Early Warning Signs of Pancreatic Cancer Diabetes, especially if it comes on suddenly. Recently, the Mayo Clinic published startling research showing that 40 percent of pancreatic cancer patients had been diagnosed with diabetes one to two years before discovering they had a pancreatic tumor. Researchers believe the diabetes is caused by tumors that simply haven't been detected yet. The problem is, diabetes is very common, and the majority of diabetes isn't pancreatic cancer, so doctors are trying to develop screening tools to tell the difference. Right now, they say family history is an important clue. If you're diagnosed with diabetes that seems to come on suddenly and you have no family history of diabetes, bring this to your doctor's attention and ask for further screening for pancreatic cancer. Yellowing of the eyes or skin. Even a small pancreatic tumor can block the bile duct in the head of the pancreas, causing bile to build up. This causes jaundice. Itchy skin, palms, and soles of feet. A little-known side-effect of jaundice is itchy hands and feet. It's due to a skin reaction to the bilirubin, the yellowish brown liver chemical that causes jaundice. Lack of appetite. An Italian study found that six to eight months before being diagnosed with pancreatic tumors, patients reported a sudden drop in their appetite and a tendency to feel full after eating very little. Changes in taste. In the same Italian study, some of the patients surveyed said they'd suddenly lost their taste for coffee, wine, and smoking. In fact, they said, they felt "disgust" for the smell and taste of coffee and alcohol. Abdominal pain. Pancreatic cancer sufferers remember this pain as a gnawing pain, rather than a sharp cramp or ache, and it radiates toward the back. A characteristic clue: the pain goes away when you lean forward. An enlarged gall bladder. The same blockage of the bile duct that causes jaundice can also cause an enlarged gallbladder, as the bile builds up behind the duct. The good news is that an enlarged gallbladder can be seen on imaging tests, and it may even be possible for a doctor to feel it during a physical exam. Pale, floating, smelly stools. If a pancreatic tumor prevents digestive enzymes from reaching the intestine, the result is an inability to digest fatty foods. So you end up with loose, smelly "floaters" as a result of the excess fat. Doctors say this symptom, in particular, can be an early clue and is too often overlooked. Dark, tarry stools. Bleeding in the upper intestines causes this symptom. Sudden, unexplained weight loss. Weight loss is not always, as many people mistakenly believe, a sign of advanced cancer that's spread to the liver. It can also happen because a lack of pancreatic enzymes is causing fat to pass through the body undigested. New videos everyday! On July 3, 2015 he was diagnosed with stage IV pancreatic cancer. The next day he asked me to record this video diary. I thought this was going to be a long-term project focusing on a 49-year-old man fighting a disease. I was wrong.

Teri shares her family's story about her father's diagnosis with pancreatic cancer and his death in less than three months after diagnosis. Her father had always lived a healthy life, ate a good diet and was physically active. Teri noticed he looked ill and took him to UCLA where he was diagnosed with pancreatic cancer. She helped him search for options but learned he was past the point of surgery. Chemotherapy had no good outcome, in fact his tumors doubled. Shortly after, Teri's father died. She met Agi Hirshberg, founder of the Hirshberg Foundation, who talked with Teri about her experiences. Teri dove into work with the Foundation; it actually became her therapy as she says. She helped the Hirshberg Foundation plan events to raise funds for pancreatic cancer research and started a charity in her father's name to support further investigation and help spread awareness about this disease. Teri states, "My passion is that no one should go through what my dad did. He was gone in under three months; this should not happen to anyone. I had no knowledge of pancreatic cancer, but learned more than I need to know. In the last eleven years we've come a long way, but we are nowhere near where we need to be. I'll continue to fight till we find that cure."

