Study: New Exercises Help Athletes Manage Dangerous Breathing Disorder

(Denver, CO) – Millions of young athletes experience a scary and potentially dangerous breathing disorder known as exercise-induced laryngeal obstruction (EILO). EILO, commonly misdiagnosed as asthma, is unresponsive to asthma medications. So doctors at National Jewish Health have developed new breathing techniques that are helping athletes control their EILO symptoms, even during high-intensity exercise when they tend to be most severe. “We found that if EILO patients can control and change the airflow through their mouth, they’re also able to control their throat,” said Tod Olin, MD, a pediatric pulmonologist at National Jewish Health who developed the exercises now known as the Olin EILOBI breathing techniques. “After teaching them these new techniques, 80 percent of patients reported them to be extremely helpful and two-thirds of patients were able to control their EILO symptoms by using them during exercise.” Dr. Olin developed the breathing exercises based on observations of patients’ airways during very intense exercise. He asks patients to pedal as fast as they can on a stationary bike while a small camera in their throat captures images of their vocal cords. In those with EILO, their vocal cords tighten and close the top of the airway, causing noisy, labored breathing. Patients spend several sessions learning to perform the techniques step-by-step. By mastering them, athletes are able to use these techniques when EILO symptoms strike, even if they’re out of breath and may be panicked. Dr. Olin says that’s what makes the new breathing techniques so helpful when traditional EILO exercises fall short. “Some of the traditional techniques that we use for EILO simply can’t be performed during high-intensity exercise.” he said. “Specifically when patients are asked to inhale through their nose, it’s simply not something you can do if you’re running as fast as you can.”

Their main danger comes from the damage they can do when they react with important cellular components such as DNA, or the cell membrane. Tip Drink plenty of water during and after physical exercise and before you use the sauna. Without adequate fluid intake, you put yourself at risk for dehydration and heat-related illnesses, such as heat exhaustion and heatstroke. You can compare the number against future measurements, helping to confirm when an attack may be imminent and determine if your medicine is working. Step 6: Know when to get help Go to the emergency room if you can't speak more than short phrases, are straining to breathe, or if your peak-flow reading is less than 50 percent of your personal best. AKA paradoxical vocal cord dysfunction, paradoxical vocal fold motion, and factitious asthma. Vocal cord dysfunction is a syndrome in which inappropriate vocal cord motion produces partial airway obstruction, leading to subjective respiratory distress. When a person breathes normally, the vocal cords move away from the midline during inspiration and only slightly toward the midline during expiration. However, in patients with vocal cord dysfunction, the vocal cords move toward the midline during inspiration or expiration, which creates varying degrees of obstruction. Patients with vocal cord dysfunction typically present with recurrent episodes of subjective respiratory distress that are associated with inspiratory stridor, cough, choking sensations, and throat tightness. Laryngospasm - is a subtype of vocal cord dysfunction, is a brief involuntary spasm of the vocal cords that often produces aphonia and acute respiratory distress. Laryngospasm is a common complication of anesthesia. Precipitating Factors Vocal cord dysfunction is associated with a variety of precipitating factors, but no clear unifying pathophysiology has been identified. Exercise is a common cause of vocal cord dysfunction. Exercise-induced vocal cord dysfunction is often misdiagnosed as exercise-induced asthma. It should be strongly considered in patients with dyspnea on exertion who have been diagnosed with exercise-induced asthma, particularly if they respond poorly to usual treatment with bronchodilators. Studies have reported associations between vocal cord dysfunction and multiple psychological conditions, including posttraumatic stress disorder, anxiety, depression, and panic attack. Anxiety disorders appear to be particularly common in adolescent patients with vocal cord dysfunction.However, associated depression and anxiety may also be consequences of persistent respiratory symptoms, rather than causes. Exposure to environmental and occupational irritants has been found to precipitate respiratory symptoms consistent with vocal cord dysfunction. Common airborne irritants associated with vocal cord dysfunction include ammonia, dust, smoke, soldering fumes, and cleaning chemicals. Studies have shown a clear temporal relationship between exposure and onset of symptoms. Postnasal drip associated with rhinosinusitis has been linked to airway hyperresponsiveness. A high prevalence of