Less than one percent of men will be diagnosed with breast cancer this year.Jeff Stokes was recently diagnosed with breast cancer. Last December I noticed I was itchy on my left breast, Jeff said.Jeff Stokes was in the shower when he noticed he had a lump. He called his doctor and tests were ordered. His doctor was concerned that Jeff may have breast cancer.At first, Jeff was skeptical. Of course I thought, well of course its not, Im male. It is going to be fine.Jeff had surgery and test results confirmed his doctors fear.The surgeon called me one day while I was leaving my house and he said interestingly the results came back and you do have cancer, Jeff said. And wow, the world just fell apart for meI remember opening the door of the truck and then I closed it. I was like how am I going to tell my wife this. You know. This just doesnt happen.Doctors say breast cancer is not as common in men as it is in women. In fact, less than one percent of men will be diagnosed with breast cancer this year.Doctor Brett Parkinson, a radiologist at the Intermountain Medical Center Breast Care Center, says that most men arent on the lookout for breast cancer because they think only women are at risk of getting it. A man who gets a diagnosis of breast cancer is really surprised. It hits them from out of nowhere because, number one, they dont expect to have breast cancer anyway.Jeff certainly didnt think he could get breast cancer. He initially thought doctors made a mistake. He thought that it was possible doctors mixed up samples.A second surgery revealed that the cancer had not spread. Right now, Jeff is undergoing radiation. Doctors say his prognosis is good. Breast Cancer in Male and Surgical Technique. Male breast cancer (male breast neoplasm) is a rare cancer in males that originates from the breast. Many males with breast cancer have inherited a BRCA mutation, but there are other causes, including alcohol abuse and exposure to certain hormones and ionizing radiation. As it presents a similar pathology as female breast cancer, assessment and treatment relies on experiences and guidelines that have been developed in female patients. The optimal treatment is currently not known. About one percent of breast cancer develops in males. Known risk factors include radiation exposure, exposure to female hormones (estrogen), and genetic factors. High estrogen exposure may occur by medications, obesity, or liver disease, and genetic links include a high prevalence of female breast cancer in close relatives. Chronic alcoholism has been linked to male breast cancer. The highest risk for male breast cancer is carried by males with Klinefelter syndrome. How to Diagnosis Male Breast Cancer? Typically self-examination leads to the detection of a lump in the breast which requires further investigation. Other less common symptoms include nipple discharge, nipple retraction. swelling of the breast, or a skin lesion such as an ulcer. Ultrasound and mammography may be used for its further definition. The lump can be examined either by a needle biopsy where a thin needle is placed into the lump to extract some tissue or by an excisional biopsy where under local anesthesia a small skin cut is made and the lump is removed. Not all palpable lesions in the male breast are cancerous, for instance a biopsy may reveal a benign fibroadenoma. In a larger study from Finland the average size of a male breast cancer lesion was 1.8 cm. Beside the histologic examination estrogen and progesterone receptor studies are performed. Further, the HER2 test is used to check for a growth factor protein. Its activity can be increased in active cancer cells and helps determine if monoclonal antibody therapy may be useful. Male breast cancer can recur locally after therapy, or can become metastatic. How Male breast cancer is Differences from female breast cancer? There are significant differences between male and female breast cancer. Lesions are easier to find in males due to the smaller breast size; however, lack of awareness may postpone seeking medical attention. The presence of gynecomastia may mask the condition. The diagnosis is made later in males—at age 67 on average—than in females with their average at 63. Lesions are less contained in males as they do not have to travel far to infiltrate skin, nipple, or muscle tissue. Thus, lesions in males tend to be more advanced. Almost half of male breast cancer patients are stage III or IV. In familial cases, male BRCA2 carriers are at higher risk, rather than BRCA1 carriers. With the relative infrequency of male breast cancer, randomized studies are lacking. What is the surgical Treatment of Breast cancer in male? The initial treatment is surgical and consists of a modified radical mastectomy with axillary dissection or lumpectomy and radiation therapy with similar treatment results as in females. Also, mastectomy with sentinel lymph node biopsy is a treatment option. In males with node-negative tumors, adjuvant therapy is applied under the same considerations as in females with node-negative breast cancer. Similarly, with node-positive tumors, males increase survival using the same adjuvants as affected females, namely both chemotherapy plus tamoxifen and other hormonal therapy. There are no controlled studies in males comparing adjuvant options. In the vast majority of males with breast cancer hormone receptor studies are positive, and those situations are typically treated with hormonal therapy. Locally recurrent disease is treated with surgical excision or radiation therapy combined with chemotherapy. Distant metastases are treated with hormonal therapy, chemotherapy, or a combination of both. Bones can be affected either by metastasis or weakened from hormonal therapy; bisphosphonates and calcitonin may be used to counterbalance this process and strengthen bones. In this video, Judith Balmaña addresses the role of the BRCA1 and BRCA2 proteins in cancer risk assessment, early detection and cancer