Chalotte Rae, familiar to millions of TV viewers as the housemother on the '80s sitcom The Facts of Life, was diagnosed in 2009 with pancreatic cancer, an often-silent killer with few if any symptoms until it is too late. Some 40,000 new cases of pancreatic cancer are diagnosed each year, and by the time diagnosis is made, up to 80 percent of patients are no longer candidates for treatment. "I had no symptoms," Rae says. "I had absolutely no symptoms. None whatsoever." But Rae was, in a way, lucky. Because she had a family history of the disease - her mother, uncle and older sister all died from the disease - she underwent early screening, which detected the cancer at an early stage. "We're working hard to develop tests for earlier diagnosis," says Howard Reber, M.D., director of the UCLA Pancreatic Cancer Program. The goal is to create something similar to the PSA test now done to detect prostate cancer in its early stages, before it has had a chance to spread. "In patients where we know that there's an increased likelihood of the development of the disease, we can screen them, we can get CT scans, we can get endoscopic ultrasounds," Dr. Reber says. In Rae's case, the cancer was detected and found to be contained, but it was growing fast. Surgery was performed to remove the cancer, and now, following surgery and chemotherapy, Rae is cancer free. While health experts say pancreatic cancer is uncommon, there are 52,000 new cases diagnosed each year. It’s the third most common cause of cancer related deaths. But how it’s caused is a question medical experts are still trying to answer. Dr. Mark Bloomston, a surgical oncologist on staff at Lee Memorial Health System, says pancreatic cancer is difficult to diagnose. “If we could figure out what’s causing pancreatic cancer then maybe we could make some moves to prevent it. “ He says there are certain risk factors that have been identified like family history, smoking, obesity, diabetes, and an unhealthy lifestyle. “It doesn’t seem to select a race, age, or gender, which is kind of the difficult thing about it because you can’t pick out a population that’s at the highest risk.” Some patients will experience symptoms like weight loss, jaundice, chronic inflammation of the pancreas, and abdominal pain. “Unfortunately, about 90 percent of them, by the time they’ve realized they have the cancer it has already spread and surgery is not an option,” said Dr. Bloomston. But he says if it’s caught in time, the only treatment is surgery to remove the pancreas. “You can actually live without your pancreas. So you can remove the entirety of the pancreas, we prefer not to because it does instantly cause diabetes.” For those at a higher risk, health experts recommend screening using an endoscopic ultrasound. If the tumor is found in time chemo therapy can be used instead of surgery. “Wer’re finally seeing in the last few years chemo therapy that’s starting to work, not to cure it, not to get rid of it, but we are seeing tumors start to shrink, stabilize and become removable.” Giving patients a better chance. Pancreatic cancer begins in the tissues of your pancreas, a pear-shaped organn located behind the lower part of your stomach Pancreatic cancer is difficult to detect early and it typically spreads rapidly, affecting other parts of the body. It is common for people with pancreatic cancer to experience jaundice, characterized by yellowing of the eyes or skin. Every death is individual, just as we are all different in life. There are a few common changes that happen. Knowing what these changes are and how to manage them may be helpful. It is usual and normal for people to experience these changes when death is near: More tiredness and sleeping more Eating and drinking less Less passing urine and faeces Being more drowsy and less alert There is no need to be worried or concerned about these changes as they are part of the usual process of preparing for death. Eating and drinking – If the person who is dying does not ask for food or drink or declines them there is no need to worry or try to force them to eat or drink Mouth care – The person who is dying might not feel thirsty but their mouth can become dry and this can feel uncomfortable. You can help keep their mouth moist by giving them ice chips to suck or using a moistened swab [demonstrate use of swab] Breathing – Sometimes people can have very noisy breathing; this breathing can sound very alarming but the person who is dying is not usually distressed by it. There is medication that can reduce this sound. Pain and agitation – There is medication that can reduce these symptoms Confusion – The person who is dying may be a bit confused. There is medication that can assist with this. You can also assist by telling them where they are and that they are being looked after. Confusion may be lessened by playing music that the person likes and having family members (familiar