What is the normal range of T3, T4 and TSH? - Dr. Mahesh DM

The normal ranges of T3, T4 and TSH is actually depends on the type of assay and how the test is done in different labs. In most of the labs what they use is the nanograms per ml or micrograms per deciliter for T3 and T4 and for TSH are microintrernational units per ml. For TSH the range is 0.4 to 5.4, which is the normal range for TSH. But we have to remember that the normal range varies with age. So for a youngster who is 25 years of age, women, planning pregnancy, the normal is to keep the TSH below 3.the elderly people it is okay even if the TSH is 7 or 8 for a dose for example more than 60 years. So pelase consult your doctor to consult whether the TSH is normal for your age rather than checking the lab values. Even if the value is normal, it may vary according to the conditions. For T4 values, it has a wider range from 5 to 12 micrograms per deciliter. But in free T4, the normal range is between 0.8 to 2 nanograms per ml. So you have to check if it is total T4 or it is unbound T4 that is free T4. The last but not very important for the interpretation is the T3 values. the T3 has to be considered only in specific situations. So don’t give you much importance to the T3 values whenever you check for the thyroid function. If the T4 and TSH are normal, then you need not worry much but the normal range for T3 is from 0.4 to 2 nanograms per ml and it is different from the previous values. So it varies from lab to lab. So always confirm with your doctor if the values are normal or abnormal and how much importance you have to give to the thyroid values. So when you do the thyroid function, please look at the unbound T4 that is free T4. These are the 2 important values that you have to know and interpret them according to the age and any other associated condition like if you are taking any drugs or you are pregnancy so you know that your thyroid function is normal for that particular condition. So you need not take medicines if it is in the physiological range.

The thyroid is a gland in the body which produces two hormones T3 and T4. Pituitary is another gland which is the master gland of the body which secretes thyroid stimulating hormone which stimulate the thyroid to release T3 and T4. If the thyroid is functioning excessively T3 and T4 increases in the body and pituitary gets a message. So the TSH is secreted less and thyroid is less stimulated and T3, T4 levels come back to normal. The condition where T# T4 is elevated and TSH is less is called hyperthyroidism. Normal TSH levels varies from 0.3 to 4.2. There are some kits which quote as 0.3 to 5.2 as normal. If the value is between this we find that thyroid is not having any major issue. Patient should not be treated with any medicine at that point. If the TSH values less than 2 or 1.5 most of them later on in life will turn to hypothyroid subjects. Suppose TSH is low for a longer time there is less stimulation of thyroid and these people gradually can have hypothyroidism. In this condition the patient should not take any medication but should take care of the diet. You should be aware of the goitrogenic foods. Cabbage, cauliflower, broccoli, processed and preserved food and refined sugar are known to be not good for the thyroid gland. Cranberry and salt containg foods are very effective in hyperthyroidism. Certain homeo medicines like throidinum, calcaria iode, sulphur iode, naturum iode are found to be very effective in thyroid functions. Iodium and spongia are other two major medicines used in the function of thyroid gland. Even by examining the hair we can tell whether this person has got a thyroid problem or not.

Sometimes while interpreting thyroid function test we come across the result where it says that the thyroid hormones T3 & T4 are normal where as the TSH is suppressed how can this happen. So that can happen when there is a mile hyper function in the thyriod gland, so if there is a slight excess of thyroid hormone being produced TSH can become supress to a very low level, this is also because of a log linear relationship between the thyroid hormone and the TSH value. TSH is actually a hormone produced in the pituitary, thyroid hormones actually produced in the thyroid glands located in the neck. But under the influence of T4, there is a small increase of 1 unit increase in P4 that can be attained 10 unit reduction in the TSH. So that's why you can get a very sharp drop in the TSH before that before actually rises above its normal range, if you look at the range of the normal for it is between 4 nanogram per deciliter to about 12 nanograms per deciliter. So it is in the range of a few points actually even 1 or 2. Increase T4 can causes 10 point reduction in the TSH, the increase in T4 is mirrored by a corresponding 10-12 decrease or increase in TSH so that's the reason for the curious picture of T4 & T3 being normal and TSH being abnormally low.
