At around 3 p.m., riding in a 2013 Chevrolet Malibu, Moreno and her best friend, Ashley Theobald, took a selfie only moments before the crash. The two decided to overtake a truck on the two-lane road because it was emitting black fumes and causing discomfort to Moreno who suffered from asthma. However, the two did not see an oncoming 1990 Dodge Ram that had just crested the hill until it was too late. Both vehicle tried to swerve to avoid each other, but the Ram struck the Malibu's passenger side. Theobald and the driver of the Ram, 25-year-old Melvin Garber, escaped with minor injuries, while Moreno was taken to a Columbia hospital and was later pronounced dead. Moreno was set to marry her fiancé on July 26th. But it’s not just adults who are at risk. Colby was 6 years old when he suddenly collapsed in his first grade classroom. His heart had stopped beating. Dr. Naomi Kertesz, Director of Electrophysiology and Pacing at Nationwide Children’s Hospital, says Colby is alive today because his teacher knew how to use an AED. Automated External Defibrillators can shock the heart back into its normal rhythm. A person’s chance of survival decreases 10% each minute that AED use is delayed. “You have five minutes,” says Dr. Kertesz. “After five minutes, you are likely to have an irreversible brain injury.” Dr. Kertesz is the medical director of Project ADAM Ohio, a chapter of a national organization formed in 1999 after the sudden death of teen Adam Lemel on a basketball court. She is passionate about making AEDs more accessible in schools and on athletic fields. “It’s not only about saving someone’s life. It’s about making sure they return to the same person they were before they had their sudden cardiac arrest,” Kertesz emphasized. Dr. Kertesz says it’s important for people to do three things if someone suddenly collapses. 1. Call 911 2. Get somebody who knows CPR 3. Get the AED Colby is a living, breathing example of the difference an AED can make in an emergency. Today, he remains in Dr. Kertesz's care at Nationwide Children's. But instead of having a life-long brain injury caused by his sudden cardiac arrest, Colby is an honor roll student, an athlete and a typical little boy who wants to be a cowboy when he grows up.

Asthma is a growing concern, especially in athletes. Learn the reasons why athletes have trouble breathing, and what can be done about the problem. Though asthma is a lung symptom it is often not an actual lung cause. Asthma can be a symptom of a variety of causes such as under-functioning adrenal glands, immune system issues, inflammation, poor diet, digestive issues, or emotional stress. Dr. Gangemi explains why it’s important to identify the causes of exercise induced asthma. It’s common to treat the symptoms of exercise induced asthma with corticosteroids like Flovent and Qvar, or an asthma inhaler like Proventil or Albuterol. Other inhaler types include antihistamines, leukotriene modifier inhalers, and anti-inflammatory medications. However, an important problem with asthma medications is that they can further strain the adrenal system. So, as a natural treatment for asthma, it may be important to identify why your body is lacking the natural corticosteroids to begin with. Beyond natural remedies for asthma, Dr. Gangemi is encouraging a more holistic chronic asthma treatment approach. This can provide long-term lasting benefits as an asthma treatment for children, for athletes, or for anyone who needs to take a puff in order to function, play sports, or to exercise.