faces) around (especially if the person is in an unfamiliar environment such as the hospital). Changing position – An air bed or a hospital bed and air mattress can be provided to help the person who is dying to feel comfortable in bed and to help you help them to change position every few hours. A person can be close to death but not yet passed away for days or sometimes weeks. Sometimes a person can move close to death but then recover again; they may move in and out of this stage. Even if a person wants to die at home, sometimes the final stages can be too much; it can be overwhelming. It is okay to call for help and ask for the person to be admitted to the local hospital. There are palliative care rooms available at Ararat, Maryborough, Willaura, Stawell, St Arnaud, and Beaufort. These rooms provide some privacy and opportunity for you to be with the dying person and nursing staff are available to assist you in caring. There may be the opportunity for a carer to sleep overnight at the hospital with the person who is dying – if this is what you would like to do, ask the staff about this option. What are the symptoms? How do you know if you have pancreatic cancer? Should you be screened? Over the next several minutes, I will give an overview of pancreatic cancer. Pancreatic cancer is the fourth leading cause of cancer-related deaths in the United States. The pancreas is the organ that sits in the central upper abdomen, is located right behind the stomach organ. The pancreas is about six to eight inches long and about two inches wide, and it extends from the head of the pancreas, which is on the right side of the abdomen, connected to the small intestine, near the gallbladder and the liver, and the pancreas extends to the tail of the pancreas, which is more on the left side of the abdomen. The pancreas is the organ that's responsible for producing certain hormones such as insulin, which regulates your blood sugar. The pancreas also produces certain digestive enzymes or digestive juices, which help us to digest fat. There are two types of pancreatic cancer. One, which comes from the endocrine glands or the hormone-producing cells such as an insulinoma. The other type of pancreatic cancer comes from the exocrine glands or from the pancreatic ducts, and this exocrine gland pancreatic cancer is the most common type of pancreatic cancer. What are the symptoms of pancreatic cancer? One is weight loss. Patients with pancreatic cancer often have early satiety, meaning they get full quickly off of just small amounts of food, or they may just lose their appetites altogether. Another symptom, abdominal pain. Patients often have a dull, aching abdominal pain in the central upper abdomen, and it often radiates to the back. Another symptom, jaundice. Jaundice occurs when the patient's skin turns yellow or the whites of the eyes turn yellow, and this is due to a blockage of the flow of bile by the pancreatic cancer. Patients who have jaundice, this yellow skin, will often also have a change in their stools. How can you be diagnosed with pancreatic cancer? If you have any of the symptoms I described, it is important that you consult your physician as soon as possible. Your physician will do a complete history and physical, and they will likely do some imaging studies such as an ultrasound of the pancreas or a CT, computed tomography, of the pancreas or a test called an ERCP, endoscopic retrograde cholangiopancreatography. If they are suspicious or if they see a mass that looks like pancreatic cancer, then they may do a biopsy, which is when they have a physician to take a small sample of the mass, and a pathologist is another type of physician who will look at that and determine if it is or if it isn't pancreatic cancer. The treatment for pancreatic cancer. The treatment really depends on the staging. If the cancer is caught early, there's a chance that you can have surgery or surgical removal of the cancer. and patients often will have an adjuvant therapy, which means they will receive chemotherapy and/or radiation, especially if the pancreatic cancer has spread. In cases of stage four or metastatic pancreatic cancer, a lot of those patients will also receive chemotherapy. However, that chemotherapy is usually just to shrink the tumor or to help to decrease the symptoms of the tumor. Stage four pancreatic cancer is usually not curable. So what are the risk factors for pancreatic cancer? Well, five to 10 percent of patients who have pancreatic cancer also have family members with pancreatic cancer. So, we do believe that there is some hereditary component. Other things that have been associated with pancreatic cancer include cigarette smoking, obesity, physical inactivity, the Helicobacter pylori infection. But quite honestly, we really don't always know the cause of pancreatic cancer. This ends my overview of pancreatic cancer symptoms, signs, and risk factors. I