rhinosinusitis in patients with vocal cord dysfunction and case reports of resolution of vocal cord dysfunction symptoms with treatment suggest that rhinosinusitis may play a role in some patients. Gastroesophageal reflux disease (GERD) has been implicated in triggering vocal cord dysfunction. In some studies, a high prevalence of GERD was identified in patients with vocal cord dysfunction; however, treatment of GERD was only effective in decreasing vocal cord dysfunction in some patients. Neuroleptic drugs, specifically phenothiazines, may cause transient vocal cord dysfunction. This appears to be a focal dystonic reaction and is associated with extra-pyramidal signs, such as torticollis. Diagnostic Approaches The most valuable diagnostic tests for vocal cord dysfunction are pulmonary function testing with a flow-volume loop and flexible laryngoscopy. Other testing, such as measurement of arterial blood gases, may be useful in ruling out other possible diagnoses. PULMONARY FUNCTION TESTING Pulmonary function testing with a flow-volume loop is the most commonly used diagnostic test to confirm vocal cord dysfunction. In the flow-volume loop, it is typical for the expiratory loop to be normal and the inspiratory loop to be flattened, which is consistent with an extrathoracic upper airway obstruction Exercise flow-volume loops, performed in conjunction with exercise testing, may be useful in identifying patients with exercise-induced vocal cord dysfunction. FLEXIBLE LARYNGOSCOPY Flexible laryngoscopy is considered the diagnostic standard for vocal cord dysfunction. Direct observation of abnormal vocal cord movement toward the midline during inspiration or expiration confirms the diagnosis. Most patients with symptomatic vocal cord dysfunction will demonstrate the abnormal movement, and more than one half of patients who are asymptomatic will be diagnosed.Stimulating asymptomatic patients with panting, deep breathing, phonating, or exercising may increase the sensitivity of the test. Researchers at the University of Alabama at Birmingham's School of Medicine monitored the lung function of over 5,000 young men and women over 20 years between 1985 and 2006, and found that smoking pot for seven 'joint years' - which means 365 joints a year - does not alter lung function. He will go into detail about how people that have poor health or chronic health problems will have problems with exercising. An important factor that shows this correlation is the quantity of people's breath per minute. The medical norm for breathing at rest is 6 litres per minute. This is a very small amount and there would be barely any noticeable bodily movements when breathing. On the Normalbreathing.com homepage, it shows sick people breathe 2.5 times more than the medical norm or 15 litres per minute. This is a significant difference in air quantity and the type of health these people experience. People that breathe a lot less than the medical norm have about 60 seconds for the body-oxygen test. Due to their unconscious and light breathing patterns, they have a lot more oxygen in their body. This means a lot more energy and a higher capacity to do intensive physical exercise. Also, they naturally breathe with their nose even during intensive exercise. Additionally, they sleep around 4 hours or less. There are many other associated health benefits for having such a high level for the body-oxygen test. Sick people have around 12 seconds for the body-oxygen test. When they exercise they breathe with their mouth and have other problems. In turn, mouth breathing has drastic effects on body oxygen levels. If these individuals engage in light jogging, they need to use about 10 times more energy and oxygen than at rest. Also, they receive more CO2 in the body at this time. Furthermore, minute ventilation will be increased about 10 times. A regular person will breathe about 60 liters per minute when exercising. During exercise, their breathing will get slightly louder. Sick people breathe about 15 liters per minute at rest. While they breathe about 10 times more when exercising and that is about 150 liters per minute. This is the maximum a person can breathe. This rate dramatically reduces their ability to physically exercise. In order to improve endurance, breathing patterns from the whole day should be examined. Breathing exercises used by Dr. Artour Rakhimov include the Buteyko breathing technique. Dr. Buteyko was a leading Soviet physiologist. In the early 1960's, he achieved the position as the Manager of the Respiratory Laboratory created for classified scientific research for the Soviet Cosmos and the first outer space missions (which had the 1964 flight of Yury Gagarin, the first man in space). The Buteyko Method has been applied by medical doctors on more than 100,000 asthmatics, over 30,000 people with cardiovascular issues and thousands of patients with other conditions (diabetes, cancer, bronchitis, sleep apnea, HIV-AIDS, liver cirrhosis and more). Dr. Buteyko would instruct sick patients to do up to 2 to 3 hours of daily breath work.
