Upon examination of the throat, the uvula is swollen, pale, and somewhat translucent (uvular hydrops). If greatly enlarged, the uvula might rest on the tongue and move in and out with respiration. There might be an associated rash or a history of exposure to phsical stimuli, allergens, or a recurrent seasonal indicence. What to do: Because of the known association of uvular with hypopharyngeal edema, watch for signs of airway compromise. If a patient complains of resiratory difficulty or breathes with stridor, commence treatment with intravenous lines and intubation and cricothyrotomy equipment at the bedside, and a crosstable lateral soft tissue neck x ray to rule out epiglottic swelling. If there is no acute respiraory difficulty, ask about precipitating events. Consider foods, drugs, physical agents, inhalants, insect bites and hereditary angioedema. When fever, sore throat and pharyngeal injection are present, culture the throat with a rapid strep screen and give an antibiotic that covers Haemophilus influenzae (e.g., Biaxin, Augmentin, Bactrim). It is reasonable to obtain a complete blood count with a manual differential to demonstrate eosinophilia to support the possibility of an allergic reaction or a high leukocyte count with increased granulocytes and bands to support a bacterial infection. Initially the patient should receive parenteral H1 and H2 antihistamines like hydroxazine 50-100mg im or diphenhydramine 25-50mg iv along with cimetidine 300mg iv or po or ranitidine 50mg iv or 150mg po. More severe cases should receive repeated doses of epinephrine 0.3ml of 1:1000 sq every 20 minutes x3. Nebulized isomeric or racemic epinephrine or albuterol are also effective. Parenteral corticosteroids like SoluMedrol 125mg iv are also typically used, although efficacy remains umproven. If there is a history of recurrent episodes of edema and there is a family history of the same, consider ordering a C4 complement level or C'1 esterase inhibitor levels as a screening test for hereditary angioedema. In this condition, the edema often involves the uvula and soft palate together. Uvular decompression may be useful in patients that are resistant to medical therapy or whose symptoms progress rapidly. This procedure consists of grasping the uvula with forceps and either making several lacerations with a sterile needle or snippint the distal centimeter as a patial uvulectomy. All patient s should be observed for an adequate period of time to insure that there is either improvement or no further worsening of the swelling before being discharged home. Upon discharge, they should receive 4-5 days of H1 and H2 blockers and steroids if required.

Structurally, most bacteria consist of a cell membrane surrounded by a cell wall and, for some bacteria, an additional outer layer. Internal to the cell membrane is the cytoplasm which contains ribosomes, a nuclear region and in some cases granules and/or vesicles. Depending on the bacterial species, a number of different external structures may be found such as a capsule, flagella and pili. In gram negative bacteria, the gap between the cell membrane and the cell wall is known as the periplasmic space. Most gram positive bacteria do not possess a periplasmic space but have only periplasm where metabolic digestion occurs and new cell peptidoglycan is attached. Peptidoglycan, the most important component of the cell wall, is a polymer made of N-acetyl muramic acid alternating with N-acetyl glucosamine which are cross-linked by chains of four amino acids. The function of the bacterial cell wall is to maintain the characteristic shape of the organism and to prevent the bacterium from bursting when fluid flows into the organism by osmosis. Synthesis of peptidoglycan and ultimately the bacterial cell wall occurs in a number of stages. One of the first stages is the addition of 5 amino acids to N-acetyl muramic acid. Next, N-acetyl glucosamine is added to the N-acetyl muramic acid to form a precursor of peptidoglycan. This peptidoglycan precursor is then transported across the cell membrane to a cell wall acceptor in the periplasm. Once in the periplasm, the peptidoglycan precursors bind to cell wall acceptors, and undergo extensive crosslinking. Two major enzymes are involved in crosslinking: transpeptidase and D-alanyl carboxypeptidase. These enzymes are also known as penicillin binding proteins because of their ability to bind penicillins and cephalosporins. Eventually, several layers of peptidoglycan are formed all of which are crosslinked to create the cell wall. Gram positive bacteria have many more layers than gram negative bacteria and thus have a much thicker cell wall. Beta-lactam antibiotics include all penicillins and cephalosporins that contain a chemical structure called a beta-lactam ring. This structure is capable of binding to the enzymes that cross-link peptidoglycans. Beta-lactams interfere with cross-linking by binding to transpeptidase and D-alanyl carboxypeptidase enzymes, thus preventing bacterial cell wall synthesis. By inhibiting cell wall synthesis, the bacterial cell is damaged. Gram positive bacteria have a high internal osmotic pressure. Without a normal, rigid cell wall, these cells burst when subjected to the low osmotic pressure of their surrounding environment. As well, the antibiotic-penicillin binding protein complex stimulates the release of autolysins that are capable of digesting the existing cell wall. Beta-lactam antibiotics are therefore considered bactericidal agents. Bacterial resistance to