To pill or not to pill? That is the question. Hormonal Birth Control Pills Hormonal or chemical birth control pills will be explained in this video, with the pros and cons of each type. But before I demonstrate the different types, I would like to explain to you how hormonal birth control pills work. When a lady is pregnant, ovulation (or egg release) does not occur. That’s how our body’s system normally works. The hormones in a woman’s body are balanced in a certain way that makes the hormones that stimulate the release of the eggs – called the ovulation hormones – to become inactive during pregnancy. So we thought: If we give the woman pregnancy hormones in pills, we will be able to inhibit ovulation, in other words: to stop it from happening, which, in turn, would prevent the pregnancy to occur. So the scientists decided to put the pregnancy hormone and the female hormone in a pill. This gives us an idea about the first type of birth control pills. This type consists of synthet-ic forms of hormones – the pregnancy hormone progesterone (called progestin) and the female hormone oestrogen. On the other hand there’s another type of birth control pills that consists of the pregnancy hormone only, and in a very small amount. It is called the mini Pill or the Progesterone-Only Pill (P.O.P.). The pills I will talk about are not the only hormonal birth control methods. We can take other forms of chemical or birth control at the doctor’s office, which would have the same mechanism of action. So, back to the pills. Let me explain them now. Let’s start with a combined pill. “Combined” because it has a combination of hormones. Let’s know more about it. The combined pill consists of progestin – a synthetic progester-one (the pregnancy hormone) – and oestrogen (the female hormone). You will need to take a pill daily for 21 days, starting from the 7th day of your menstruation. Even if you’re still bleeding, you must start using the pill. Take a pill daily for 21 days and stop the pill for 7 days, taking into consideration that your period cycle is 28 days. I'm on birth control pills, but have missed taking them once or twice. Can I get pregnant?

You mentioned that you're taking oral contraceptives, and over the course of the last month, you've missed a pill, or possibly even more, and you're wondering if pregnancy is a possibility. And overall, oral contraceptives are a good form of contraception. Pregnancy rates are about 0.1%, but that's if it's taken perfectly - no days are missed, and it's taken at the same time every day. Oral contraceptives work by giving your body a hormone that suppresses ovulation, or in other words, prevents your body form releasing an egg that can be fertilized by sperm. So if you miss a pill at just the right time (or I guess you could say at just the wrong time) of the month, then that means that you might ovulate, and pregnancy is a possibility. Failure rates for oral contraceptives are high when women are taking them irregularly, or not taking them at the same time each day. So my recommendation to you is to wait until your next period should start, and if it never does, then take a home pregnancy test. And if that first test result is negative and you still haven't started your period for another week, then take another test. If that one is negative, and it's been about 5 to 6 weeks since your last period, then the chances of pregnancy are very very low. And in the future, if you're wanting to prevent pregnancy, be sure to take your pill everyday around the same time, and if you do that, then it should prevent pregnancy. They contain hormones that suppress ovulation. The most effective type is the combination pill which contains both estrogen and progestin - a synthetic form of progesterone. These 2 hormones interfere with the normal menstrual cycle to prevent ovulation. The menstrual cycle refers to the monthly events that occur within a woman’s body in preparation for the possibility of pregnancy. Each month, an egg is released from an ovary in a process called ovulation. At the same time, the lining of the uterus thickens, ready for pregnancy. If fertilization does not take place, the lining of the uterus is shed in menstrual bleeding and the cycle starts over. The menstrual cycle is under control of multiple hormones secreted by the hypothalamus, pituitary gland, and ovaries. Basically, the hypothalamus produces gonadotropin-releasing hormone, GnRH; the pituitary secretes follicle-stimulating hormone, FSH, and luteinizing hormone, LH; while the ovaries produce estrogen and progesterone. These hormones are involved in a REGULATORY network that results in monthly cyclic changes responsible for ovulation and preparation for pregnancy. The 2 hormones that are required for ovulation are: FSH, which starts the cycle by stimulating immature follicles to grow and produce a mature egg; and LH, which is responsible for the release of the egg from the ovary - the ovulation event itself. Two other hormones, estrogen and progesterone, are at high levels after ovulation, in the second half of the cycle. They suppress FSH and LH