Aspirin and Prostaglandins

08:36
Aspirin, or acetylsalicylic acid, is one of the oldest and most well-known medications for headaches, fever, pain, and even prevention of heart attack and stroke. How does this drug cause relief of these symptoms? Check out this episode of Medicurio to learn about the quite interesting mechanism of aspirin in the body. Detailed Mechanism of Aspirin: There are two major forms of cyclooxygenase (COX) in cells. COX-1 is always made in the cell, while COX-2 is only made during inflammation. Aspirin permanently inhibits the COX-1 isoform (version) by acetylation of its active site. Therefore, aspirin’s effect lasts however long it takes for the cell to transcribe and translate new COX-1 enzymes, which usually takes a few hours. COX-1 decreases fever, pain, and inflammation, but also increases the risk of gastric ulcers by decreasing clotting and stomach mucous production. The COX-2 isoform is thought to only produce prostaglandins involved in fever, pain, and inflammation but not other functions; therefore, making a drug that inhibits COX-2 instead of COX-1 would give all the anti-inflammatory effects of aspirin without the risk of gastric ulcers. Such a drug would be a major upgrade and many companies came up with possible COX-2 inhibitors. One such drug, called rofecoxib or Vioxx, entered the market claiming that it would not cause gastric ulcers. What the drug company did not reveal was that during clinical trials, many patients suffered from heart attacks after chronic use. These problems began to surface once the general public began using Vioxx, leading to withdrawal of the drug and lawsuits against the company. Moral of the story? Research integrity is important, especially for a drug company as people’s lives are at stake. Trivia about Aspirin: It is debated if Felix Hoffmann was the true “inventor” of aspirin. Arthur Eichengrün, a fellow Bayer employee, claimed that he was the one who came up with the procedure and that Hoffmann simply performed the reaction following his guidelines. Bayer has denied this claim. Hoffmann synthesized aspirin for his father, who complained about the taste of salicylic acid that he was taking to treat his arthritis. Aspirin is extremely toxic to cats (which cannot break down aspirin) and to a lesser extent, dogs. Despite this, aspirin can still be used, just at low doses, to treat inflammation in animals. You can overdose on aspirin – at lethal doses, aspirin interferes with energy production in the mitochondria which leads to eventual death. Before death, unique symptoms such as ringing in the ears, hyperventilation, and dizziness, as well as non-specific symptoms such as vomiting and nausea appear. Overdoses are often due to chronic aspirin use, so if any of these symptoms apply to you, speak to a doctor immediately. Aspirin’s effect is increased by ingesting it with caffeine because caffeine is a stimulant, which increases blood flow to carry aspirin around the body faster and begin acting on tissue quicker. Acetaminophen (Tylenol) is not technically an NSAID because it does not decrease inflammation; however, it also works on COX enzymes so I have lumped it with the other NSAIDs in this video for simplicity.

How to Give Your Child Liquid Medicines / Comment donner des médicaments liquides à votre enfant

01:35
Giving medicine to children, especially to babies, can be tricky. Watch this video for helpful tips to make things easier. Il peut être difficile de donner des médicaments aux enfants et particulièrement aux bébés. Cette vidéo vous offre des conseils utiles pour vous faciliter la tâche. To learn more about child health visit www.aboutkidshealth.ca VIDEO TRANSCRIPTS: How to give liquid medicine at home Giving medicine to children, especially to babies, can be tricky. They may resist it, refuse it or spit it up once you get it into them. Here are some tips to make things easier. Babies are more likely to take their medicine when they're hungry, so give it to them before you feed them. They're also less likely to spit it up that way. We recommend, unless you have to, you don't mix medicine with food or drinks. Mixing it makes it harder to know if the full dose has been taken. If that's the only way your child will take the medicine, check with your pharmacist first, because certain foods, like yoghurt or applesauce, can stop the medication from working and others, like honey or syrup, work only if your child is not on a restricted diet. If you do use food or drink, use a very small amount, like this, so that the child will finish it all. Now, let's go over how to prepare the medicine. First, measure the dose with the spoon or syringe you got from the pharmacy. Now give the medicine to your child in small amounts and watch for them to swallow before you give them more. If you're using a syringe, try putting the medicine inside their cheek pouch. That will prevent them from gagging. If you're still having trouble, try giving your baby a popsicle or a freezie before giving them medicine. The sweet taste, and slight numbing sensation will "help the medicine go down". When giving medicine to any child, it's important that you double check the amount and the dose with the instructions that are written on the bottle. If you use a log or a medicine diary, that can help to prevent either giving too many doses, or missing some doses. If your child still can't swallow the medicine or keep it down, or if they're showing any side effects, get in touch with your doctor or pharmacist. Some oral medicines can be made to be given in other ways. Talk with your pharmacist about this to see if it's possible. - Give medicine according to instructions (Remember to give medicine to your child according to the instructions.) - Check about mixing foods or other medicines (Ask your pharmacist about foods or other medicines that may stop it from working properly.) - Check dosing amounts (If you have any concerns about the amount of medicine, or if your child is refusing to take it, speak with your pharmacist.) Key points: • Give medicine to your child before meals • Administer small amounts • Put it in their cheek pouch, rather than the back of their throat ___________________________________________________ This video is provided for general information only. How Fighting Fevers With Tylenol Makes You Sicker

