LIFE Before Death Cancer Pain

Over 12 million people are likely to be diagnosed with cancer this year alone and 7 million will die of the disease. Cancer kills more people than AIDS and tuberculosis combined. "There are at least 5 million people who die of cancer each year with untreated pain," states Dr Kathleen Foley (USA). "We think this is an underestimate of pain in the world because there are 12 million survivors of cancer, people living with cancer, and we know about a third of them experience pain." Dr Eva Rossina Duarte Juarez (Guatemala) agrees that cancer is indeed painful. "In the situation of advanced illnesses, people need to be able to die in peace." Breast cancer patient Carmen (Australia) reflects that she doesn't fear death however she did fear the essential medications designed to treat her cancer pain. "I had a lot of pain on every level, whether it was physical or emotional." Dr Mark Boughey (Australia) of Caritas Christi Hospice in Melbourne explains, "When Carmen got here pain was the big issue... She had that really problematic neuropathic pain so you couldn't even touch her skin without her screaming and being distressed." "This problem is very much with us, it's a global problem, but also there are things we can do to bring the escalating problem of cancer under control," Professor David Hill. Carmen reflects that professional treatment of her pain brought back her sanity and quality of life. "I'm very blessed to have had this opportunity ... I've had really good care." Early Signs that Cancer is Growing in Your Body
Don’t rely on routine tests alone to protect you from cancer. It’s just as important to listen to your body and notice anything that’s different, odd, or unexplainable. Here are some signs that are commonly overlooked: 1. Wheezing or shortness of breath One of the first signs many lung cancer patients remember noticing is the inability to catch their breath. 2. Chronic cough or chest pain Several types of cancer, including leukemia and lung tumors, can cause symptoms that mimic a bad cough or bronchitis. Some lung cancer patients report chest pain that extends up into the shoulder or down the arm. 3. Frequent fevers or infections These can be signs of leukemia, a cancer of the blood cells that starts in the bone marrow. Leukemia causes the marrow to produce abnormal white blood cells, sapping your body’s infection-fighting capabilities. 4. Difficulty swallowing Trouble swallowing is most commonly associated with esophageal or throat cancer, and is sometimes one of the first signs of lung cancer, too. 5. Swollen lymph nodes or lumps on the neck, underarm, or groin Enlarged lymph nodes indicate changes in the lymphatic system, which can be a sign of cancer. 6. Excessive bruising or bleeding that doesn’t stop This symptom usually suggests something abnormal happening with the platelets and red blood cells, which can be a sign of leukemia. Over time, leukemia cells crowd out red blood cells and platelets, impairing your blood’s ability to carry oxygen and clot. 7. Weakness and fatigue Generalized fatigue and weakness is a symptom of so many different kinds of cancer that you’ll need to look at it in combination with other symptoms. But any time you feel exhausted without explanation and it doesn’t respond to getting more sleep, talk to your doctor. 8. Bloating or abdominal weight gain Women diagnosed with ovarian cancer overwhelmingly report unexplained abdominal bloating that came on fairly suddenly and continued on and off over a long period of time. 9. Feeling full and unable to eat This is another tip-off to ovarian cancer; women say they have no appetite and can’t eat, even when they haven’t eaten for some time. 10. Pelvic or abdominal pain Pain and cramping in the pelvis and abdomen can go hand in hand with the bloating that often signals ovarian cancer. Leukemia can also cause abdominal pain resulting from an enlarged spleen. 11. Rectal bleeding or blood in stool This is a common result of diagnosing colorectal cancer. Blood in the toilet alone is reason to call your doctor and schedule a colonoscopy. 12. Unexplained weight loss Weight loss is an early sign of colon and other digestive cancers; it’s also a sign of cancer that’s spread to the liver, affecting your appetite and the ability of your body to rid itself of wastes. 13. Upset stomach or stomachache Stomach cramps or frequent upset stomachs may indicate colorectal cancer. 14. A red, sore, or swollen breast These symptoms can indicate inflammatory breast cancer. Call your doctor about any unexplained changes to your breasts. 15. Nipple changes One of the most common changes women remember noticing before being diagnosed with breast cancer is a nipple that began to appear flattened, inverted, or turned sideways. 16. Unusually heavy or painful periods or bleeding between periods Many women report this as the tip-off to endometrial or uterine cancer. Ask for a transvaginal ultrasound if you suspect something more than routine heavy periods. 17. Swelling of facial features Some patients with lung cancer report noticing puffiness, swelling, or redness in the face. Small cell lung tumors commonly block blood vessels in the chest, preventing blood from flowing freely from your head and face. 18. A sore or skin lump that doesn’t heal, becomes crusty, or bleeds easily Familiarize yourself with the different types of skin cancer —melanoma, basal cell carcinoma, and squamous cell carcinoma — and be vigilant about checking skin all over your body for odd-looking growths or spots.