during this time, preventing the ovaries from releasing more eggs. If fertilization occurs, estrogen and progesterone levels REMAIN HIGH throughout pregnancy, providing a continuous suppression of ovulation. On the other hand, in the absence of pregnancy, their levels FALL, causing menstrual bleeding. The levels of estrogen and progesterone in the combination pills mimic the hormonal state after ovulation, tricking the ovaries into thinking that ovulation has already occurred; FSH and LH are constantly suppressed, no egg is matured or released. The pills are taken every day for three weeks, followed by one week of placebo pills containing no hormones. During the week of placebos, estrogen and progesterone levels fall, triggering a so-called withdrawal bleeding, or fake periods. The bleeding serves as a convenient indication that fertilization did not happen, but it is not required for birth control. In fact, there exist continuous-use contraceptive pills with less or no placebos, resulting in less or no menstrual periods. These pills are particularly beneficial for women who suffer from menstrual disorders such as excessive menstrual bleeding, painful menstruation and endometriosis. For lactating women, or those who cannot tolerate estrogen, there are mini-pills that contain only progestin. These are not as effective as combination pills at preventing ovulation. Their effect relies more on the ability of progestin to promote secretion of a thick cervical mucus to obstruct sperm entry.

You have some questions about oral contraceptives, and exactly how they work, and how things work with the sugar pills that are at the end of a 28-day pack. Oral contraceptives are very effective at preventing pregnancy when taken as prescribed. When taken as prescribed, the chance of getting pregnant is less than 1%. However, a lot of women miss pills or don't start pills when they should, and so they end up getting pregnant 8% of the time. The pills that you take in your pack contain hormone that help to suppress ovulation, and they're given at specific times in your cycle to prevent ovulation. So it's important to start on the first day that you're supposed to start, and then at the end of a 28-day pack, the last 7 pills are sugar pills, but a lot of people like to have those available, because it just makes it so you're used to taking a pill every day and you don't forget to start on day 1 of your next cycle. Basically, you're able to get pregnant (or you're fertile) for 6 days out of each month - 5 days leading up to ovulation and on the day of ovulation. And ovulation typically happens about mid-cycle, and so of a 28-day cycle, you'd probably ovulate on day 14 and be able to get pregnant for 5 days leading up to that day, because sperm can actually live inside a woman's reproductive tract for 3 to 5 days. So you could potentially have unprotected intercourse on day 9 and actually get pregnant if you ovulate on day 14. It's possible, but the chance is low - about 5%. The chance of getting pregnant increases when you have intercourse closer to ovulation. The chance is actually greatest on the day before ovulation at about 30% to 35%. But of course, this all only applies if you're not taking contraceptive pills, using any other forms of contraception, or if you're not taking it as prescribed. Then you might ovulate, and pregnancy is possible. To specifically answer your question, you're not going to get pregnant when you're on your period, because you're not ovulating and that will never fall during your fertile window. You mentioned that you've been trying forever to have a baby, and first is foremost I want you to know that my heart goes out to you for that. I'm sure that this has been a very difficult time for you. It sounds like you went to the doctor and the doctor prescribed birth control pills. Those are usually given to women who aren't well regulated, who aren't having regular cycles because it gets their body back on to having regular cycles and then after being on it for a few months they can stop taking the pill and then at that point their body usually has 28 to 30 days cycles with predictable ovulatory cycles and this increases your chances of getting pregnant and also helps you to know if you are pregnant once you miss a period. Women who have irregular periods probably aren't ovulating and have no idea if they're pregnant because they never know when that time comes when you miss a period. That's when you should take a pregnancy test if you're suspicious about pregnancy. You asked specifically, can you get pregnant when you stop taking birth control pills? The answer is yes. Like I mentioned, sometimes they're prescribed to help women get more regular so that they can increase their chances of getting pregnant once they've gone off them. Birth control pills do suppress ovulation but a lot of women get pregnant while taking them because they missed a couple of pills just at the right time of the month. As soon as you stop taking them that hormone is no longer in your body and you can ovulate and you can get pregnant. I hope the very best for you.
