And they’re scattered throughout the brain that typically represents a metastatic cancer. That’s one that has seeded the brain in a number of areas within it, from some source outside of the brain,” says Dr. Mantz. Determining the true nature is critical in treating the cancer. “A diagnosis of a metastatic cancer to the brain can impact subsequent therapies we would do for those patients that may look very different from how we treat a patient who has a primary brain tumor,” says Dr. Mantz. Some primary cancers are slow-growing and less harmful. Cancers that traveled to the brain tend to be more aggressive and may have spread to other parts of the body as well. “That discovery will inform subsequent decision for other types of therapies such as chemotherapy, radiation therapy, or perhaps surgery,” says Dr. Mantz. Neurons are probably the more important and better-known cells of the brain as they carry out the brain’s communication function. Less known are some trillions of support cells called glia, or glial cells. The glia may not be the center of attention, but without them, neuron functions would be impossible. The major types of glial cells in the brain include: oligodendrocytes, microglia, and astrocytes. Oligodendrocytes are specialized cells with long processes that wrap around axons of neurons to form the myelin sheath. The myelin sheath acts like an electrical insulator around a wire. It helps to speed up the electrical signals that travel down an axon. Without oligodendrocytes, an action potential would propagate 30 times slower! Microglia are special macrophages found only in the central nervous system. They wander through the brain tissue, phagocytizing dead, injured cells and foreign invaders. High concentrations of microglia are an indication of infection, trauma or stroke. Astrocytes are the most abundant and functionally diverse glia. These star-shaped glial cells provide supportive frameworks to hold neurons in place. They provide neurons with nutrients such as lactate. They also produce growth factors that promote neuron growth and synapse formation. It’s been suggested that astrocytes can control how a neuron is built by directing where to make synapses or dendrites. Through their processes, known as perivascular feet, astrocytes induce endothelial cells of blood vessels to form tight junctions. These tight junctions are the basis of the blood brain barrier that restricts the passage of certain substances from the bloodstream to the brain tissue. Astrocytes help to maintain the chemical composition of the extracellular fluid. They express membrane transporters for several neurotransmitters such as glutamate, ATP and GABA, and help to remove them from synaptic spaces. Astrocytes also absorb potassium ions released by neurons at synapses. This helps to regulate potassium concentrations in the extracellular space. Abnormal accumulation of extracellular potassium is known to cause epileptic neuronal activity. Recently, it has been shown that astrocytes can also communicate electrically with neurons and modify the signals they send and receive. In a manner similar to neurons, astrocytes can release transmitters, called gliotransmitters, upon stimulation. This suggests that astrocytes maybe involved DIRECTLY in the communication functions of the brain. From the clinical viewpoint, neurons have little capacity for renewal and therefore rarely form tumors. On the contrary, glial cells are capable of dividing throughout life and are the primary source of brain tumors. Neurosurgeons at Boston Children's Hospital will remove a brain tumor employing functional mapping of the cortex on a 13-year-old pediatric patient during a live Webcast. The Webcast will feature Boston Children's intraoperative MRI system, known as the MR-OR, the first and only system of its kind at a pediatric hospital in the country. Developed by IMRIS, the iSPACE surgical imaging suite captures digital images through a unique, ceiling-mounted, movable MRI scanner that can be used to take high-resolution, real-time patient scans before, during and after a surgical procedure. This advanced technology allows surgeons to determine the extent of a tumor while the patient is undergoing surgery to ensure its accurate removal. "Unlike other intraoperative MR machines, the mobile MRI lets surgeons use their usual metal surgical tools because the unit is moved into the shielded garage when surgeons are operating," says Joseph R. Madsen, MD, a neurosurgeon in the Department of Neurosurgery at Boston Children's Hospital and associate professor of Surgery at Harvard Medical School. Dr. Madsen will operate on a patient with oligodendroglioma, a low-grade tumor arising from glial cells in the central nervous system. The tumor lies near motor and sensory areas of the brain, which will require electrocorticography and physiological tests to map the normal brain around the tumor before the surgery. Once the mapping has been completed, Dr. Madsen will then perform a microsurgical resection of the tumor. Brain tumors are the most common solid tumors in children—approximately 1,800 are diagnosed in the United States each year. Today, more than half of all children diagnosed with a brain tumor will be cured of the disease. The most effective form of treatment is the surgical removal of all or part of the tumor without jeopardizing any of the brain's critical functions. In order to decide which areas of the tumor can safely be removed, neurosurgeons use the technique of brain mapping. "The cutting edge of neurosurgery is to identify and remove as much of the undesirable pathologically damaging brain tissue without disturbing the functioning areas of the brain," says Dr. Madsen. "Through the use of physiological mapping and the MR-OR, we are able to achieve this and assure our patients the best possible surgical outcomes." Dr. Madsen will be assisted by neurologist Frank H. Duffy, MD, radiologist Caroline Robson, MB, ChB, and associate anesthesiologist-in-chief Mark A. Rockoff, MD. They will also serve as Webcast commentators, providing additional information about the procedure throughout the surgery. Moderating the live broadcast will be neurosurgeon Mark R. Proctor, MD. Neurosurgeon-in-chief R. Michael Scott, MD, will introduce the Webcast and offer insight into pediatric brain tumors. Liliana C. Goumnerova, MD, and Mark Kieran, MD, PhD, the surgical and medical directors of the Brain Tumor Clinic, a collaborative program with Dana-Farber Cancer Institute, will also be on-hand to discuss the neuro-oncological aspects of the procedure, while Craig D. It’s estimated that each year nearly 80,000 people are diagnosed with primary brain tumors and 100,000 with metastatic brain tumors. Nearly everybody is at risk for developing a brain tumor. Brain tumors can affect people from childhood to the last years of their lives. Men are slightly more affected than women and the causes of most brain tumors are not known. There are a number of unique challenges in treating brain tumors. One challenge is that primary tumors can have indistinct margins that are difficult to see. Another challenge is that the tissue around a brain tumor is uniquely important and may impact things like language, visual and motor function. The AMIGO Suite, opened in 2011 at Brigham and Women’s Hospital, is the Advanced Multimodality Image Guided Operating Suite. It's an NIH-funded national center which was developed with the goal of translating technological advances into improvements in surgical and interventional care for patients. In the AMIGO Suite, there is an intraoperative MRI scanner which can be brought in and out of the operating room during surgery to help surgeons visualize a patient’s tumor better. Image-guided surgery uses the information obtained from advanced imaging and translates that into the planning and execution of surgery by acquiring high resolution and specialty structural images of the brain and also functional images of the brain. These images can be registered to one another and then to the patient's head during surgery.
