Normal blood pressure is important for proper blood flow to the body’s organs and tissues. The force of the blood on the walls of the arteries is called blood pressure. Blood pressure is measured both as the heart contracts, which is called systole, and as it relaxes, which is called diastole. Normal blood pressure is considered to be a systolic blood pressure of 115 millimeters of mercury a diastolic pressure of 70 millimeters of mercury (stated as "115 over 70"). If an individual were to have a consistent blood pressure reading of 140 over 90, he would be evaluated for having high blood pressure. If left untreated, high blood pressure can damage important organs, such as the brain and kidneys, as well as lead to a stroke. High blood pressure is a common disease in which blood flows through blood vessels (arteries) at higher than normal pressures. Measuring Blood Pressure Blood pressure is the force of blood pushing against the walls of the arteries as the heart pumps blood. High blood pressure, sometimes called hypertension, happens when this force is too high. Health care workers check blood pressure readings the same way for children, teens, and adults. They use a gauge, stethoscope or electronic sensor, and a blood pressure cuff. With this equipment, they measure: Systolic Pressure: blood pressure when the heart beats while pumping blood Diastolic Pressure: blood pressure when the heart is at rest between beats Health care workers write blood pressure numbers with the systolic number above the diastolic number. For example: 118/76 mmHg People read "118 over 76" millimeters of mercury. Normal Blood Pressure: Normal blood pressure for adults is defined as a systolic pressure below 120 mmHg and a diastolic pressure below 80 mmHg. It is normal for blood pressures to change when you sleep, wake up, or are excited or nervous. When you are active, it is normal for your blood pressure to increase. However, once the activity stops, your blood pressure returns to your normal baseline range. Blood pressure normally rises with age and body size. Newborn babies often have very low blood pressure numbers that are considered normal for babies, while older teens have numbers similar to adults. Abnormal Blood Pressure Abnormal increases in blood pressure are defined as having blood pressures higher than 120/80 mmHg. The following table outlines and defines high blood pressure severity levels. The ranges in the table are blood pressure guides for adults who do not have any short-term serious illnesses. People with diabetes or chronic kidney disease should keep their blood pressure below 130/80 mmHg. Although blood pressure increases seen in prehypertension are less than those used to diagnose high blood pressure, prehypertension can progress to high blood pressure and should be taken seriously. Over time, consistently high blood pressure weakens and damages your blood vessels, which can lead to complications. Types of High Blood Pressure There are two main types of high blood pressure: primary and secondary high blood pressure. Primary High Blood Pressure Primary, or essential, high blood pressure is the most common type of high blood pressure. This type of high blood pressure tends to develop over years as a person ages. Secondary High Blood Pressure Secondary high blood pressure is caused by another medical condition or use of certain medicines. This type usually resolves after the cause is treated or removed. Blood pressure (BP) is the pressure exerted by circulating blood upon the walls of blood vessels. When used without further specification, "blood pressure" usually refers to the arterial pressure in the systemic circulation. It is usually measured at a person's upper arm. Blood pressure is usually expressed in terms of the systolic (maximum) pressure over diastolic (minimum) pressure and is measured in millimeters of mercury (mm Hg). It is one of the vital signs along with respiratory rate, heart rate, oxygen saturation, and body temperature. Normal resting blood pressure in an adult is approximately 120/80 mm Hg. Blood pressure varies depending on situation, activity, and disease states. It is regulated by the nervous and endocrine systems. Blood pressure that is low due to a disease state is called hypotension, and pressure that is consistently high is hypertension. Both have many causes which can range from mild to severe. Both may be of sudden onset or of long duration. Long term hypertension is a risk factor for many diseases, including kidney failure, heart disease, and stroke. Long term hypertension is more common than long term hypotension in Western countries. Long term hypertension often goes undetected because of infrequent monitoring and the absence of symptoms. Accessed June 14, 2016. ======================================= 🌡 What Is Systolic & Diastolic Pressure & The Ideal Numbers ======================================= I talk a lot about the importance have having proper blood pressure numbers not only because I have a family history of hypertension