Both antihistamines and corticosteroids are both medications used in allergic rhinitis. They are not cures. If you are allergic to something you may not get cured, you may outgrow the problem or may stop reacting to the particular allergen after few years of exposure. But mediation giving you cure is unlikely. The first line of treatment for allergic Reaction is antihistamines. Antihistamines are tablets like cetirizine, levocetrizine, fexofenadine. These are antihistamines that we use to control symptoms of allegoric reaction like sneezing, itching, watery eyes, runny nose and nasal obstruction. A minority of the patients, they do well only with antihistamines. The problem with antihistamines is they have certain side effects like you can feel drowsy. There are people who feel their mouth and nose an upper airway feeling very dry. Some don’t tolerate the medicines too well and can affect their morning activities and they don’t prefer using it. Two, is they act only for a certain duration of time, that is you take a tablet for today, then it may act only for 24 hours and if you don’t take the tablet tomorrow, and at the end of the day they become symptomatic. So there is a limitation of the medication’s duration of action. So long as it is there in your blood stream, your symptoms are under control and once it is secreted out of your system within the next 6 hours if you are exposed to the same allergen you are again symptomatic. So you need to be on mediation as long as you are allergic and you are symptomatic. There is a subset of people, who don’t do well with only one antihistamine, then you need to be on other medicine like corticosteroids, which are available as nasal sprays. So certain patients a combination of antihistamines, along with steroids help. Steroids are anti-inflammatory drugs. So they also act on the allergy pathway and they act topically, that is they act only on the nasal mucosa and reduce the allergic response to whatever allergen they have exposed to and hence reverses the symptoms. How long they have to be used for what duration, what are the side effects, you need to sit with your allergy specialist and ENT surgeon and they should be able to explain.

Check with your doctor or pharmacist first, and if your spray needs to be primed, spray it into the air, away from your face, a few times before using. 6. With your head upright and tilted forward slightly, press your finger against one side of your nose to close one nostril. 7. Gently insert the nasal spray into the other nostril. With your thumb supporting the bottom of the bottle, press down on the white applicator with two fingers. Breathe in through your nose and out through your mouth. 8. If using spray in both nostrils, Repeat steps 6 and 7 for the other nostril. If you are using more than one spray at a time repeat steps 6 and 7. 9. Put the cap back on the nasal spray container. 10. Your medication Fluticasone Fluticasone is also known by the brand name: Flonase. Fluticasone comes in a 16 gm bottle that contains 120 sprays. This medication is also available as an over the counter product in different sized bottles. Fluticasone is most commonly used for allergy symptoms. Fluticasone is always used in the nose. Do not take this drug by mouth. You will need to prime the pump by spraying the nasal spray into the air until you see a fine mist come out. Close one nostril off by placing your finger on top of it while you spray into the other nasal cavity. Breathe in through the nose while you press down once to release spray, and then breathe out from your mouth. Standard dosing of fluticasone ranges from 1 spray in each nostril once daily up to 2 sprays in each nostril once daily. If you miss a dose of fluticasone: • Use a missed dose as soon as you think about it. • If it is close to the time for your next dose, skip the missed dose and go back to your normal time. • Do not use 2 doses at the same time or extra doses. Usually when patients have problems with asthma/allergies/sinusitis (especially asthma) the typical conventional response is to give them medications to inhibit their immune system, mainly steroids and prednisone. As a matter of fact, it's the first line treatment that's recommended to treat people with those types of problems. They created Advair (it's a medicine called serevant and prednisone) so you don't necessarily know that there are steroids in that medication. It's a spray that works every twelve hours. What it does is that it dilates your bronchioles, gives you a bunch of steroids that would get systemically absorbed, and over time, you form less of your own steroids (or cortisol) that you would need to fight off allergies. The best way is to get at the cause of the problem, use certain vitamins and nutrients, and you can shut off some of the allergic response, and get people off some of these medications. I was on prednisone - twenty milligrams a day for two and a half years - so I know a lot about prednisone (that was for a type of joint pain and inflammation). The bottom line is: get at the source of the problem, make your body more alkaline/less acidic, and eliminate the foods and chemicals that act as triggers, and you can get people off these medications that cause all sorts of problems down the road. Conversely, less potent steroids are usually recommended for the thinner skin of the face and genital area. Prolonged use of corticosteroids can lead to side effects, including the thinning of skin (atrophy). Due to increased risk of side effects associated with potent corticosteroids, the use of Group 1 and 2 formulations is generally not recommended for periods longer than two weeks. Corticosteroids can also cause some skin conditions to worsen, including rosacea, fungal infections, and scabies. If used for longer than one month, they can create additional skin disorders including perioral dermatitis and "steroid acne." Speak to your doctor if your condition worsens while on treatment.