want you to take care of yourselves, and if you have any of the symptoms that I described, please consult your physician immediately. Those symptoms are weight loss, abdominal pain, jaundice. Pancreatic cancer is the fourth most common cause of cancer related deaths in the United States. It is the second most common cause of gastrointestinal cancers, behind colon cancer. Patients often do not experience symptoms until the cancer involves the surrounding blood vessels or spreads beyond the pancreas, and they are initially treated with chemotherapy and occasionally radiation therapy. Surgical treatment is offered to patients with early stage tumors that are confined to the region of the pancreas; it includes a Whipple resection, which is a complex surgery. This lecture will address the surgical and medical treatment of patients with both early and late stage disease. Up to date information on improved survival associated with chemotherapy regimens and biologic agents will be reviewed. We will also highlight the diverse resources that are available within the UCLA Center for Pancreatic Diseases, including clinical trials, palliative treatments and support services. When patients diagnosed with cancer are admitted to the hospital, many times we see them at their lowest. From chemotherapy and radiation treatments to surgeries and diagnostic test. It can be an emotional roller coster for not only the patient but also all of their loved ones… including their medical staff… I remember about 2 months ago, I was feeling down. Normally I’m an extremely upbeat person. However, working on the oncology unit was taking it’s tole on me. I realized, that I was mistakingly focusing on some of the patients who didn’t make it. Unfortunately, many of the patients who recover fully do not make their ways back in the the hospital. I can’t blame, the hospital would not be high on my list also. Recently, I attended a fundraising event with my husband and I heard a beautiful dynamic speaker by the name of Camille. Don’t tell anyone, but I actually cried while she was talking… By the time she finished speaking, I knew I had to ask her to share her story on my channel. I waited for 2 hours to talk with her and after the crowds had died down, she graciously accepted my offer. Spending time with Camille was like taking a breath of fresh air. Imagine walking into someones home for an interview and a bubbly 60 year old greets you with tons of energy, gorgeous hair and a wholesome outlook and love for life. This is who Camlile is, just a loving bundle of joy - I left her house inspired and grateful. I hope you feel the same way after listening to her story. Camille was given one of the worst diagnosis… Had I been reading her chart, I unfortunately would have not spent too much time with it… I have never met anyone that survived Pancreatic Cancer, much less; Stage 4 Pancreatic Cancer with metastasis aka METS. The first doctor she went to told her that she that with stage 4 Pancreatic Cancer with spots on in her lungs, liver and kidneys; there was nothing they could do and she was to go home and prepare to die. However, one thing that she strongly recommends is to get a second opinion, which is what she did. She went to University of Miami’s Sylvester Comprehensive Cancer center. She found a doctor that believed in her and she trusted him. She will be the first to tell you that her story is not the norm and oftentimes told me how it hurts her to know that many others have had the same diagnosis and not the same outcome. However, her main focus is to share that no matter how bad things seem, you can always and MUST always have hope. Thankfully Camille is not alone these days. Diabetes, especially if it comes on suddenly. Recently, the Mayo Clinic published startling research showing that 40 percent of pancreatic cancer patients had been diagnosed with diabetes one to two years before discovering they had a pancreatic tumor. Researchers believe the diabetes is caused by tumors that simply haven't been detected yet. The problem is, diabetes is very common, and the majority of diabetes isn't pancreatic cancer, so doctors are trying to develop screening tools to tell the difference. Right now, they say family history is an important clue. If you're diagnosed with diabetes that seems to come on suddenly and you have no family history of diabetes, bring this to your doctor's attention and ask for further screening for pancreatic cancer. 2. Yellowing of the eyes or skin. Even a small pancreatic tumor can block the bile duct in the head of the pancreas, causing bile to build up. This causes jaundice. 3. Itchy skin, palms, and soles of feet. A little-known side-effect of jaundice is itchy hands and feet. It's due to a skin reaction to the bilirubin, the yellowish brown liver chemical that causes jaundice. 