The CDMRP Grant Review Process:  Maximizing Success
On August 4-5, the DoD Prostate Cancer Research Program (PCRP) hosted IMPaCT 2016, a meeting intended to provide new investigators and physician-scientists with an opportunity to discuss their research with their peers, leading prostate cancer experts, and survivors, and to foster new collaborations that will facilitate research aimed at addressing critical issues in prostate cancer. One of the sessions also focused on the grant review process for Congressionally Directed Medical Research Programs (CDMRP). Post surgery (Bye Bye Tata's part 2)
October 5th 2010. Right after my Bilat-Mastectomy with tissue expanders. I had just finished ACT Chemo (A-Adriamycin, C-Cytoxan, T-Taxol) a month before so my hair and eyebrows and lashes were gone. I was diagnosed with Stage 3 Breast cancer in May of that year at the age of 36. I then did 28 rounds of Rads (was supposed to be more) then had my overies, fallopian tubes and gallblader out. Then my expanders out and implants in. Now (Oct 15, 2011) I am trying to find an Aromatase inhibitor that will not have so many SE's. My cancer is Estrogen fed, so we need to stop my estrogen to keep the cancer from growing back, thus the Bilateral-Oophorectomy So far tamoxifen (I know its not a "AI") and Arimidex both were impossible for me to stay on. And next I try Aromasin. If that doesnt work then we will try Femara. I have to stay on it for at least 5-10 yrs. So I have to be able to tolerate the SE's, especially since I have 3 young children. Fun stuff!

Ascites Treatment: Home remedies can be extremely helpful in treating the causes and symptoms of liver/abdominal Ascites. Remedy 1: Soak a handful of fenugreek seeds overnight. Next morning, mix the seeds and water gently and strain the liquid with a muslin cloth or strainer. Drink the liquid to keep yourself healthy. Remedy 2: Dilute 30 ml of bitter gourd root juice with water and consume the liquid at least thrice a day for rapid relief. Remedy 3: Take a handful of chick peas. Soak them overnight in water. Strain the liquid early in the morning and drink it to get relief from ascites. Remedy 4: Consumption of fresh garlic juice daily for at least two to three weeks. Spotting is bleeding between periods. It can be anything from from a light drop of blood, to a heavier flow like your period. Blood – tinged discharge is also considered spotting, but if the discharge is dark yellow-ish or smells gross, there could be a problem – like an infection. Spotting is usually normal, and sometimes occurs during ovulation because estrogen levels drop when the ovary releases an egg. It could also signal the start of your period. It’s hard to tell if there’s any specific meaning to your spotting – when you get it, and how you get it is specific to your own body and likely isn’t reason for concern. Causes The hormones estrogen and progesterone control your cycle. If they get out of balance, your cycle could become irregular and cause spotting. Thyroid problems, ovary problems, or inconsistency in birth control pills can contribute to hormone imbalance. Sometimes stress can cause your cycle to be irregular. This can result in spotting during your period. Spotting can also be caused by irregularity in the lining of the uterus because the lining becomes fragile. Spotting may also be a sign of pregnancy, or a sign of labor. Spotting also can happen during perimenopause as your body transitions into not having periods. The color of blood spotting can tell you how fresh the blood is. Bright red is fresh, pink is diluted fresh blood, brown is days old and black is very old. Spotting may not be normal if you have heavily bleeding between periods for more than a couple days, light spotting for 3 cycles, heavy bleeding after sex or anything that isn’t normal to your typical pattern. There are some things you can do to prevent spotting. Taking your birth control as prescribed. Reducing asprin intake. Asprin is a blood thinner and can worsen spotting. Maintaining a healthy weight. J. Michael Bishop (UCSF) Part 3: The cancer genome and therapeutics
