your deadline and make sure it is realistic. Here is an example of a common goal I hear. Is this goal SMART? "To lose weight." Nope -- It's not SMART. How much weight do you want to lose? When do you want to lose it? This goal may be Relevant to the person but it is not Specific, Measurable or Time-limited. You may think it is attainable, but even if you have the needed skills and resources to lose weight it is going to be hard to achieve your goal with it written like this. It is too vague so it will not help to motivate you. How about this goal? "To lose 30 pounds in the next two weeks." Wow! 30 pounds in only two weeks? That is not healthy or realistic according to current standards! I would say that in most situations that is not an attainable goal (unless maybe you are having it cut off). Lets try again. How about "To lose 10 pounds in the next six weeks." I would say it is specific and measurable. To assess if it is attainable think about how you are planning to lose weight. Through healthy eating and exercise this goal would be attainable because it means losing less than 2 pounds per week, which is in line with current recommendations for sustainable weight loss. Weight loss is relevant to an overall healthy lifestyle for many people. The goal is time limited to two weeks, which sets a deadline for evaluation. YAY! It is a SMART goal! Let's look at a some learning goals. Are there any SMART goals? 1. To accurately perform vital signs. 2. To improve hand washing techniques before the next clinical day. 3. To learn and perform all assigned assessments for the semester in one clinical day. 4. To perform a head to toe assessment according to the guidelines in Jarvis (2009) by the end of the clinical placement. They are all relevant to the role of a student nurse in the clinical setting. #1 does not have a time limit. When will you perform vital signs? So it is not SMART. #2 is not measurable. What kind of improvement are you aiming for? So it is not SMART. #3 is not attainable. So it is not SMART. #4 is Specific, Measurable, Attainable, Relevant and Time-limited. YAY!! Do you see the difference? Finally lets look at some goals for a client who has just had a newborn baby and wants to breastfeed. Which one is SMART enough for the care plan? #1 is not realistic because she will need to wake up in the night to feed the baby. So it is not SMART. Remove it from the care plan! #2 is Specific, Measurable, Attainable, Relevant and Time-limited. YAY!! #3 is not time-limited. So it is not SMART. Add a realistic time frame and it can stay in the care plan. #4 is Specific, Measurable, Attainable, Relevant and Time-limited. YAY!! For more information about learning goals for nurses visit the College of Nurses website and use the worksheet in this document to help you formulate a SMART learning goal. Six cardinal fields of gaze assessment in nursing, also called the six cardinal positions of gaze, helps us to assess how the six extraocular eye muscles are working. In addition, this assessment allows us to check cranial nerves III, IV, and VI during a neuro assessment, cranial nerve assessment, or head-to-toe assessment. The eye muscles that the six cardinal fields of gaze will check include the following: -Superior Rectus -Superior Oblique -Lateral Rectus -Medial Rectus -Inferior Oblique -Inferior Rectus During the assessment, we'll ask the patient to move the eyes in the upper right, lower left, left upper, right lower, and then left to right positions. We're looking for a smooth motion in the eyes as they change direction, which indicates normal movement. If they eyes jerk or shake, that could indicate nystagmus. Again, this can also be used as a cranial nerves 3, 4, and 6 examination.
