We apologize for any inconvenience this may cause. My name is Michael Agus, I'm a pediatric intensive care doctor and a pediatric endocrinologist at Boston Children's Hospital. I'm the Director of the Medicine Critical Care Program here Children's and an assistant professor at Harvard Medical School. Hyperglycemia. Why does one get hyperglycemic in DKA? Why does the blood sugar go up at all? Well it's really the same reason you get hyper- anything-emic in any clinical situation. It's some mismatch of increased-ins and decreased-outs. So what are the increased-ins of glucose into the blood stream? Well one major one is the one that patients are doing at home. What are they drinking at home? In general they're drinking Pedialyte or juice or some dextrose containing fluid and they don't realize that they are driving their blood sugar higher and higher. The other actor that's adding glucose to the bloodstream is the liver. The liver believes the blood sugar is extremely low because it can't get any glucose interstitially because of the insulin deficiency and so it turns on gluconeogenesis at full throttle. And of course that's also what produces the ketoacidosis: the lipolysis freeing up those circulating keytones and free fatty acids as well. Now where should the glucose be going? Well if insulin were present there would be normal cellular uptake. But with a decrease insulin production increased insulin resistance there is a functional severe insulin deficiency and so there is dramatically decreased cellular uptake. Now these three factors will give you blood sugars in the mid to high 100s. What really pushes you over the edge is when you begin to decrease perfusion to the kidneys. We know that the renal threshold ia about 180, and if your blood sugar is anything over 180 and you are adequately hydrated eventually the kidney will dump all that glucose. It may get dehydrated in the process and you'll need to replete those fluids. But the glucose should come down. In the setting where you begin to hypoperfuse the kidneys, that's when the glucose really shoots up, and that's when the glucose will hit 800, 1,000, 1,200 milligram per deciliter range. One important caveat to the construct I've mentioned here is the middle aged or adolescent patient who happens to be at home rehydrating with water. That's the one patient where the glucose may not be as elevated, the glucose may be only in the 400 range, 500. They may have quite significant diabetic ketoacidosis, however, they are not as dehydrated and the usual patient with DKA. These are rare patient but it's a patient where you really want to think twice when the blood sugar is not that high to ask yourself the question, maybe the kidneys are a little bit better perfused than I'm used to and maybe they don't need the amount of fluid that I'm about to to give. But for most patients they come to the emergency department, they have a blood sugar in say the 1,000 range and they will get their normal saline bolus, usually ten milliliters per kilogram over the first hour. And that will almost invariably, significantly drop their blood sugar because we saw on the prior slide we're now re-perfusing the kidneys. The kidneys are now able to dump glucose. And as such, the glucose will begin to plummet. After a few hours the trajectory of that glucose concentration will change. And now you're looking at the effect of insulin. Insulin will significantly decreases the glucose concentration but it won't do so at the rate that reperfusion will. We know that if we don't do something about the glucose decrease as a insulin infuses, is it will go all the way down to zero. But we maintain a target range about 150 to 250 and in many hands, the appropriate range is really 200 to 300 milligrams per deciliter. And we achieve that range by adding dextrose to our IV fluids. Now one has to understand that the range here is chosen really to make sure that the glucose doesn't go severely low.

DKA diabetic ketoacidosis nursing management pathophysiology & treatment. DKA is a complication of diabetes mellitus and mainly affects type 1 diabetics. DKA management includes controlling hyperglycemia, ketosis, and acdidosis. Signs & Symptoms include polyuria, polydipsia, hyperglycemia greater than 300 mg/dL, Kussmaul breathing, acetone breath, and ketones in the urine. Typically DKA treatment includes: intravenous fluids, insulin therapy (IV regular insulin), and electrolyte replacement. How to Prevent and Control Diabetes Symptoms Diabetes prevention: 5 tips for taking control Changing your lifestyle could be a big step toward diabetes prevention — and it's never too late to start. Consider these tips. When it comes to type 2 diabetes — the most common type of diabetes — prevention is a big deal. It's especially important to make diabetes prevention a priority if you're at increased risk of diabetes, such as if you're overweight or you have a family history of the disease. Diabetes prevention is as basic as eating more healthfully, becoming more physically active and losing a few extra pounds. It's never too late to start. Making a few simple changes in your lifestyle now may help you avoid the serious health complications of diabetes down the road, such as nerve, kidney and heart damage. Consider the latest diabetes prevention tips from the American Diabetes Association. 