and heart disease… But also because high blood pressure is known as the “silent killer”. There are no real symptoms to warn you. And surprisingly, ⅓ of adults over the age of 20 have high blood pressure. So the problem starts early. And the older we get, the worse the numbers become. For example, over 60% of the population over the age of 65 has high blood pressure. For women, it’s even worse because of menopause.2,3 Sadly, 70% of the population doesn’t even know they have high blood pressure, which greatly increases your risk of death by 43%, in addition to other health ailments.2,3 Now, before I give you the ideal blood pressure numbers, let’s first talk about what the numbers even mean. The top number, known as Systolic, refers to the amount of pressure in your arteries during the contraction of your heart muscle. The bottom number, called Diastolic, refers to your blood pressure when your heart muscle is between beats. Both numbers are important in determining the state of your heart health. Numbers greater than the ideal range, indicate that your heart is working too hard to pump blood to the rest of your body. This obviously puts more stress on your heart, as well as your veins, blood vessels and arteries. In fact, this puts more pressure on your brain as well! So, what’s the ideal range?... Here’s a quick chart on blood pressure levels. As you can see, anything in the red that’s over 140/90 is considered “high” blood pressure. The yellow zone is better, but still a problem. Ideal range is in the green, which is LESS than 120/80. I prefer around 110/70 or 115/75. Now, the reason I like it lower is because studies1 have found that lower than 120 mmHG Systolic (top number) caused 43% lower risk of cardiovascular death 38% lower “heart failure” 27% in less deaths 25% lower deaths from “first” heart attacks 17% lower heart attacks 11% lower strokes So lower is better, as long as you feel fine… and preferably done naturally - through specific diet, exercise and supplements. So if you want to live longer, lowering your blood pressure levels to 120/80 or lower, is a huge benefit and a necessity for total health. I would skip the drugs and try to do it naturally with diet, exercise and supplements. And this is really important if you’re over the age of 40 because lots of clinical evidence that aging is a major contributing factor to hypertension because of the change in your hormones. Like I said, this is a very important topic to me personally, because of my family history. An introductory video explaining how to manually measure blood pressure. In this video, we are sharing with you the auscultatory method of determining systolic and diastolic arterial pressures. The cuff is placed above the antecubital artery and the cuff is above the upper arm. The stethoscope is placed near the antecubital artery and no sound will be hear when there is enough pressure to close off the artery. When the pressure is slowly released, the sound of blood jetting through the vessel by the vibrations of the wall is called the Korotkoff sounds. The first sound is also the systolic pressure. The diastolic pressure is when a muffled quality occurs. Please note that the sound is often overestimated because sound can still be heard even after deflation of the cuff. This method is not 100% accurate but will give values within the 10% range. Blood pressure (BP) is the pressure exerted by circulating blood upon the walls of blood vessels. When used without further specification, "blood pressure" usually refers to the arterial pressure in the systemic circulation. It is usually measured at a person's upper arm. Blood pressure is usually expressed in terms of the systolic (maximum) pressure over diastolic (minimum) pressure and is measured in millimeters of mercury (mm Hg). It is one of the vital signs along with respiratory rate, heart rate, oxygen saturation, and body temperature. Normal resting blood pressure in an adult is approximately 120/80 mm Hg. Blood pressure varies depending on situation, activity, and disease states. It is regulated by the nervous and endocrine systems. Blood pressure that is low due to a disease state is called hypotension, and pressure that is consistently high is hypertension. Both have many causes which can range from mild to severe. Both may be of sudden onset or of long duration. Long term hypertension is a risk factor for many diseases, including kidney failure, heart disease, and stroke. Long term hypertension is more common than long term hypotension in Western countries. Long term hypertension often goes undetected because of infrequent monitoring and the absence of symptoms. This video is about Making sense of your echo report Lets go through the information that the report will contain - When you have an echo you will also have a single lead ECG done at the same time and this is to allow the tallying of mechanical activity with electrical activity so usually the first thing on the echo report will be some mention of the heart rhythm and rate on the