https://www.ibiology.org/human-disease/oncogenes-genetic-paradigm-cancer/#part-3 Bishop begins his lecture with a historical review of the experiments that resulted in the realization that cancer has a genetic basis. He explains that mutations can cause normal cellular genes known as proto-oncogenes to become oncogenes, analogous to jammed accelerators causing uncontrolled cell division. Alternatively, mutations in tumor suppressor genes are analogous to a failed brake system. An accumulation of both types of mutation leads to the development of cancer. In part 2 of his lecture, Bishop describes how the sequencing of cancer genomes will advance the study and management of cancer on various fronts, including the discovery of causes, the improvement of early detection, the prediction of outcome and the development of new therapeutics. He stresses the promise of designing therapeutics that can be targeted to those patients most likely to benefit from these drugs. Bishop expands on this theme in part 3 where he describes experiments to find drugs that exploit vulnerabilities created by cancer genes, utilizing a genetic strategy known as "synthetic lethality."

Suspense: The Twist / The Visitor / The Blue Hour
The program's heyday was in the early 1950s, when radio actor, producer and director Elliott Lewis took over (still during the Wilcox/Autolite run). Here the material reached new levels of sophistication. The writing was taut, and the casting, which had always been a strong point of the series (featuring such film stars as Orson Welles, Joseph Cotten, Henry Fonda, Humphrey Bogart, Judy Garland, Ronald Colman, Marlene Dietrich, Eve McVeagh, Lena Horne, and Cary Grant), took an unexpected turn when Lewis expanded the repertory to include many of radio's famous drama and comedy stars — often playing against type — such as Jack Benny. Jim and Marian Jordan of Fibber McGee and Molly were heard in the episode, "Backseat Driver," which originally aired February 3, 1949. The highest production values enhanced Suspense, and many of the shows retain their power to grip and entertain. At the time he took over Suspense, Lewis was familiar to radio fans for playing Frankie Remley, the wastrel guitar-playing sidekick to Phil Harris in The Phil Harris-Alice Faye Show. On the May 10, 1951 Suspense, Lewis reversed the roles with "Death on My Hands": A bandleader (Harris) is horrified when an autograph-seeking fan accidentally shoots herself and dies in his hotel room, and a vocalist (Faye) tries to help him as the townfolk call for vigilante justice against him. With the rise of television and the departures of Lewis and Autolite, subsequent producers (Antony Ellis, William N. Robson and others) struggled to maintain the series despite shrinking budgets, the availability of fewer name actors, and listenership decline. To save money, the program frequently used scripts first broadcast by another noteworthy CBS anthology, Escape. In addition to these tales of exotic adventure, Suspense expanded its repertoire to include more science fiction and supernatural content. By the end of its run, the series was remaking scripts from the long-canceled program The Mysterious Traveler. A time travel tale like Robert Arthur's "The Man Who Went Back to Save Lincoln" or a thriller about a death ray-wielding mad scientist would alternate with more run-of-the-mill crime dramas. http://en.wikipedia.org/wiki/Suspense_%28radio_drama%29

My advice is this: Settle! That's right. Don't worry about passion or intense connection. Don't nix a guy based on his annoying habit of yelling "Bravo!" in movie theaters. Overlook his halitosis or abysmal sense of aesthetics. Because if you want to have the infrastructure in place to have a family, settling is the way to go. Based on my observations, in fact, settling will probably make you happier in the long run, since many of those who marry with great expectations become more disillusioned with each passing year. (It's hard to maintain that level of zing when the conversation morphs into discussions about who's changing the diapers or balancing the checkbook.) Obviously, I wasn't always an advocate of settling. In fact, it took not settling to make me realize that settling is the better option, and even though settling is a rampant phenomenon, talking about it in a positive light makes