4. Lack of appetite. An Italian study found that six to eight months before being diagnosed with pancreatic tumors, patients reported a sudden drop in their appetite and a tendency to feel full after eating very little. 5. Changes in taste. In the same Italian study, some of the patients surveyed said they'd suddenly lost their taste for coffee, wine, and smoking. In fact, they said, they felt "disgust" for the smell and taste of coffee and alcohol. 6. Abdominal pain. Pancreatic cancer sufferers remember this pain as a gnawing pain, rather than a sharp cramp or ache, and it radiates toward the back. A characteristic clue: the pain goes away when you lean forward. 7. An enlarged gall bladder. The same blockage of the bile duct that causes jaundice can also cause an enlarged gallbladder, as the bile builds up behind the duct. The good news is that an enlarged gallbladder can be seen on imaging tests, and it may even be possible for a doctor to feel it during a physical exam. 8. Pale, floating, smelly stools. If a pancreatic tumor prevents digestive enzymes from reaching the intestine, the result is an inability to digest fatty foods. So you end up with loose, smelly "floaters" as a result of the excess fat. Doctors say this symptom, in particular, can be an early clue and is too often overlooked. 9. Dark, tarry stools. Bleeding in the upper intestines causes this symptom. 10. Sudden, unexplained weight loss. Weight loss is not always, as many people mistakenly believe, a sign of advanced cancer that's spread to the liver. It can also happen because a lack of pancreatic enzymes is causing fat to pass through the body undigested. What do you do if you're worried about any of these symptoms? Document all symptoms, and report them to your doctor in as much detail as possible. If your doctor is convinced you have legitimate concerns (and remember, you may have to do some convincing), tests such as an ultrasound, a CT scan, and an endoscopy followed by a biopsy can be used to search for a pancreatic tumor. A blood test is now available for a biomarker called CA 19-9 that is released by pancreatic cancer cells. Unfortunately, by the time CA 19-9 reaches detectable levels, the cancer is usually no longer in the early stage, although the test is useful during treatment and for detecting recurrence. Another biomarker test, CEA (carcinoembryonic antigen), is also not sensitive enough to provide early diagnosis. While tragic early deaths like Steve Jobs' are the hard reality of pancreatic cancer, there are also people living productive lives thanks to an early diagnosis of pancreatic cancer. Such was the case with Supreme Court Justice Ruth Bader Ginsburg, who's still practicing after having a tiny pancreatic tumor surgically removed. Dr Eva Rossina Duarte Juarez (Guatemala) agrees that cancer is indeed painful. "In the situation of advanced illnesses, people need to be able to die in peace." Breast cancer patient Carmen (Australia) reflects that she doesn't fear death however she did fear the essential medications designed to treat her cancer pain. "I had a lot of pain on every level, whether it was physical or emotional." Dr Mark Boughey (Australia) of Caritas Christi Hospice in Melbourne explains, "When Carmen got here pain was the big issue... She had that really problematic neuropathic pain so you couldn't even touch her skin without her screaming and being distressed." "This problem is very much with us, it's a global problem, but also there are things we can do to bring the escalating problem of cancer under control," Professor David Hill. Carmen reflects that professional treatment of her pain brought back her sanity and quality of life. "I'm very blessed to have had this opportunity ... I've had really good care." Pancreas locationThe pancreas is an organ about 6 inches long and shaped like a thin pear lying on its side. The wider end of the pancreas is called the head, the middle section is called the body, and the narrow end is called the tail. Where is the pancreas? The pancreas is found deep inside the body, behind the stomach and in front of the spine. It has two main jobs in the body, making: Enzymes – these help to digest (break down) foods. Hormones – such as insulin and glucagon, which control blood sugar levels. In this way, the pancreas helps the body use and store the energy it gets from food. A tube called the pancreatic duct connects the pancreas to the first part of the small intestine, known as the duodenum. Digestive enzymes pass through this tube to help break down food. Another tube, called the common bile duct, passes through the head of the pancreas. This tube carries bile – a substance that helps to digest fats – from the liver and gall bladder to the small intestine. The bile duct may get blocked when a pancreatic tumour invades it. This causes jaundice (yellowing of the eyes and skin and dark urine). The pancreas contains two types of glands: Exocrine glands – create the enzymes which help digest (break down) foods.
