Like the DNA of all living organisms, it contains the unique genetic code for all of the proteins required for bacterial survival. These include the proteins required for reproduction, growth, repair and regulation of metabolism. It also codes for the 3 kinds of RNA that are essential for carrying out protein synthesis. These are known as ribosomal RNA (rRNA), messenger RNA (mRNA) and transfer RNA (tRNA). In order for the bacteria to begin protein synthesis, the double-stranded DNA molecule must first unwind and separate in the region which codes for the specific protein that is to be made. Only one strand of the DNA serves as the template for this process known as transcription. Transcription results in the formation of messenger RNA (mRNA) which is a mirror copy of the DNA segment. Once the strand of mRNA is complete, it will detach from the DNA template and in turn become attached to ribosomes. Bacterial ribosomes are made of a small (30S) and a large (50S) subunit. After the 2 subunits join together around the strand of mRNA, synthesis of the polypeptide chain begins. This step involves the aligning of transfer RNA (tRNA) molecules in sequence along the mRNA. Each tRNA carries a unique amino acid (determined by the sequence of the tRNA) which, when aligned along the mRNA and ribosome, join together to form the polypeptide chain. This step is known as translation. The ribosome will continue to add amino acids to the growing polypeptide chain until it reaches a point along the mRNA that signals it to stop. At this point it releases the finished protein molecule. Macrolide antibiotics such as erythromycin, act as inhibitors of protein synthesis by attaching to the 50S ribosomal subunit. By so doing, they block the ability of the ribosome to synthesize the polypeptide chain. By inhibiting protein synthesis, macrolides are considered bacteriostatic antibiotics. However, at higher concentrations and with lower bacterial density or during rapid bacterial growth, macrolides may be bactericidal. Changes or modifications to the 50S ribosomal subunit (i.e. the target binding site for macrolide antibiotics) will confer resistance to macrolides and sometimes other classes of antibiotics. This type of resistance may be of a high level. This mechanism of resistance is mediated by the erm (erythromycin ribosome methlylation) gene which is found on plasmids or transposons (i.e. small genetic elements which are capable of moving from one bacterium to another and integrating into the host chromosomal DNA). Copies of the erm gene are transported to other bacteria via plasmids or transposons through "pili-like" channels. The erm gene is incorporated into the new bacterial genome. During the process of protein synthesis, this bacterium will transcribe and translate the genetic code of the erm gene resulting in the production of a protein enzyme capable of methylating the 50S ribosomal subunit at a specific position. This altered 50S subunit results in decreased binding affinity for macrolides and other antibiotics such as lincosamides (e.g. clindamycin) and streptogramin type B. This pattern of resistance is referred to as the MLS phenotype. Because the macrolide antibiotic is unable to bind to the 50S ribosomal subunit, it is unable to inhibit protein synthesis and thus the bacteria itself is not harmed, continuing to produce polypeptide chains of amino acids. A second mechanism of bacterial resistance to macrolide antibiotics is mediated by efflux pumps. These efflux pumps are encoded by the mef(A) gene which is a transposable element. Because they confer resistance to only macrolides and not lincosamides or streptogramin B they are referred to as the M phenotype. The efflux pumps are energy-dependent and for S. pneuomoniae result in moderate levels of resistance. These pumps traverse the cell membrane of the bacteria and function to "pump out" the macrolide antibiotic after it has entered the bacterium. It should be noted that for other bacteria such as Staphylococcus aureus for example, a different efflux system which is plasmid-mediated and encoded by the msr(A) gene results in macrolide resistance as well as lincosamide and streptogramin resistance in some cases. Despite the presence of these efflux pumps, macrolide antibiotics continue to enter the bacteria. However, once inside the cytoplasm of the bacteria, these efflux pumps actively remove the macrolide antibiotics before they have a chance to reach their target, the 50S ribosomal subunit and bacterial protein synthesis is unaffected.

