Rapidly effective regimes such as CyBorD are unable to overcome poor prognosis in very advanced AL amyloidosis (Venner et al and Palladini et al Leukemia 2014). The median survival of moderately advanced AL is ~60% at 2 years with CyBorD (Palladini et al Blood 2015). Last year, we reported a small series suggesting that oral doxycycline improves survival in cardiac AL amyloidosis (Wechalekar et al ASH abstract 2014) based on the possible cardio-protective effect of doxycycline on the heart from toxic amyloidogenic light chains. Side effects of prednisone in dogs Side effects of prednisone for dogs are not mild, and can affect multiple internal and external systems, and yield a number of behavioral changes. Unless the situation is dire indeed, prednisone and prednisolone should be avoided when it comes to treating puppies, very young dogs, and dogs that have diabetes or are pregnant. It is particularly dangerous for young dogs and puppies, since long-term use of prednisone can slow or inhibit their normal patterns of growth and restrict their progress toward physical maturity.

Urethritis is nothing but inflammation of the urethra. Woman is mainly prone for infections mainly because of the short size of the urethra, it measures about 1 and a half inches, it is mainly caused by bacteria. So what are the organs the causes urethritis? The most common organism is the E.coli and the other bacterias that are present in the stool next common is the sexually transmitted diseases caused by gonorrhoea caused by gonococci, trichomoniasis caused by trichomonas vaginalis and other sexually transmitted disease caused by chlamydia caused by chlamydia trichomatis and herpes simplex can also lead to urethritis. So these are the sexually transmitted diseases which limits itself to the urethra. Sometimes if it is not taken properly it can ascend to the reproductive organ causing pelvic inflammatory disease and future infertility. Women who is having urethritis can manifest in different ways, she can find it difficult to pass urine or initiation of the act itself may itself may be difficult or she may be passing very scanty urine and there can be painful micturition, she may experience abdominal cramps at time and there can be vaginal discharge and there can be purulent discharge from the opening. In a man he may notice blood in his semen or in the urine. So how to diagnose this particular condition by looking at her symptoms she can conclude that she is suffering from urethritis. Next thing is the examination of the abdomen and the genital examination, next thing is urine can be tested for the bacteria and the pus cells and the vaginal discharge or the discharge from the urethra can be tested. From blood investigation syphilis and other sexually transmitted disease can be ruled out majority of the patients may have these problems. This is a curable condition and complete cure can be there. Majority of the infections are because of the bacteria. So specific antibiotics can be given for these patients. If it is a gonococcal infection, then we have to treat it with aa single dose of ceftriaxone injection, 250 mg intramuscularly along with 1000 grams of azithromycin orally. Trichomonas infection can be treated with trinidazole, sednidazole, ornidazole, etc and chlamydial infection can be treated with doxycycline . Herpes simplex infection leading to urethritis can be treated with acyclovir. Majority of the patients it can be a mixed infection, where we have to use combination of drugs, so effectively all these have to be treated and if it is treated completely and complications have to be avoided. What are the complications associated with urethritis? If it is into treated timely and aggressively the infection can spread from urethra to the bladder, ureter and kidneys. It needs parenteral antibiotics. Again further if it is not treated at this stage it will spread to the blood causing sepsis and it is a life threatening risk and it has to be very aggressively managed, otherwise we may lose the patient. Sexually transmitted disease if it is not treated properly pit can ascend and affect the reproductive organs leading to PID and it may block the tubes also, ectopic pregnancy and sometimes infertility also majority of the patients should be treated diagnosed effectively, timely so that we can avoid all these complications. How to prevent these conditions. Having safe sex, avoid multiple partners, using condoms during intercourse and get yourself tested if you are having any doubt and if you are having any doubt for any infections, your partner needs to be tested. Urethral hygiene is very important. Nowadays vaginal wash or lotions are available to keep the area clean as well as to maintain the pH and regular intake of water in abundance is very important and frequent urination and not holding on to the urine is very important and post intercourse voiding urine is very important. So simple measures lie this are very important and avoid complications.