beta-lactam antibiotics may be acquired by several routes. One of the most important mechanisms is through a process known as transformation. During transformation, chromosomal genes are transferred from one bacterium to another. When a bacterium containing a resistance gene dies, naked DNA is released into the surrounding environment. If a bacterium of sufficient similarity to the dead one is in the vicinity, it will be able to uptake the naked DNA containing the resistance gene. Once inside the bacterium, the resistance gene may be transferred from the naked DNA to the chromosome of the host bacteria by a process known as homologous transformation. Over time, the bacterium may acquire enough of these resistance genes to result in a remodelling of the segment of the host DNA. If this remodelled DNA segment codes for cross-linking enzymes (i.e. penicillin binding proteins), the result is the production of altered penicillin binding proteins. These altered penicillin binding proteins can still cross-link the peptidoglycan layers of the cell wall but have a reduced affinity for beta-lactam antibiotics thus rendering the bacterium resistant to the effects of penicillin and other beta-lactam agents. This transfer process has resulted in penicillin-resistant S. pneumoniae through the acquisition of genes from other naturally occurring penicillin-resistant Streptococcus species. A second important mechanism by which bacteria become resistant to beta-lactam antibiotics is by the production of enzymes capable of inactivating or modifying the drug before it has a chance to exert its effect on the bacteria. I believe there was no "Fall", and the only innocence we're returning to is the innocence we felt as children. Evolutionary biology is showing us that we are born from something more complex than the snap of God's fingers, and whatever our genesis, it includes severe conflict and competition. And this competition goes deep into our beings. For example, the book Sperm Wars by Robin Baker shows that a good portion of the sperm in a man's ejaculate is designed to fight and kill-off sperm from another male in the woman's cervix. Reflection on this biological fact unwinds for me so much Christian theology, especially the premise referenced above in JP2's two books. They're built on Jesus words in Mathew 19:8... that before "the fall" man and woman (Adam and Eve) were one in marriage, and that it was a result of the fall we need to return to how it was "in the beginning." Jesus is the supposed Redeemer who will deliver us to the harmony of how it was "in the beginning," but we know that the beginning was an awful place. Our physical bodies tell us that millions of years ago reproduction was a gang bang nightmare where after the men nearly fought to the death over who'd have the woman first, even then their sperm had to fight like gladiators. Is this God's design from 6000 years ago? I don't think so. And it is because I can't reconcile traditional Catholic theology with science and its reasonable methods my conscience and prayer-life are being pushed to find a new understanding. Today I argue that the Christian mission is not to have us return to some mythical Eden, but to have us evolve into our potential... to go where we have never been before. It's no longer Jesus as savior delivering us back to Eden's innocence, but Jesus as older, wiser friend inviting us over for dinner, and in that conversation, we discover for ourselves those areas that need maturity and purity. It's a new Christology, and maybe it converts much of Jesus into allegory, but it connects the reality of evolutionary science with our human experience... and by that, I mean that deep quiet desire we all have to want to be more than we are and to live up to our potential. Jesus lived up to his potential, and modeling our lives after his brings out the virtues, which are universal and are not exclusive to any religion. [Side note: it just occurred to me that the virtues are nothing more than God's personality traits. This might be worthy of meditation.] As a complement to this way of looking at life, I believe one's own personal physical nature is to be in harmony with the mind and spirit. Medicines are important in this as they help heal us when we're sick, but the contraceptive pill is not a medicine. It is not fixing something that is broken, it is breaking something that is working just fine (i.e., female fertility). This is why I'm opposed to it, and I think there's a better way for couples to manage their reproduction. And, I think people should know the facts and risks regarding how "The Pill" may cause embryonic abortions. This alone should bother a lot of Christians, especially those precious few who still believe that although we are "fallen," that's not an excuse to be passive, but that we still have an obligation to try to live up to our potential.

Pneumonia is also known to be the 6th leading cause of death worldwide. The most common cause of a bacterial pneumonia is Streptococcus pneumoniae. In this form of pneumonia, there is usually an abrupt onset of the illness with shaking chills, fever, and production of a rust-colored sputum. The infection spreads into the blood in 20% - 30% of cases (known as sepsis), and if this occurs, 20% - 30% of these patients die. Antibiotics as treatment Antibiotics often used in the treatment of this type of pneumonia include penicillin, amoxicillin and clavulanic acid (Augmentin), and macrolide antibiotics including erythromycin, azithromycin, and clarithromycin.