1. Get more physical activity There are many benefits to regular physical activity. Exercise can help you: Lose weight Lower your blood sugar Boost your sensitivity to insulin — which helps keep your blood sugar within a normal range Research shows that aerobic exercise and resistance training can help control diabetes. The greatest benefit comes from a fitness program that includes both. 2. Get plenty of fiber It's rough, it's tough — and it may help you: Reduce your risk of diabetes by improving your blood sugar control Lower your risk of heart disease Promote weight loss by helping you feel full Foods high in fiber include fruits, vegetables, beans, whole grains and nuts. 3. Go for whole grains It's not clear why, but whole grains may reduce your risk of diabetes and help maintain blood sugar levels. Try to make at least half your grains whole grains. Many foods made from whole grains come ready to eat, including various breads, pasta products and cereals. Look for the word "whole" on the package and among the first few items in the ingredient list. Diabetes is a serious disease. Following your diabetes treatment plan takes round-the-clock commitment. But your efforts are worthwhile. Careful diabetes care can reduce your risk of serious — even life-threatening — complications. Here are 10 ways to take an active role in diabetes care and enjoy a healthier future. 1. Make a commitment to managing your diabetes Members of your diabetes care team — doctor or primary care provider, diabetes nurse educator, and dietitian, for example — can help you learn the basics of diabetes care and offer support along the way. But it's up to you to manage your condition. Learn all you can about diabetes. Make healthy eating and physical activity part of your daily routine. Maintain a healthy weight. Monitor your blood sugar level, and follow your doctor's instructions for managing your blood sugar level. Ask your diabetes treatment team for help when you need it. 2. Don't smoke Smoking increases your risk of various diabetes complications, including: Reduced blood flow in the legs and feet, which can lead to infections, ulcers and possible removal of a body part by surgery (amputation) Heart disease Stroke Eye disease, which can lead to blindness Nerve damage Kidney disease Talk to your doctor about ways to help you stop smoking or using other types of tobacco. 3. Keep your blood pressure and cholesterol under control Like diabetes, high blood pressure can damage your blood vessels. When these conditions team up, they can lead to a heart attack, stroke or other life-threatening conditions. Schedule regular physicals and eye exams Schedule two to three diabetes checkups a year, in addition to your yearly physical and routine eye exams. During the physical, your doctor will ask about your nutrition and activity level and look for any diabetes-related complications — including signs of kidney damage, nerve damage and heart disease — as well as screen for other medical problems. BLOOD GLUCOSE TESTING: Blood glucose testing, also known as blood glucose monitoring, is one of the main tools involved in controlling diabetes. Not everyone with diabetes will test their blood glucose levels but it is regarded as being very beneficial for helping to make diet and medication dosing decisions. If you are on any medication that can lead to hypoglycaemia (most notably insulin), you should test your blood glucose levels. WHAT IS A LANCING DEVICE: Lancing devices are used to obtain samples of blood for glucose testing using a lancet. A variety of lancets are on the market, the most common of which are automatic lancing devices (Used in the video). TESTING BLOOD GLUCOSE METER & TESTS STRIPS: Control solutions are used to check that the meter and test strips are working together properly and that the test is performing correctly. Most control solutions last 90 days after opening, please check yours for the actual date. The acceptable range of glucose for that solution is listed on the back of your test strip vial or control solution. USEFUL TIPS: Please watch the full video for all of them but here are some important ones, • Use a new lancet every time you use your lancing device. I know it's time consuming but if you don't change, it's going to hurt more and you're at risk of a skin infection. • Diabetic patients tend to build a collection of different blood glucose meters over time. It’s really important that you only ever use one meter to build up a trend of results because different meters will give slightly different results. HOW TO TEST BLOOD GLUCOSE: • Prepare your kit for testing. • This should include: your meter, test strip, lancing device, cotton wool, monitoring diary and sharps bin. • Ensure that the lancing device is primed with a new lancet. • Wash and dry your hands - to ensure that the result is not influenced by any sugars that may be present on your fingers. • A fuller drop of blood will be obtained if your fingers are warm, so it’s worth warming your hands by washing with warm water and rubbing them for 10 seconds. • Put a test strip into your meter, make sure it switches on and is ready. • Prick your finger with the lancing device at the sides of the finger as there are less nerve ending here than at the tips or the ‘pads’. Switch fingers regularly to prevent thickening of the skin. You may want to avoid using your little finger due to the skin being thin. • If your hands are warmed up you shouldn't need to squeeze your finger for a blood drop, if necessary apply light pressure to the surrounding area until a blood drop appears. Squeezing too hard can interfere with results. • Wipe away the first blood drop with clean cotton wool and use the second blood drop for testing (careful not to smear the drop). • Gently touch the blood drop with the test strip in the meter, wait a few seconds for result to appear. • If the test is successful, clean any blood off your finger – with the cotton wool if necessary. • Record the result/details in a monitoring diary. • Dispose of the test strip and ensure that the lancet used is put into a sharps bin. It also shows how to use glucometer or glucose meter Glucometer or glucose meter companion kit has battery operated glucometer device, lancing device, lancets, glucometer strips with code chip, cotton swab and dairy , it has the socket in the