First described by George Hoyt Whipple in 1907 and commonly considered a gastrointestinal disorder, Whipple's disease primarily causes malabsorption but may affect any part of the body including the heart, brain, joints, skin, lungs and the eyes. Weight loss, diarrhea, joint pain, and arthritis are common presenting symptoms, but the presentation can be highly variable and approximately 15% of patients do not have these classic signs and symptoms. Whipple's disease is significantly more common in men, with 87% of the patients being male. When recognized and treated, Whipple's disease can usually be cured with long-term antibiotic therapy; if the disease is left untreated, it can be ultimately fatal. The most common symptoms are diarrhea, abdominal pain, weight loss, and joint pains. The joint pains may be due to migratory non-deforming arthritis, which may occur many years before any digestive tract symptoms develop; they tend to involve the large joints but can occur in any pattern and tend not to damage the joint surface to the point that the joint becomes deformed. Fever and chills occur in a small proportion of people. In its more advanced form, malabsorption (insufficient absorption of nutrients from the diet) leads to wasting and the enlargement of lymph nodes in the abdomen. Neurological symptoms (discussed below) are more common in those with the severe form of the abdominal disease. Chronic malabsorptive diarrhea leads to the poor absorption of fat, causing steatorrhea (fatty, offensive-smelling stool), flatulence, and abdominal distension. Protein-losing enteropathy may also occur, causing depletion of albumin, a blood protein, which may lead to peripheral edema caused by the lowered oncotic pressures. Hyperpigmentation of the skin occurs in almost half; some also have skin nodules. Various eye problems, such as uveitis, may occur; this is typically associated with deteriorating vision and pain in the affected eye. Endocarditis (infection of the heart valve) has been reported in a small number of cases, sometimes in people with no other symptoms of Whipple's disease; this is typically noticed as breathlessness and leg swelling due to fluid accumulation as the heart is unable to pump fluid through the body. Of those affected by Whipple's disease, 10–40% of people have problems related to the involvement of the brain; the symptoms relate to the part of the brain that is affected. The most common problems are dementia, memory loss, confusion, and decreased level of consciousness. Eye movement disturbances and myorhythmia (rapidly repetitive movements of the muscles) of the face, together referred to as oculomasticatory myorhythmia, are highly characteristic for Whipple's disease. Weakness and poor coordination of part of the body, headaches, seizures, as well as a number of more uncommon neurological features, are present in some cases. Common clinical signs and symptoms of Whipple's disease include diarrhea, steatorrhea, abdominal pain, weight loss, migratory arthropathy, fever, and neurological symptoms. Weight loss and diarrhea are the most common symptoms that lead to identification of the process, but may be preceded by chronic, unexplained, relapsing episodes of non-destructive seronegative arthritis, often of large joints. Diagnosis is made by biopsy, usually by duodenal endoscopy, which reveals PAS-positive macrophages in the lamina propria containing non-acid-fast gram-positive bacilli. Immunohistochemical staining for antibodies against T. whipplei has been used to detect the organism in a variety of tissues, and a PCR-based assay is also available. PCR can be confirmatory if performed on blood, vitreous fluid, synovial fluid, heart valves, or cerebrospinal fluid. PCR of saliva, gastric or intestinal fluid, and stool specimens is highly sensitive, but not specific enough, indicating that healthy individuals can also harbor the causative bacterium without the manifestation of Whipple's disease, but that a negative PCR is most likely indicative of a healthy individual. Treatment is with penicillin, ampicillin, tetracycline, or co-trimoxazole for one to two years. Any treatment lasting less than a year has an approximate relapse rate of 40%. Recent expert opinion is that Whipple's disease should be treated with doxycycline with hydroxychloroquine for 12 to 18 months. Microscopic Examination Extracellular Can be adjacent to basement membranes, if the deposits are more, it encroaches on cells and destroy Can be perivascular or vascular Amorphous, eosinophilic, glassy/hyaline like Should be differentiated from collagen, fibrin etc v1. Congo Red Stain Ordinary light: Amyloid appears red/pink Polarizing microscopy: Apple Green Birefringence Cross beta pleated sheet confirmation is the reason for this special staining property! Diagnosis of amyloidosis Despite strong suspicion, the diagnosis of systemic amyloidosis has to be confirmed by tissue diagnosis/ histopathological examination Abdominal fat pad aspiration /biopsy is the preferred method for diagnosis. Simplicity, low cost, less complications and good accuracy Rectal and Gingival biopsy ( previously these were preferred) or labial salivary gland biopsy. vRectal and Gingival biopsy ( previously these were preferred) or labial salivary gland biopsy. To know the type of Amyloid: Immunohistochemistry is currently the standard method for amyloid typing in routine practice.