ECG during the scan. Next: The left atrium: this structure is the left atrium and what we have started realising is that the left atrium gives you great insight into your overall heart health. Healthy hearts usually have normal sized left atria. What is normal? About 4 cm across. What does it mean if the left atrium is enlarged…usually it means that the heart is having to work harder than it needs to. Also patients who have AF will have larger left atria.Is it dangerous - no not at all but it does mean that you just need to work out why the heart is working harder and manage accordingly. The next structure is the Mitral valve. This is this valve here. Now most normal valves are described as being thin and mobile. if the valve is described as being thickened, calcified and restricted then that tells you the valve is structurally abnormal. Either it has been subjected to wear and tear from old age or it has been damaged due to an infection such as rheumatic fever. Functionally valves can still be normal or they can be thickened to the point that they are narrowed and impede blood from passing through in which case they are described as being stenosed or stenosis. The left ventricle. This is the most important chamber of the heart because it is responsible for pumping blood to the whole of the body and patients who have a weak left ventricle have a much worse prognosis than patients who dont. So the echo will firstly comment on the size of the left ventricle. Normal size is dependant on body surface area but in general you want the left ventricle at its widest to be no more than 5.5 cm in women and 6 cm across in men. if the ventricle looks bigger than that then you worry that it is being stretched either because of a leaky valve or because it was damaged in some way. The problem is the more it stretches the more likely it is to lose its stretchability in the long run and therefore it can weaken. So there will be some comment on the echo report on the size of the ventricle. As long as it is not dilated you can feel very comfortable and reassured. The second thing is a mention of something called regional wall motion abnormality. Different parts of the left ventricle receive blood from different blood vessels and therefore if a patient has had a heart attack because one of the blood vessels has blocked, then that part of heart muscle will die and therefore not contract normally whereas the other normal parts will and this is described as a regional wall motion abnormality. So if you have no regional wall motion abnormality then you dont have any substantial damage or scar in your heart. There will also be a mention of function of the heart… how much blood does it pump to with every beat and this is called the ejection fraction. This is a very crude indicator which is usually visually assessed and the normal value is about 60%. if the heart is not pumping out as much blood as you would expect, it is described as left ventricular impairment. Mild left ventricular impairment is if the ejection fraction is 45-55%, moderate is 35-45% and severe is less than 35%. Now many people get very worried because they may have had 2 echo and they say well last time it was described as 62% and now it is 58% - does this mean that it has gotten worse and the answer is definitely not… the ejection fraction is a very crude measure and variations of 5-10% are completely acceptable. Finally there will be a mention of the thickness of the walls of the feft ventricle. These should usually be no more than 12mm thick and if they are thicker than that it means that either the heart is working harder than it should (high blood pressure) or there is some form of muscle disease which has caused the heart muscle to be thicker than you’d expect and thickened walls are described as left ventricular hypertrophy. if one wall is thickened it is described as asymmetrical and if all walls are thicked it is described as concentric or symmetrical

It is the product of STROKE VOLUME – the amount of blood pumped in one heartbeat, and HEART RATE – the number of beats in one minute. An INcrease in either stroke volume or heart rate results in INcreased cardiac output, and vice versa. For example, during physical exercises, the heart beats faster to put out more blood in response to higher demand of the body. It is noteworthy that the ventricles do NOT eject ALL the blood they contain in one beat. In a typical example, a ventricle is filled with about 100ml of blood at the end of its load, but only 60ml is ejected during contraction. This corresponds to an EJECTION fraction of 60%. The 100ml is the end-DIASTOLIC volume, or EDV. The 40ml that remains in the ventricle after contraction is the end-SYSTOLIC volume, or ESV. The stroke volume equals EDV minus ESV, and is dependent on 3 factors: contractility, preload, and afterload. Contractility refers to the force of the contraction of the heart muscle. The more forceful the contraction, the more blood it ejects. PRELOAD is RELATED to the end-diastolic