Many (but not all) scientists believe that lupus develops in response to a combination of factors both inside and outside the body, including hormones, genetics, and environment. Hormones. Hormones are the body’s messengers. They regulate many of the body’s functions. Because nine of every 10 occurrences of lupus are in females, researchers have looked at the relationship between estrogen and lupus. While men and women both produce estrogen, its production is much greater in females. Many women have more lupus symptoms before menstrual periods and/or during pregnancy when estrogen production is high. This may indicate that estrogen somehow regulates the severity of lupus. However, no causal effect has been proven between estrogen, or any other hormone, and lupus. And, studies of women with lupus taking estrogen in either birth control pills or as postmenopausal therapy have shown no increase in significant disease activity. Researchers are now focusing on differences between men and women, beyond hormone levels, which may account for why women are more prone to lupus and other autoimmune diseases. Genetics. Researchers have now identified more than 50 genes which they associate with lupus. These genes are more commonly seen in people with lupus than in those without the disease, and while most of these genes have not been shown to directly cause lupus, they are believed to contribute to it. In most cases, genes are not enough. This is especially evident with twins who are raised in the same environment and have the same inherited features yet only one develops lupus. Although, when one of two identical twins has lupus, there is an increased chance that the other twin will also develop the disease (30% percent chance for identical twins; 5-10% percent chance for fraternal twins). Lupus can develop in people with no family history of it, but there are likely to be other autoimmune diseases in some family members. Certain ethnic groups (people of African, Asian, Hispanic/Latino, Native American, Native Hawaiian, or Pacific Island descent) have a greater risk of developing lupus, which may be related to genes they have in common. Environment. Most researchers today think that an environmental agent, such as a virus or possibly a chemical, randomly encountered by a genetically susceptible individual, acts to trigger the disease. Researchers have not identified a specific environmental agent as yet but the hypothesis remains likely. While the environmental elements that can trigger lupus and cause flares aren’t fully known, the most commonly cited are ultraviolet light (UVA and UVB); infections (including the effects of the Epstein-Barr virus), and exposure to silica dust in agricultural or industrial settings. Other examples of potential environmental triggers include: • Ultraviolet rays from the sun and/or fluorescent light bulbs. • Sulfa drugs, which make a person more sensitive to the sun, such as: Bactrim and Septra (trimethoprim-sulfamethoxazole); sulfisoxazole (Gantrisin); tolbutamide (Orinase); sulfasalazine (Azulfidine); diuretics. • Sun-sensitizing tetracycline drugs such as minocycline (Minocin). • Penicillin or other antibiotic drugs such as: amoxicillin (Amoxil); ampicillin (Ampicillin Sodium ADD-Vantage); cloxacillin (Cloxapen). • Infection, colds or viral illnesses. • Exhaustion. • Emotional stress, such as divorce, illness, death in the family, or other life complications. • Anything else that causes stress to the body such as surgery, physical harm, injury, pregnancy, or giving birth. Can anything be done to slow or prevent the development of lupus? Early diagnosis, as well as avoiding triggers for the disease, such as excessive exposure to the sun, can aid in the management of lupus. A rheumatologist -- a specialist in conditions of the muscles, joints, and bones -- can treat lupus with a number of medications that can effectively slow the progress of disease. What is a Bleach Pregnancy Test ? and How accurate is it ? Some women want to do DIY (Do It Yourself) home pregnancy tests either because they can't afford a home pregnancy test from the store, or they're into holistic medicine. There are a lot of different DIY tests out there. I've heard about a toothpaste test, vinegar, sugar, bleach, the dandelion test, and really, when it comes down to it, these are not very reliable. You have a 50/50 chance of being pregnant when you do one of those, and the results are not conclusive. The first sign of pregnancy is actually a missed period, so if your period doesn't come, that's when you should think about pregnancy, and go to the store and buy a home pregnancy test, and take it. And actually, they can be very inexpensive, you can even go to the dollar store and get one for a dollar, and those are just as accurate as the more expensive ones. But the key is just taking it after your missed period, because of how home pregnancy tests work. They work by detecting a hormone called human chorionic gonadotropin, or HCG, and that's a hormone your body starts to release early in pregnancy. And levels rise exponentially each day until about 8 to 11 weeks gestation when they peak. So you basically have to wait at least 2 weeks from the time of conception in order to get accurate results. You can get false negatives if you take a home pregnancy test too soon. But let's say you took a test on the day of your missed period, it was negative, I do suggest waiting 5 to 7 days, and then re-testing if you still haven't had your period yet, because again, false negatives are possible if the test is taken a little bit too early. Another important thing to keep in mind is read the test result within the amount of time specified by the manufacturer of the test. Most say to read it within 3 to 5 minutes, and so any result you see at that point is what it is, but a lot of girls are tempted to set it on the toilet, or they throw it in the trash, and they go look at it later, and they might see a faint line, and then they think that it means they're pregnant. But that result not reliable - only the one that you see in the timeframe specified by the manufacturer of the test. If you take a test and you have questions about the results, call your doctor. And after asking you more questions about your observations and concerns, they'll decide if further testing is warranted. In some cases, a blood test is necessary. And blood tests are much more sensitive than urine home pregnancy tests. And actually, a blood test can detect pregnancy a week after conception. So it can detect it earlier, and it's more accurate, and it can even actually give you an HCG level if they order a certain type of test. And this is necessary in some situations, but not all. So to sum it all up, the most accurate way to find out if your pregnant or not is by doing a home pregnancy test, one that you can purchase from your local store or pharmacy. And if you have questions about that result, call your doctor, and they'll order a blood test if necessary. But bleach tests, toothpaste tests, sugar, all of those other tests are not reliable, so you can't count on those results to tell you if you're pregnant or not.
