How to Use Ear Drops Properly

MEDICATION USAGE INSTRUCTIONS 1. Wash your hands well with soap and water. 2. Warm the eardrop bottle by holding it in the palm of your hand for a few minutes. This will make the eardrops more comfortable to instill. 3. If instructed by your pharmacist or doctor, gently shake the bottle for 10 seconds. 4. Remove the cap or dropper from the ear drop bottle. Make sure nothing touches it or the tip of the container. 5a. Tilt your head to one side, or lie down with the affected ear facing up. 5b. For adults or children over three, gently pull the top of the ear upward and backward. 5c. For children under three, gently pull the bottom of the ear back and down. This will straighten the ear canal. 6. Instill the correct number of drops in your ear, as instructed on your prescription label. Ensure the dropper or the tip of the container does not touch the ear or other surfaces, to avoid contamination. 7. Keep your ear tilted up for three to five minutes. 8. Replace and tighten the cap or dropper right away. Make sure not to touch the tip of the container or the dropper. 9. Wash your hands to remove any medication. 10. If you are instilling drops into both ears, wait 5-10 minutes between ears to allow the ear drops to run into the ear canal. *Always follow the directions provided by your doctor or pharmacist. These instructions are meant to be a general guideline. Check with your pharmacist if you have any questions.

Pupil Dilatation With Intracameral Lignocaine During Phaco in One Eyed Case Allergic Dilator Drops

In this video, we share pearls for achieving pupillary dilatation while peforming cataract surgery in a one eyed patient. This patient was allergic to topical mydriatic eyes drops. Intracameral injection of 0.2 ml of preservative-free lidocaine was attempted as an alternative to mydriatric drops achieving excellent papillary dilatation while performing phaco surgery. An AMO Signature Phacoemulsification Machine was used with Ellips Transverse Fx Ultrasound & Fusion Fluidics. AMO Tecnis IOL implantation was done with post op vision of 20/30. Cataract surgery requires adequate mydriasis, which is usually achieved by topical and/or intracameral administration of anticholinergic and/or sympathomimetic mydriatic agents, commonly cyclopentolate, tropicamide, phenylephrine and homatropine. These regimens, however, have some disadvantages such as slow onset of dilation, which increases the waiting time before operation, allergic reaction, adverse ocular and systemic effects, which are specially important in high-risk groups such as hypertensive patients and children and their tendencies to wear off during surgery. In our experience injection of 0.2 ml of 1% preservative-free lidocaine (lignocaine) in anterior chamber can have a persistent, stable and satisfactory pupil dilation for a safe phacoemulsification and IOL implantation in different patients including currently shown patient- allergic to topical dilators eye drops, as well as diabetic and non-diabetic and eyes with pseudoexfoliation. Lidocaine is an antiarrhythmic drug as well as an effective local anesthetic. Intracameral injection of preservative-free lidocaine is used widely for local anesthesia and discomfort relief in cataract . Intracameral lidocaine has some advantages over topical mydriatics. It doesn′t need time-consuming preoperative program for pupillary dilation, doesn′t have systemic side-effects of topical mydriatics and provides satisfactory pupillary dilation as well as simultaneous anesthetic effect in phacoemulsification cataract surgery. Intracameral injection of preservative-free lidocaine was attempted as an alternative to reduce the potential disadvantages of common mydriatics. surgery. Lidocaine causes no additional inflammation and endothelial cell loss and its safety has been confirmed in previous studies. BRIEF BIODATA: DR. Removal of a Dermoid cyst through a Lid Crease incision This is Dr. Scott Larson at the University of Iowa department of ophthalmology and I would like to show you a removal of a dermoid cyst through a lid crease incision First a lid crease incision was marked and the area was infiltrated with Lidocaine 1% with Epinephrine 1:100,000. A 15 blade was used to make a skin incision and then sharp dissection though the orbicularis is performed with Westcott scissors. Another option at this point would be to use a cutting/cauterizing instrument. Once through the orbicularis, the dermoid cyst is gently grasped and carefully dissected free of any fascial attachments or adhesions. The cyst is filled with a white paste that is quite inflammatory so care should be taken to avoid rupturing the cyst. A skin hook can be placed and held by an assistant to facilitate exposure. If the cyst is accidently entered, care should be taken to attempt to fully remove the parts of the cyst and not leave remnants behind. Copious irrigation can help remove cyst contents from the tissues. In this case you can see the cyst was bi-lobed. The skin is then closed with a two layer closure. In this case 5-0 Vicryl is used to close the orbicularis with an interrupted mattress style and this layer provides the strength of the closure. A few 6-0 plain gut sutures are then placed to close the approximated skin edges. Erythromycin ophthalmic ointment can then be placed on the cleaned wound. This incision will heal with the scar hidden in the upper lid crease. Thank you for watching.