device to insert code chip, this chip comes with test strips. It has memory button which when you press and hold for few seconds the screen displays previous reading and when you press scroll button it takes you through the saved readings. It has container that has test strips, here you can see the no. of strips, code no. (please note this code should match with the code on the chip that comes with the strips and is inserted in the device) expiry date and batch no., test strip with an arrow mark on the strip which indicates the direction of the end which goes inside the device and the green square is the space for blood drop. National diabetes care home survey highlights 'concerning' results October 4th, 2013 Almost one in five care home residents with diabetes self-medicate without any checks, according to new research. Self-management is usually encouraged, but vulnerable people with diabetes, especially those on the potentially-dangerous drug insulin, should be checked to ensure they have taken their medicine to prevent complications, including descending into a coma. The first-ever National Care Home Diabetes Audit also revealed that more than 60 per cent of the 2,043 premises surveyed did not have a designated member of staff with responsibility for diabetes management. The audit was carried out by the Institute of Diabetes for Older People (IDOP) in partnership with ABCD (Association of British Clinical Diabetologists) and a group of further key collaborators (listed below). The audit results found only one in 10 care home residents were reported as having diabetes although this figure is at odds with previous research that showed as many as 26 per cent of all care home residents actually have diabetes. However, despite this high number, 63.2 per cent of homes did not have a designated member of staff with responsibility for diabetes management. Also, 64.5 per cent of care homes had no policy on screening for diabetes. The audit seeks to identify quality standards that can be picked up by the Care Quality Commission (CQC) to be used for assessment of the quality of diabetes care being delivered in care home settings. Professor Alan Sinclair, audit lead and Director of IDOP, said: "We undertook this research to firstly ascertain what areas of diabetes care within residential settings can be further supported and, secondly, to gain better insight into the difficulties of providing enhanced care in these rather complex settings. "We know care home staff are working hard to care for their residents but it was sad and concerning to discover some of the results. Especially, the fact 17.3 per cent of homes had no system in place to examine whether those who self-medicate for diabetes have taken their medication. We encourage self-medication but it needs to be checked in a care setting, especially". "However, this audit has the potential to improve care for older people with diabetes living in care homes in England, and give insight on how to provide staff with the training and support that they need, as well as assisting managers and policymakers to allocate resources." Peter Winocour, former chairman of ABCD, said: "We are very pleased to support this important work which really must be seen as a call to action. "Regarding the care of this vulnerable group of individuals, we know that when it comes to hospital admissions at least one in five have diabetes and the average age of these patients is 75. A significant proportion of these vulnerable patients will come from nursing homes. So the care that they receive in the nursing homes has to be optimised." The audit also found over a third of residents (35.17 per cent) did not know about the signs and symptoms of hypoglycaemia -- a complication of diabetes which can result in hospital admission due to a coma if left untreated. More than 35 per cent of homes did not have a written policy for managing hypoglycaemia. The survey had the full support and collaboration of multiple stakeholders. This project follows the recently-published Diabetes UK national guidance on diabetes in care homes (2010), which has sought to enhance the standard of care received by residents with diabetes. IDOP, in partnership with the InDependent Diabetes Trust and the English Community Care Association (ECCA), launched a 'Passport' for people with diabetes in care settings earlier this year.

88 Year Old Woman Recovering from Alzheimer’s and Diabetes Using Coconut Oil Coconut Oil Lowers Blood Sugars Through Rubbing on Skin I brought my 88 year old mother out of 7 years of nursing home neglect/abuse on February 14, 2016. I’ve been studying the effects of coconut oil and Alzheimer’s disease. She was taking 4 units of insulin at meals and 10 units of insulin at night. After applying coconut oil all over her feet and body, I noticed her sugars were continuing to lower without insulin shots. The first 2 days, I saturated her skin and added 1 teaspoon in protein shakes. Her sugars went from 300’s to 140. Mother’s Health Makes Dramatic Improvement in 5 days I am using it on her body after bathing in the morning and before bedtime only. I have not given her any oil in her drink other than first two days. Her sugar reading are running 118 – 140 without any shots now for 5 days. I gave her an ice cream with 22 grams of sugar, this morning her cranberry juice has 36 grams sugar per 8oz. She ate scrambled eggs, sausage, 1 waffle with syrup and I checked her sugars again at noon – now after all that sugar and no insulin for 4 days, her reading was 121. Diabetes refers to a group of conditions characterized by a high level of blood glucose, commonly referred to as blood sugar. Too much sugar in the blood can cause serious, sometimes life-threatening health problems. There are two types of chronic diabetic conditions: type 1 diabetes and type 2 diabetes. Pregnant women