When Zahra was seven years old her Mom suggested that the whole family go back to Iraq to see her side of the family, and she wanted to stay for awhile so that Zhara and her sisters and brotherts could really get to know them. Her Dad thought it was a crazy idea - that her Mom was out of her mind - after all, there was a war going on, it was dangerous, and not everyone there in Iraq was guaranteed to survive. But her Mom felt very strongly about it - she wanted Zahra and her sisters and brothers to see the place where she had been born and raised, where her family was - their grandparents and other relatives. And so, finally, their Dad gave in and agreed that they would go - war or no war - and they moved. Everything was fine for a couple of years, until Zahra turned nine. One night - it was actually 4:00 AM - Zahra woke to the sounds of yells, screams, and crying. She was confused at first, but something was definitely going on outside on the street. She went to the window and there, across the street, an entire building was on fire. She rushed into her Mom's room, woke her, and dragged her down the hall to her own room to show her. Her Mom let out a scream and then went quickly to wake the rest of the family. It didn't take long for them to realize that the war had come to them, their city was being attacked. Bombs were falling. There was nothing much they could do, in order to survive they needed to shelter in place and try to wait it out for as long as they could. But, after a few weeks they were running very low on food - they had been inside trying to avoid the war and survive since those first bombs fell. Her Dad saw that they wouldn't survive much longer without food and supplies and so he and Zahra's older sister packed some gear, gave out hugs and kisses to the family, and set out into the war torn city in search of food. After four days without any sign or word from her Dad and sister her Mom started to panic and they all started to fear for their safety and survival. They began to lose hope as time continued to drift by without their Father and sister returning, and finally, they felt there was nothing they could do except to pray, and so they did - and, a miracle, a mixed blessing, but a blessing nonetheless - their father appeared out of the smoke and haze of the street, alone. He was injured, exhausted, and not in good shape, crying - Zahra's sister was gone, forever. It has taken time, but Zahra has faith and a strong belief, and she knows that there was a purpose for her sister leaving this earth. And, she realizes that now her sister will be with her, by her side, forever and no matter what. As soon as they could they moved back to England, and now at thirteen years old, she is happy again. Her life is good and full of blessings. She survived the war but lost her sister, and she misses her of course, but, she also knows now that no matter how tough things get, even in war, they can and do get better. - - - What's your story? storybooth wants to hear it! These are the same immune processes that are necessary to prevent infection, but they create problems when they are hyperactive or misguided. The way I remember the four different types of hypersensitivity is my ABCD mnemonic. I = Allergic Anaphylaxis and Atopy II = antiBody III = immune Complex IV = Delayed * Types I, II and III all can involve antibodies. Therefore, having antibody in the mnemonic for type II may be confusing. However, the “classic” role of antibody involving complement activation, NK cell activation and opsonization are part of Type II which is why I feel that is the simplest way to remember it. Type I Hypersensitivity is triggered by an innocuous foreign substance (like dust, pollen or animal dander) that would cause no problems in the majority of people. The antigen simultaneously binds more than one membrane bound IgE on a mast cell (or basophil). This process, called Crosslinking, triggers the release of mast cell granules full of histamine. Histamine then goes on to signal the various changes associated with allergies similar to how it functions during acute inflammation. This allergic reaction happens almost instantly and the symptoms can become evident within minutes. This is possible because the mast cells are Pre-Sensitized to the innocuous substance which means they have pre-formed membrane bound IgE that recognizes the particular innocuous antigen. To be pre-sensitized the immune system must have seen the antigen previously, because antibody (IgE) formation takes time. Histamine’s role in Type I Hypersensitivity is why Anti-Histamines (Histamine Antagonists) like Diphenhydramine or Loratidine can control some allergy symptoms. Type I Hypersensitivity is the process that leads to various different “allergies”. A more mild form would include Allergic Rhinitis (seasonal allergies) that cause things like coughing, sneezing, watery eyes and nasal congestion. Some individuals with these allergies also experience an atopic rash referred to as Urticaria (Hives) or Atopic Dermatitis (Eczema). This red rash is raised and pruritic (itchy). Through a similar mechanism Type I Hypersensitivity can lead to exacerbation of Allergic Asthma by environmental triggers. This type of hypersensitivity is also the mechanism behind more serious conditions like peanut or bee sting allergies that can lead to swelling of the lips/tongue/throat, shortness of breath, stridor, and anaphylactic shock. Anaphylatic shock is a life threatening condition in which systemic histamine directed inflammation causes hypotension via global vasodilation, an increase in vascular permeability and significant fluid movement into the tissue. It is treated with epinephrine, often in the form of an “Epi Pen” when outside of a medical setting. Epinephrine activates Alpha-1 Adrenergic Receptors to raise blood pressure via vasoconstriction and increased contractility of the heart. Type II Hypersensitivity is the process by which IgG or IgM binds to a cell to cause injury or death (Antibody Dependent Cytotoxicity). This process has the same mechanism of action as normal humoral immunity except it is targeted at the body’s own cells instead of pathogens. The variable region of the antibody binds to the host cell while the constant portion interacts with NK