prevention, and the implication of BRCA deficiency as a predictive biomarker for targeted therapies in different tumour types. BRCA1 and BRCA2 encode two large proteins, which localize to the cell nucleus and are widely expressed in different tissues during the S and G2 phases. These proteins work to preserve chromosome stability, mainly through DNA repair, cell cycle control, and fork stabilization during replication. BRCA1 is a pleiotropic DNA damage repair protein that functions in both checkpoint activation and DNA repair. BRCA1 links DNA damaging sensing and DNA damage repair effectors. BRCA2 is a mediator of the core mechanism of homologous recombination repair, responsible for error-free repair of double DNA strand breaks. Inactivation of either of the two proteins leads to carcinogenesis. Germline mutations in these genes are relatively rare. It is estimated that their prevalence in the population is 1 out of 200-400 individuals, so 0.25%. This prevalence is higher in certain groups, such as individuals of Ashkenazi ancestry, where the prevalence is 1 out 40 individuals, or 2.5%, due to three founder mutations. In unselected women with breast cancer, germline mutations in these genes are observed in 2-5%. This frequency is higher in women with triple negative breast cancer, or those of young onset, or with a family history. Between 8-18% of women with invasive epithelial non-mucinous ovarian cancer might have a germline BRCA1 or BRCA2 mutation. In pancreatic ductal adenocarcinoma, their frequency is 2-4%, and in prostate cancer the prevalence ranges from 1-2% if young onset or localised, to 4-6% among metastatic prostate cancer patients. The cumulative risk of breast cancer in BRCA1 carriers is 40% at age 50, and 72% at age 80. The cumulative risk of breast cancer in BRCA2 carriers is around 35% at age 50, and 68% at age 80. Cumulative ovarian cancer risk is almost 40% for BRCA1 carriers, and almost 20% for BRCA2 carriers. The cumulative risk of pancreatic cancer, mainly in BRCA2 carriers, is between 1.5% and 3%, and that of prostate cancer, also mainly in BRCA2 carriers, is between 5% and 15%. These prostate tumours typically present earlier, with a more aggressive phenotype and a reduced survival than sporadic cancer. Early detection of ovarian cancer still remains a challenge. Risk reducing bilateral salpingoophorectomy is recommended, usually around age 40, and around age 45 in BRCA2 carriers. In mutation carriers, this intervention has been associated with an ovarian cancer risk reduction of 80-95% and a mortality reduction. Salpingectomy alone is a surgical procedure that is potentially as efficient as salpingoophorectomy, but it is still considered to be an investigational approach. Prophylactic mastectomy has also been associated with a 90% or more breast cancer risk reduction and it is an option to individualise in women with a BRCA mutation. Regarding surveillance options, women are recommended to undergo breast MRI from the age of 25, and add a mammogram at age 30, on a yearly basis. CA125 levels with transvaginal ultrasound every 6 months might be considered in women until the decision on prophylactic salpingoophorectomy. In men, prostate cancer screening is recommended from the age of 40-45 years. Cancer cells with BRCA mutations lead to homologous recombination repair deficiency and are more sensitive to DNA damaging agents, such as platinum agents or PARP inhibitors. Indeed, platinum-based regimens have shown higher efficacy in patients with advanced breast, ovarian, and pancreatic tumours associated with BRCA-deficiency. PARP inhibitors have been approved for patients with advanced ovarian and breast cancer. In prostate cancer, a breakthrough designation by FDA has been granted; and in pancreatic cancer, clinical trials are currently ongoing. Therefore, the question is can we survive breast cancer? Can we find it, catch it in time so our survival chance is bigger? Is catching breast cancer early helping to escape death? A 25 years long Canadian study with nearly 90.000 women participants aged 40 to 59 finds no reduction in the mortality rate because of breast cancer between the control group that was receiving physical exams and the group that was having mammography screening and physicals. Overdiagnosis, false positive and false negative are either pushing healthy women for more radiation or keeping already sick women from getting treatment they need. Self-examination, MRI and ultrasound are not catching breast cancer any better than mammograms and are in no way improving our health or survival chances. What can and should we do? Should we try to escape mammography screenings and if we do, do we have a bigger chance of escaping breast cancer or at least at surviving it? WAYNE, NJ -- Cheri Ambrose, Founder of the Male Breast Cancer Coalition, raises awareness about the rise in breast cancer diagnoses in men at the annual Komen North Jersey Women’s Wellness Expo held at the Passaic County Technical Institute. “The American Cancer Society puts the statistics out every year. This year alone, 2,550 men just here in the United States are estimated to be stricken with breast cancer and another 480 are expected to die from the disease, due to lack of awareness and misdiagnosis,” Ambrose explained. “When you hear breast cancer, you automatically think of the pink. You automatically think of women’s breast cancer,” said Kenya Medard, who lost her husband to breast cancer. The Male Breast Cancer Coalition is a global organization that unites survivors of male breast cancer from around the world to share their journeys in the hopes of preventing other men from going through this disease alone. “We bring them together with resources and oncologists and genetic specialists from around the world as well,” said Ambrose. “People are getting more aware, the more that we’re out there,” she continued. “A lot of men we noticed that are finding us on the Internet, they’re coming forward at early stages.