Clark's cancer doctor is also his cousin. Dr. Skip Holden said that Clark has had Cipro before with no issues and that he uses it because the risk of infection with that biopsy is so great. Urine is a sterile fluid waste excreted by your kidneys whose normal color varies from pale yellow, caused by pigment urochrome or urobilin, to deep amber, depending on the concentration of the urine. Darker urine is usually a sign that you are not drinking enough fluid. The daily quantity of urine an adult eliminates depends on the individual's hydration, activity level, weight, health & surrounding climate. Roughly 1.5 L of urine produced daily in 6 to 8 urinations is normal. The following symptoms need immediate medical attention: -Polyuria-too much urine, more than 2.5 L/day. -Oliguria -too little urine, less than 400 mL/day. -Anuria-kidney failure to produce urine, less 100 mL per day. If your kidneys are healthy, they will produce around 6.5 cups or 1.5 L of urine a day usually in 4-8 trips to the bathroom. Most of us do not pay attention to our urine, until it looks or smells differently than usual. My advice-always look at your urine before flushing the toilet. Urine is usually pale to deep-dark yellow, depending on how much fluid you consume which affects its concentration. To preserve the health of your kidneys drink plenty of water, eat home-made, freshly-cooked food, eat more fruits & vegetables than animal proteins, reduce salt, potassium, phosphorus, alcohol, & coffee, avoid pre-packaged foods and drinks, exercise daily and keep a normal body mass index, BMI.

Giampaolo Merlini, MD of Fondazione IRCCS Policlinico, San Matteo Pavia, Italy gives an overview of his talk on light-chain amyloidosis or AL amyloidosis at the 2016 European Multiple Myeloma Academy (EMMA) in Madrid, Spain. Although it is a rare disease, it represents 12–15% of patients with multiple myeloma (MM). Further, there is a condition called primary AL amyloidosis, where there is a small clone in the bone marrow that produces a protein (a light chain) with an abnormal confirmation and, due to this, the protein tends to aggregate; it can move to the kidney, heart and other organs where it causes progressive dysfunction and damage. It is possible to recover completely if intervention occurs in time; early diagnosis can change the outlook for the patient. Therefore, time is of the essence, as Dr Merlini explains, and it is vital for doctors to consider that their patient may be affected. Dr Merlini further highlights how this concerns the management of monoclonal gammopathy of undetermined significance (MGUS) patients. Dr Merlini further outlines how the diagnosis is made and that there are several proteins that can cause systemic amyloidosis. The second-most common one is produced by the liver; it is called transthyretin and can also affect the heart, in particular, in older men (known as wild-type ATTR amyloidosis). The available treatments now are extremely powerful, according to Dr Merlini. The treatment of AL amyloidosis is the most successful treatment of all types of amyloidosis because it is possible to suppress the production of the proteins that is causing the disease with drugs. Therefore, the production of light chains is stopped and this usually translates into very rapid improvement in the clinical condition of the patient. If the treatment is started early, organ dysfunction can be recovered and survival is extended; more than 30% of patients survive more than 10 years, which was unthinkable a few years ago. Further, the same drug that is used for treatment is also being developed in myeloma as the clone is very similar. This programme has been supported by Celgene and Amgen through an unrestricted educational grant to the Video Journal of Hematological Oncology.