06:57
If you reach for a bottle of Tylenol when you come down with a fever, you'll actually prolong your illness, make yourself more susceptible to infection, and increase your chances of passing on your illness to others. In this video we look at the science behind the use of antipyretic drugs like Tylenol (acetaminophen), Advil (ibuprofen), and aspirin for suppressing fevers, and how they can impair your body's immune response. We'll Talk About: 1) What happens when critically ill patients with fevers are randomized into either a fever suppressing treatment or no fever treatment at all? Which group has more survivors and infections? 2) What happens when children with chicken pocks are given antipyretics? 3) What happens when adult volunteers injected with rhinovirus or influenza are given these drugs? Check out this video to learn more about the science of fighting fevers. Help us caption & translate this video! http://amara.org/v/H5I1/

Acute Otitis Media (AOM)

01:50
It is a middle ear infection that is characterized by fever, otalgia, otorrhea, and hearing loss. While otitis media is usually associated with pain, the absence of earache does not rule it out. Infants and toddlers may present with nonspecific signs and symptoms, such as irritability, fussiness, ear pulling, poor feeding, and sleep disturbances. If the patient reports a sudden relief of ear pain and fever, then the tympanic membrane may have ruptured. Spontaneous healing of the perforation usually occurs within a few days. Otoscopy typically reveals a bulging, erythemic or cloudy, and immobile tympanic membrane. In regards to erythema, keep in mind that injection can occur due to a variety of causes, such as high fever, crying, and irritation of the auditory canal inflicted during the removal of cerumen. However, intense redness or a clear difference in redness between the two ears is supportive of the diagnosis. If the tympanic membrane has ruptured, there may be purulence in the ear canal. And if pus or cerumen is blocking the view of the tympanic membrane then it should be removed, but NOT via rinsing with a syringe. In addition to analgesic and antipyretic relief that can be obtained with acetaminophen or ibuprofen, treatment with amoxicillin is indicated for adults and most children. Now, antibiotics may be WITHHELD for 48 hours in healthy children ≥ 2 years of age with mild, non-suppurative unilateral otitis media. Watching waiting, however, is NOT recommended for adults. When first-line therapy fails (i.e., lack of symptomatic improvement within 48 to 72 hours) then treatment with amoxicillin-clavulanate is indicated. As well, antibiotic therapy should commence with amoxicillin-clavulanate if the patient has a concomitant purulent conjunctivitis or has received a beta-lactam antibiotic in the previous 30 days. Acetaminophen Safe Use Basics

00:31
Did you know that more than 50 million Americans use a medicine that contains acetaminophen each week? Acetaminophen is the most common drug ingredient in America, found in more than 600 different over-the-counter (OTC) and prescription (Rx) medicines, including pain relievers; fever reducers; sleep aids; and numerous cough, cold, and allergy medicines. When used as directed, acetaminophen is safe and effective. But there is a limit to how much you can take in one day. Taking more acetaminophen than directed is an overdose and can lead to liver damage. The U.S. Food and Drug Administration recommends taking no more than 4,000 milligrams (mg) of acetaminophen in a 24-hour period. Follow these four steps to use acetaminophen safely: 1. Always read and follow the label. 2. Know if your medicines contain acetaminophen. 3. Take only one medicine at a time that contains acetaminophen. 4. Ask your healthcare provider or a pharmacist if you have questions about dosing instructions or medicines that contain acetaminophen. In partnership with the Acetaminophen Awareness Coalition (AAC), the Know Your Dose campaign educates consumers on how to safely use medicines that contain acetaminophen.

