Can you actually make a disease disappear? Dr Rangan Chatterjee thinks you can. Often referred to as the doctor of the future, Rangan is changing the way that we look at illness and how medicine will be practised in years to come. Since 2011, he has served as Chairman of Surgery at Rush Oak Park Hospital and as the head of the Orthopedic Master's Program at Rush. Dr. Cole’s research interests include cartilage restoration, therapeutic biologics, and minimally invasive surgical techniques for the treatment of the knee, elbow, and shoulder. A day in the life of a paramedic can involve a 12-hour shift, comforting heartbroken relatives and even treating stab wounds.After ITV's This Morning's resident doctor Dr Ranj Singh joined the North West ambulance service for a shift, he learnt how the team work 'day-in, day-out', often without breaks, with no idea what the next 12 hours may bring.Some are even physically and mentally abused by the very people whose lives they are trying to save, making them frightened of what they may encounter at every job.At the end of a shift that involved having to break it to a husband that his wife had died and treat critical internal bleeding from a knife attack, Dr Singh said he felt 'broken' and without 'the mental reserve to keep going'.Last year, around 2,500 paramedics were signed off work for stress.PATIENTS FORCED TO WAIT FIVE-AND-A-HALF HOURS AS 23 AMBULANCES QUEUED OUTSIDE AN A&E DEPARTMENT  Patients waiting to be admitted were forced to wait for up to five-and-a-half hours on a Friday night  in December as 23 ambulances queued outside a hospital's A&E department. Queen Alexandra Hospital in Portsmouth, whose A&E department has previously been described by the Quality Care Commission as 'chaotic', experienced an 'exceptionally difficult weekend' after a backlog of patients turned up with flu and other respiratory and winter illnesses. NHS figures released last week also reveal 1,336 people across the UK have been admitted to hospital with the winter vomiting bug, up from 790 the week before. Experts said this spike in norovirus cases is adding pressure to hospital beds, with 94.6 per cent being occupied, which is causing the health service to buckle under the strain, according to an ITV Meridian report. Shadow health secretary Jonathan Ashworth also blames the recent drop in temperatures for the increase in demand on hospital services.On the way to treat a life-threatening case of a 'female not breathing', with no further details, paramedic Pete Sutcliffe told Dr Singh: 'I can’t think of many other jobs where for 11-and-a-half hours you have to be 100 per cent correct because a mistake can be life-threatening,' ITV reported.When they arrived at the scene, 85-year-old Elizabeth Roberts had suffered a cardiac arrest. A board certified orthopaedic surgeon, Dr. Urban has been a member of OrthoWest since 2004. As a son of a physician in the Army Medical Corps, he spent his childhood in West Germany, West Berlin, and several stateside locales. After Army life, his family settled in central Nebraska where he attended high school in Kearney, Nebraska. He went to Cornell College and graduated with majors in History and Religion with an additional emphasis in Biochemistry. He attended medical school at the University of Nebraska Medical Center in Omaha and graduated with distinction before completing an internship and orthopaedic residency at the combined Creighton-Nebraska Universities Health Foundation. During residency he spent an additional year of training focusing on the research of total hip and knee replacements with an emphasis on alternative bearing surfaces and infections of hip and knee replacements. After residency he spent another year of subspecialized training, a fellowship, at the Anderson Orthopaedic Research Institute in Alexandria, Virginia where he received additional training in hip and knee replacements by world renowned joint replacement surgeons Charles Engh, Gerry Engh, C. Anderson Engh, and James McAuley. Dr Urban specializes in the diagnosis, non-operative management, and surgical treatment of hip and knee arthritis. His practice also focuses on the treatment of failed hip and knee replacements, infected hip and knee replacements, peri-prosthetic fractures, and revision hip and knee replacements. Additional interests include trauma, sports injuries, and general orthopaedics. In addition to his affiliation with the Nebraska Orthopaedic Hospital, he has served as Orthopaedic Service Chief at the Nebraska Medical Center and has maintained a wound care clinic there as well. Dr. Urban has written numerous abstracts, articles, presentations and book chapters on a variety of topics related to hip and knee replacements. Cocaine Blues Elliot Charles Adnopoz was born in Brooklyn, New York to Jewish parents in 1931. Elliott grew up inspired by the rodeos at Madison Square Garden, and wanted to be a cowboy. Though encouraged to follow his father's example and become a surgeon, Elliott rebelled, running away from home at the age of 15 to join Col. Jim Eskew's Rodeo, the only rodeo east of the Mississippi. They traveled throughout the Mid-Atlantic states and New England. He was only with them for three months before his parents tracked him down and had him sent home, but Elliott was exposed to his first singing cowboy, Brahmer Rogers, a rodeo clown who played guitar and five-string banjo, sang songs, and recited poetry. Back home, Elliott taught himself guitar and started busking for a living. Eventually he got together with Woody Guthrie and stayed with him as an admirer and student. With banjo player Derroll Adams, he toured the United Kingdom and Europe. By 1960, he had recorded three folk albums for the UK record label Topic Records. In London, he played small clubs and pubs by day and West End cabaret nightclubs at night. When he returned to the States, Elliott found he had become renowned in American folk music circles. Woody Guthrie had the greatest influence on Elliott. Woody's son, Arlo, said that because of Woody's illness and early death, Arlo never really got to know him, but learned his father's songs and performing style from Elliott. Elliott's guitar and his mastery of Guthrie's material had a big impact on Bob Dylan when he lived in Minneapolis. When he reached New York, Dylan was sometimes referred to as the 'son' of Jack Elliott, because Elliott had a way of introducing Dylan's songs with the words: "Here's a song from my son, Bob Dylan." Dylan rose to prominence as a songwriter; Elliott continued as an interpretative troubadour, bringing old songs to new audiences in his idiosyncratic manner. CARE Perspectives on The NOR-SWITCH Trial
Biologic therapy has revolutionized the treatment of IBD and improved patient outcomes drastically. Many of the biologic therapies we routinely use are now or soon going off patent and competitive molecules, subsequent entry biologics (SEBs), are now being introduced. SEBs have potential implications for our own practices but also for how we manage our pharmacy budgets and how we deliver health care in Canada. The NOR-SWITCH trial was a randomized, open-label trial studying a cohort of patients who were stable or in remission on the innovator biologic, infliximab (REMICADE®). Patients were randomized to either continue with infliximab or switch to a SEB version of infliximab (INFLECTRA® in Canada). This was a cross specialty trial that included patients from a variety of diagnoses, including: rheumatoid arthritis, spondyloarthritis, psoriatic arthritis, UC, CD and chronic plaque psoriasis. Each group was followed for different endpoints over the course of the trial. CARE Gastroenterology Faculty lead, John Marshall (McMaster University), sat down with Dr. Brian Feagan (Professor of Medicine at the University of Western) to critically assess this study and discuss how the results should be interpreted and applied in Canadian practice.