cells, complement and macrophages. Examples of this reaction can be seen in Rheumatic Fever (body’s own cell look similar to Strep Pyo), Goodpasture Syndrome (Anti-Glomerular Basement Membrane Antibodies) and Hemolytic Disease of Newborns/Erythroblastosis Fetalis (when a Rh- mother has a second Rh+ child and the maternal IgG targets fetal RBCs). Pertussis/whooping cough Whooping cough is caused by a bacterium called Bordatella pertussis. It is transmitted through respiratory droplets. During the 1900s, it was one of the most common childhood illnesses in the United States with an excess of 200,000 cases reported per year prior to the vaccine. It is still a major cause of death worldwide with about 185,000 deaths in 2008. Although it has several mechanisms of injury, it is primarily a toxin-mediated disease. It has three main stages. Stage one is called the catarrhal stage with runny nose, sneezing, low-grade fever and occasional coughing. The second stage is called the paroxysmal stage and begins about two weeks after illness. It is characterized by bursts of rapid coughing followed by a deep respiratory effort, which makes the characteristic whoop sound. During these attacks, the patient may turn blue in color. The cough is worse at night and this stage may last for 1 to 6 weeks. The last stage is the convalescent stage with a gradual recovery time taking up to 2 to 3 weeks. Complications due to whooping cough include pneumonia, seizures, encephalopathy, hospitalizations, and death. Most deaths occur in infants less than three months of age. Vaccines were developed in the 1940s as a whole cell vaccine. Severe reactions were noted with this vaccine such as fever greater than 105, frequent febrile seizures and crying lasting for longer than three hours. Since 1991, we have been using acellular pertussis vaccine, which has had much fewer side effects. Frequent side effects include pain at injection site with a local reaction and fever. Vaccine should not be given to any person with a history of allergic reaction to any components of the vaccine, history of febrile seizure vaccine or fever above 105 following vaccine. Vaccines are typically given at two months, four months, six months, 12 to 15 months, with boosters given at 4 to 6 years, 11 to 12 years, and every 10 years in combination with tetanus. Known cases of whooping cough are treated with erythromycin. Erythromycin has been found to decrease the time that it is infectious and if started early enough, it can modify the disease. Immunity from disease and vaccine does not seem to be life-long and most children that acquire pertussis contract it from older siblings or parents. Because of this, it is now recommended that parents receive booster vaccines for pertussis prior to giving birth to a newborn.

Researchers in Canada said that of more than 600 adults diagnosed with asthma, one-third did not have the disease based on objective tests. Eighty percent of those people had been taking asthma drugs. That included 35 percent who were taking medication every day, the investigators found. Smoking Still Kills proposes new targets for a renewed national strategy to accelerate the decline in smoking prevalence over the next decade. The report sets out short-term objectives and longer term aims and develops the agenda for tobacco control launched in 2008 with Beyond Smoking Kills. A key recommendation of Smoking Still Kills is for the Government to impose an annual levy on tobacco companies and for the money raised to be used to pay for measures such as mass media campaigns and stop smoking services. The recommendations have been developed by an editorial board in consultation with an advisory board of academics and experts, and following feedback from four regional events with local and national tobacco control professionals. Smoking Still Kills has been endorsed by 129 national and local organisations. Injuries To Health Tobacco smoke contains more than 7000 chemical compounds. Many of these agents are toxic, and more than 69 - including polyaromatic hydrocarbons (PAHs) and tobacco-specific nitrosamines - cause cancer. See also the Safety section for more information. Smoking Kill to People Lung Cancer More people die from lung cancer than any other type of cancer. Evidence is mounting that using the pain and fever reliever acetaminophen, which is known under several trade names that include Tylenol and Paracetamol, may increase the risk of developing allergy and asthma symptoms. Two such studies are published online in the American Journal of Respiratory and Critical Care Medicine. If you have asthma, here are some tips to help you control your symptoms: • Be sure you are well- educated about asthma, including its triggers and treatments • Always take your asthma medications as directed, even if you are feeling fine • Know your triggers and how to avoid them Researchers from the Medical Research Institute of New Zealand representing the International Study of Asthma and Allergies in Childhood, or ISAAC, administered two written questionnaires and one video questionnaire to over 300,000 children aged 13 and 14 from 113 centers across 50 countries. The questionnaires were about their use of acetaminophen as well as their asthma and allergy symptoms. A significant relationship between use of acetaminophen and asthma and allergy symptoms was uncovered. Among "high" users of acetaminophen, which was defined as those who had used the drug at least once in the previous month, the risk for asthma was more than 2.5 times greater than for non-users of the drug. Similarly, "medium" users, or those who reported using acetaminophen at least once in the previous year, had a 43% increased risk for asthma. Also compared with non-users, high users of acetaminophen had a 2.39 times increased risk and medium users a 38% increased risk for hay fever. In addition, high users were about twice as likely and medium users 31% more likely to have eczema as non-users. It is impossible to tell from the above study whether the relationship between acetaminophen use and allergy or asthma symptoms is causal. Research out of Ethiopia, however, suggests that it is. This study demonstrated that, in general, the use of acetaminophen preceded the development of allergy and asthma symptoms.