volume. Preload, by definition, is the degree of STRETCH of cardiac myocytes at the end of ventricular filling, but since this parameter is not readily measurable in patients, EDV is used instead. This is because the stretch level of the wall of a ventricle INcreases as it’s filled with more and more blood; just like a balloon - the more air it contains, the more stretched it is. According to the Frank-Starling mechanism, the greater the stretch, the greater the force of contraction. In the balloon analogy, the more inflated the balloon, the more forceful it releases air when deflated. AFTERLOAD, on the other hand, is the RESISTANCE that the ventricle must overcome to eject blood. Afterload includes 2 major components: - Vascular pressure: The pressure in the left ventricle must be GREATER than the systemic pressure for the aortic valve to open. Similarly, the pressure in the right ventricle must exceed pulmonary pressure to open the pulmonary valve. In hypertension for example, higher vascular pressures make it more difficult for the valves to open, resulting in a REDUCED amount of ejected blood. - Damage to the valves, such as stenosis, also presents higher resistance and leads to lower blood output. 6 Ways to Lower Your Blood Pressure ( Hypertension ) 1. Increase activity and exercise more In a 2013 study, sedentary older adults who participated in aerobic exercise training lowered their blood pressure by an average of 3.9 percent systolic and 4.5 percent diastolic (4). These results are as good as some blood pressure medications. As you regularly increase your heart and breathing rates, over time your heart gets stronger and pumps with less effort. This puts less pressure on your arteries and lowers your blood pressure. How much activity should you strive for? A 2013 report by the American College of Cardiology (ACC) and the American Heart Association (AHA) advises moderate- to vigorous-intensity physical activity for 40-minute sessions, three to four times per week (5). If finding 40 minutes at a time is a challenge, there may still be benefits when the time is divided into three or four 10- to 15-minute segments throughout the day (6). The American College of Sports Medicine (ACSM) makes similar recommendations (7). But you don’t have to run marathons. Increasing your activity level can be as simple as: using the stairs walking instead of driving doing household chores gardening going for a bike ride playing a team sport Just do it regularly and work up to at least half an hour per day of moderate activity. One example of moderate activity that can have big results is tai chi. A 2017 review on the effects of tai chi and high blood pressure shows an overall average of a 15.6 mm Hg drop in systolic blood pressure and a 10.7 mm Hg drop in diastolic blood pressure, compared to people who didn’t exercise at all (8). 2. Lose weight if you’re overweight If you’re overweight, losing even 5 to 10 pounds can reduce your blood pressure. Plus, you’ll lower your risk for other medical problems. A 2016 review of several studies reported that weight loss diets reduced blood pressure by an average of 3.2 mm Hg diastolic and 4.5 mm Hg systolic (11). 3. Cut back on sugar and refined carbohydrates Many scientific studies show that restricting sugar and refined carbohydrates can help you lose weight and lower your blood pressure. A 2010 study compared a low-carb diet to a low-fat diet. The low-fat diet included a diet drug. Both diets produced weight loss, but the low-carb diet was much more effective in lowering blood pressure. The low-carb diet lowered blood pressure by 4.5 mm Hg diastolic and 5.9 mm Hg systolic. The diet of low-fat plus the diet drug lowered blood pressure by only 0.4 mm Hg diastolic and 1.5 mm Hg systolic (12). A 2012 analysis of low-carb diets and heart disease risk found that these diets lowered blood pressure by an average of 3.10 mm Hg diastolic and 4.81 mm Hg systolic (13). Another side effect of a low-carb, low-sugar diet is that you feel fuller longer, because you’re consuming more protein and fat. 4. Eat more potassium and less sodium Increasing your potassium intake and cutting back on salt can also lower your blood pressure (14). Potassium is a double winner: It lessens the effects of salt in your system, and also eases tension in your blood vessels. However, diets rich in potassium may be harmful to individuals with kidney disease, so talk to your doctor before increasing your potassium intake. It’s easy to eat more potassium — so many foods are naturally high in potassium. Here are a few: low-fat dairy foods, such as milk and yogurt fish fruits, such as bananas, apricots, avocados, and oranges vegetables, such as sweet potatoes, potatoes, tomatoes, greens, and spinach Note that individuals respond to salt differently. Some people are salt-sensitive, meaning that a higher salt intake increases their blood pressure. Others are salt-insensitive. They can have a high salt intake and excrete it in their urine without raising their blood pressure (15).