may acquire a transient form of the disease called “gestational diabetes” which usually resolves after the birth of baby. Pre-diabetes is when the blood sugar level is at the borderline: higher than normal, but lower than in diabetics. Prediabetes may or may not progress to diabetes. During food digestion, carbohydrates - or carb - break down into glucose which is carried by the bloodstream to various organs of the body. Here, it is either consumed as an energy source - in muscles for example - or is stored for later use in the liver. Insulin is a hormone produced by beta cells of the pancreas and is necessary for glucose intake by target cells. In other words, when insulin is deficient, muscle or liver cells are unable to use or store glucose, and as a result, glucose accumulates in the blood. In healthy people, beta cells of the pancreas produce insulin; insulin binds to its receptor on target cells and induces glucose intake. In type 1 diabetes, beta cells of the pancreas are destroyed by the immune system by mistake. The reason why this happens is unclear, but genetic factors are believed to play a major role. Insulin production is reduced; less insulin binds to its receptor on target cells; less glucose is taken into the cells, more glucose stays in the blood. Type 1 is characterized by early onset, symptoms commonly start suddenly and before the age of 20. Type 1 diabetes is normally managed with insulin injection. Type 1 diabetics are therefore “insulin dependent”. In type 2 diabetes, the pancreas produces enough insulin but something goes wrong either with receptor binding or insulin signaling inside the target cells. The cells are not responsive to insulin and therefore cannot import glucose; glucose stays in the blood. In other words, type 2 diabetics are “insulin resistant”. Here again, genetic factors predispose susceptibility to the disease, but it is believed that lifestyle plays a very important role in type 2. Typically, obesity, inactive lifestyle, and unhealthy diet are associated with higher risk of type 2 diabetes. Type 2 is characterized by adult onset; symptoms usually appear gradually and start after the age of 30. Type 2 diabetes accounts for about 80 to 90% of all diabetics. Management focuses on weight loss and includes a low-carb diet.

Remember that having diabetes complications will only worsen your problem. So, it should be in your best interest to be mindful of these three tips and prolong your life accordingly.

Blood transfusion procedure nursing lecture: transfusion reaction (hemolytic, febrile, allergic, GvHD) and complications for NCLEX. What is a blood transfusion? It’s where the nurse transfuses a patient with new red blood cells via a venous access. It is most commonly donated red blood cells. WHY do patients need blood transfusions? Because the patient is low on RBCs (erythrocytes). This may be due to anemia (body not producing enough substances to maintain or produce red blood cells like in renal failure, cancers etc.) or the patient has experienced excessive blood loss (surgery or trauma) What's the importance of RBCs? Our body can't function without them very well! A patient will start to look pale, feel weak, experience shortness of breath with activity, and tachycardia (heart has to work harder) etc. Red blood cells, with the help of the protein hemoglobin, carries oxygen it receives from the lungs throughout our body and helps remove the CO2 built up in our body to the lungs to be exhaled. The body's organs need fresh oxygen to function. When is a patient usually transfused? It depends on what is going on with the patient, but recent guidelines by the American Association of Blood Banks recommends transfusing blood when a hemoglobin levels fall 7-8 g/dL. What are normal hemoglobin level? It varies depending on gender: Men: 14 to 18 g/dL Women: 12 to 16 g/dL Nurse’s Responsibilities: as a nurse working in a hospital transfusing blood is VERY common! You need to know this for not only NCLEX but the job as well. Most hospitals only allow registered nurses to transfuse blood. The nurse will prepare the patient for the transfusion (draw labs, obtain informed consent etc.), transfuse and monitor the patient during the transfusion for a transfusion reaction. Nursing standard precautions and isolation precautions (nursing) review for the NCLEX exam. This review will cover standard precautions for infection control and transmission-based precautions (also called isolation precautions) such as contact, droplet, and airborne precautions. In addition, I will give you 3 isolation precaution mnemonics on how to remember droplet, contact, and airborne precautions. Standard precautions are precautions we follow with every patient at all times because we do not know if the person has an infectious disease or not. This is to protect ourselves and other patients. Standard precautions include: hand hygiene, wearing appropriate PPE (personal protective equipment), injection safety, respiratory/cough etiquette, patient room placement, handling of patient laundry and equipment. Isolation precautions are precautions we follow when we know or suspect a patient has an infectious disease. Depending on the type of infectious disease the patient has we will place the patient in the appropriate precautions. For example, if a patient has Pertussis (whooping cough) we will place the patient in droplet precautions. As the nurse, we would continue to follow standard precautions along with always wearing a surgical mask during patient contact. For a patient in airborne precautions, we would wear an N95 mask along with following standard precautions and place the patient in a special AIIR room (which is a negative pressure room). For a patient in contact precautions, we would always wear gloves and a gown during patient contact and follow standard precautions.