ReSTOR Cataract Surgery Performed at Christenbury Eye Center

This is an incredible step by step video of the entire ReSTOR procedure from cataract removal to lens implantation. ReSTOR can eliminate reading glasses and bifocals. ReSTOR also corrects distance and near vision. The patient is comfortably sedated and the pupil is dilated and we use numbing drops. Using a ocular microscope with high magnification, a 2-3mm incision is made to gain access to the lens which will be replaced. A side port incision is made for a second instrument. For comfort we instill 1 percent lidocaine in the eye. So with the topical and intraocular numbing medicine the patient has no discomfort. Viscous elastic is used to maintain the interior chamber which is like a jelly type collagen material. And a capsulorhexis is used to open the interior capsule of the lens. These utrata forceps are used to complete the capsulorhexis so there's a circular opening into the lens bag as we call it. The red reflex helps the visualization with the microscope. SO BSS, or saline is used to separate the cataract from the capsule, the cataract is rotated. A pea sharper is often used to segment the cataract into four manageable quadrants. And you can see how simply the cataract is removed by first dividing into four quadrants. Phacoemulsification is used to emulsify the cataract which is a hardening in opacity of the lens. The phacoemulsification then is followed by vacuuming of the emulsified fragments. This process can take anywhere from 30 seconds to a minute or two and we take our time to slowly emulsify every nuclear fragment of those four pieces or four quadrants. The second instrument is used to manipulate the pieces of the cataract as they are emulsified and vacuumed from the capsular bag. Any residual cortical pieces of the cataract are removed with irrigation aspiration tip; this is a very soft silicone tip. Has a tiny .3mm aspiration port that gently aspirates or vacuums out any cortical or remaining fibers of the cataract. Then the irrigation aspiration tip is placed in a low vacuum mode to polish the posterior capsule, to make sure every little fiber of the cataract is removed from the posterior capsule to provide the clearest vision with the ReSTOR lens implant. And this is all performed through a 2.5mm incision and your topical numbing drops or topical anesthetic. Viscous elastic which is a jelly type material that is used to inflate the bag and the ReSTOR lens has been folded and placed in a cartridge; the cartridge is inserted through the incision and slowly injected into the capsular bag. The whole lens implant is inserted through a 2.5mm incision. It slowly opens up, sinskey hook is used to orient the lens and center. You can see little arms or haptics, they will slowly open and move out to the edge of the capsule inside the capsular bag and center the intraocular lens. The Viscous elastic is removed with the irrigation aspiration tip. And viscous elastic behind the optic as well, to help center the optic. Next, miochol medication is instilled, to constrict the pupil, this helps center the ReSTOR lens. You might be able to notice the concentric circles of the 2nd optic called a near optic on the anterior surface of the ReSTOR lens. And you can see the pupil is constricting, and other medication and antibiotics are injected, and the wound is hydrated to seal it without sutures. And again, the lens is centered in the pupil to make sure it's perfectly centered to give the best distance and reading vision. And that completes the ReSTOR procedure. We often perform both eyes the same day, for patient convenience.
