It can be used safely in non-diabetics, and pre-diabetics. It can cause GI side effects, so when I start someone on it, I usually give them 1000mg pills, and the goal is to take 1000mg twice a day. Start with 1/2 pill once a day at dinner, in one week, Take 1/2 pill twice a day with food, in another week, take 1/2 pill in the morning and one at dinner, Finally advance to 1 pill twice a day. Some people cannot tolerate the full dose, and have to take somewhat less of a dose. Switching to the ER version can be helpful. If you eat lots of carbs, you will have more side effects. Limiting carbs, helps to tolerate it better AND can help you to lose more weight.

Attorney Jim Higgins with The Higgins Firms recently stopped by to explain what's going on. Jim, welcome back to the show. Jim Higgins: Thank you. Kacy Hagerty: All right. Let's talk a little bit about these allegations in these lawsuits. What happened here? Jim Higgins: This is a drug, product drug case, Januvia. It's a type 2 diabetes drug that’s used to use to help control blood sugar levels and it's been prescribed for – for a while. And, you know, type 2 diabetes is a big problem in the world. I think there's 350 million people that suffer from it. And there has been a link to this drug based upon – first came in a German study that it may cause pancreatic cancer. And so, there's been several lawsuits that had been – has been filed. There's been a couple here recently in Tennessee. It's probably maybe getting close to a hundred, last I checked, that – that have been filed alleging that this drug causes or may cause pancreatic cancer and that is just a risk that – that should not happen for people. Kacy Hagerty: Exactly. So how many lawsuits have actually gone to trial? You said about a hundred where out there but how many have gone to trial? Jim Higgins: Right. None yet. You know, and that’s what happens when we talk about these cases. What – the first thing that starts happening in any drug recall case that we see is we start seeing studies and there's always a conflict. And then people who believe that they were harmed by the drug, they’ll file the lawsuit. They get all bunched together. And then what we call a discovery process starts. And that's where these are. What happened? What do your studies show? Did you give a warning that this may happen? Why didn't you give a warning? You know, there's going to be depositions and testimony and documents reviewed. So, it's really kind of at the beginning of where this litigation is headed. Kacy Hagerty: It is quite a process. Now, if anyone out there actually has a cancer diagnosis and they're taking this drug what should they do? Jim Higgins: Well, I think, obviously, you know, like we say in any of our cases, you got to speak with your doctors. Sometimes, you know, and the cancer here that we've been looking at the cases is very specific. It's pancreatic cancer. So when you – you're going to speak with you doctor about it and, you know, you can speak to them on the front end if when they hear about this if they're taking Januvia or another drug and say, hey, do you feel this is OK with me? Are these – you know, talk to your doctor about this risk and you may be satisfied that the drug is safe for you. But if you do have that diagnosis, then you want make sure you maintain anything you've got. Records, medical prescriptions, everything and you want to – you should contact our office. Because pancreatic cancer is hard to detect in the early stages, most cases are not diagnosed until the disease has started to spread. A low dose combination of two antidiabetes medications may help stave off the development of type 2 diabetes among individuals with impaired glucose tolerance (IGT), according to research published online ahead of print in The Lancet. Here is some information about IGT: • It is a condition in which blood sugar (or glucose) levels are above normal but below those seen among those with diabetes • It is considered to be a pre-diabetes state, with many who suffer from the condition going on to develop type 2 diabetes • It is associated with a combination of impaired sensitivity to insulin and reduced release of insulin by the body Researchers from the University of Toronto randomly assigned over 200 individuals with IGT to receive a daily low dose of the antidiabetes drugs rosiglitazone, commonly known as Avandia, and metformin, commonly known as Glucophage, or a placebo. Treatment continued for an average of about four years, and the participants were regularly assessed for the presence of diabetes. Only 14% of those who took the medications developed diabetes, compared with 39% of those taking a placebo. This means that only four individuals with IGT would need to take the drugs for one case of type 2 diabetes to be prevented. In addition, 80% of those taking the medications actually developed a normal response to glucose, compared with only 53% of those on placebo. And while impaired sensitivity to insulin remained stable among those receiving treatment, it worsened among those taking a placebo. We spoke with Dr. Bernard Zinman, one of the investigator's of this study, who offered some further insight. Today's research suggests that low dose treatment with a combination of antidiabetes drugs that each have different mechanisms of action may help prevent diabetes in at-risk individuals.

Actos and Bladder Cancer - New Litigation Information
This case is being transferred to the newly-created MDL No. 2299 styled In Re: Actos Products Liability Litigation. Attorney Cal Warriner appeared at the hearing before the Judicial Panel on Multi-District Litigation in late November in Savannah, at which time the panel of federal judges heard argument from plaintiffs as well as Takeda Pharmaceuticals regarding the need for establishment of an MDL as well as the locale. Both sides supported establishment of an MDL to coordinate the litigation, which is expected to ultimately include hundreds if not thousands of cases filed on behalf of diabetics who developed bladder tumors after using the diabetes drugs: Actos Actoplus met Actoplus met XR Duetact insulin sensitizing drugs The litigation may also include claims filed by plaintiffs who contend that Actos and related drugs increased their risks of heart attacks, strokes, and other cardiac ailments.