Are painkillers safe for children during toothache? - Dr. Sai Kishore Reddy

01:50
Normally indiscriminate use of pain killers in children is very harmful. new usually report on a daily basis or a weekly basis a lot of kidney failure cases or kidney necrosis cases due to indiscriminate use of pain killers on your own. So own education is always dangerous. So self-medication is always dangerous as while giving a pain killer, the pain threshold will be more, pain bearing capacity will be more. So once you control the infection, the pain comes down. So if your give necessary or indiscriminate use of pain killers, it will not decrease the infection, but it will increase the threshold level or the pain bearing capacity of the body. So that is why indiscriminate use of pain killers is always dangerous. It will be difficult for the parent to identify the cause of the pain without knowing what is the main causing factor of the pain. Without alleviating or solving those factors, it is of no use if you give pain killers. Kidneys have lesser processing capacity for pain killers .So it will be harmful for the child’s kidney also. So it will be harmful.

Tylenol is poison & can KILL you (Russia Today)

Google The Tylenol murders (Ped Infect Dis J 1995 jul;14(7): 588-94)again why? We all know that NSAIDS like tylenol and iboprofen are implicated. Another one (Infect Med 16 (5):307, 1999 noted severe Group A beta-hemolytic strap in children who had had antipyretic medication. MMWR, May 15, 1998, Vol 47 No 18 listed chickenpox cases too.But these parents don't appear to have been asked about Nonsteroid anti-inflammatories, or - tylenol. Pediatrics Vol 103, No 4, April 1999, pg 783+ again noted the association of ibuprofen with serious complications - and elsewhere in the same issue. Acta Paediatri Jpn 1994 Aug;36(4):375-8 shows that administration of any antipyretic drugs in children with infectious diseases worsens their illnesses. Why? Because it appears that anti-pyretics down- regulate the immune response. This is only natural, since the reason for a fever is to switch on certain cytokines, and push the immune system up from the fourth gear to the tenth (prely as an imagery picture, here), to help deal with the problem. There is very clear evidence that tylenol creates an ineffective immune system in some children, and as Ped Infect Dis 1996:15: 355-53 points out about the immune system in general - not in particular to Tylenot - "an ineffective immune response to certain organisms can result in life-threatening infection." Better yet, a Journal called Family Practice, Volume 13, No 2, 1996 stated: "Paracetamol prescribing is reaching epidemic proportions and the potential dangers of hepatotoxicity and the inhibition of the immune response in children are discussed." He goes on to say: "Despite our lack of knowledge about its therapeutic mechanism, it has been claimed to be a safe frug, especially for children...there is mounting evidence that paracetamil is not the benign drug that it was formally thought to be... We would question the whole rational of prescribing the drug in near epidemic proportions...there is little concern about its use in the short term as an ANALGESIC, there is considerable controversy over its use as an antipyretic...paracetamol may decrease antibody response to infection and increase morbidity and mortality in severe infections....too many parents and health workers think that fever is bad and needs to be suppressed by paracetamio, when indeed, moderate fever may improve the immune response." In another earlier study in the Lancet March 9, 1991 pg 591 it was stated "Studies have clearly shown that fever helps laboratory animals to survive an infection whereas antipyresis increases mortality (death). Moreover, there is considerable in-vitro evidence that a variety of human immunological defences function better at febrile temperatures than at normal ones." It is a natural thing to say that if your child was vaccinated against chickenpox, they won't get it (....? maybe) so you then wouldn't have to worry about the use of tylenol. tylenol should never be used in any case of infectious disease, for several reasons - first, it suppresses the immune system. Second it makes most children sleepy. Third - the combination of those two factors can mask critical symptoms which would alert a parent to a problem, so that as in the case of many if not most of the recent chickenpox cases - by the time they get to hospital, the situation is very severe indeed. Do you think then, in the light of the literature, all parents should be warned that while they can use Tylenol, or paracetamol as as ANALGESIC - or pain relief, that they should never use it for infectious diseases?