people profoundly uncomfortable. Whenever I make the case for settling, people look at me with creased brows of disapproval or frowns of disappointment, the way a child might look at an older sibling who just informed her that Jerry's Kids aren't going to walk, even if you send them money. It's not only politically incorrect to get behind settling, it's downright un-American. Our culture tells us to keep our eyes on the prize (while our mothers, who know better, tell us not to be so picky), and the theme of holding out for true love (whatever that is—look at the divorce rate) permeates our collective mentality. Even situation comedies, starting in the 1970s with The Mary Tyler Moore Show and going all the way to Friends, feature endearing single women in the dating trenches, and there's supposed to be something romantic and even heroic about their search for true love. Of course, the crucial difference is that, whereas the earlier series begins after Mary has been jilted by her fiancé, the more modern-day Friends opens as Rachel Green leaves her nice-guy orthodontist fiancé at the altar simply because she isn't feeling it. But either way, in episode after episode, as both women continue to be unlucky in love, settling starts to look pretty darn appealing. Mary is supposed to be contentedly independent and fulfilled by her newsroom family, but in fact her life seems lonely. Are we to assume that at the end of the series, Mary, by then in her late 30s, found her soul mate after the lights in the newsroom went out and her work family was disbanded? If her experience was anything like mine or that of my single friends, it's unlikely. And while Rachel and her supposed soul mate, Ross, finally get together (for the umpteenth time) in the finale of Friends, do we feel confident that she'll be happier with Ross than she would have been had she settled down with Barry, the orthodontist, 10 years earlier? She and Ross have passion but have never had long-term stability, and the fireworks she experiences with him but not with Barry might actually turn out to be a liability, given how many times their relationship has already gone up in flames. It's equally questionable whether Sex and the City's Carrie Bradshaw, who cheated on her kindhearted and generous boyfriend, Aidan, only to end up with the more exciting but self-absorbed Mr. Big, will be better off in the framework of marriage and family. (Some time after the breakup, when Carrie ran into Aidan on the street, he was carrying his infant in a Baby Björn. Can anyone imagine Mr. Big walking around with a Björn?)

Participant Engagement and Health Equity Workshop - July 1-2, 2015 - Day 1
On July 1-2, the Precision Medicine Initiative (PMI) Working Group of the Advisory Committee to the NIH Director (ACD) held a public workshop on participant engagement and health equity as they relate to the proposed PMI national research cohort. The workshop focused on the design of an inclusive cohort, building and sustaining public trust, direct-from-participant data provision, and effective and active participant engagement characteristics of a national research cohort of one million or more volunteers. The workshop built on the big science questions developed during the April 28–29 workshop at the NIH, digital health data perspectives shared during the May 28-29 workshop, and information on the strategies to address community engagement and health disparities in a large national research cohort gathered from stakeholders through a request for information. The workshop took place on the National Institutes of Health campus in Bethesda, Maryland, and was videocast. Suspense: Dead Ernest / Last Letter of Doctor Bronson / The Great Horrell
On the second presentation of July 22, 1940, Forecast offered a mystery/horror show titled Suspense. With the co-operation of his producer, Walter Wanger, Alfred Hitchcock received the honor of directing his first radio show for the American public. The condition agreed upon for Hitchcock's appearance was that CBS make a pitch to the listening audience about his and Wanger's latest film, Foreign Correspondent. To add flavor to the deal, Wanger threw in Edmund Gwenn and Herbert Marshall as part of the package. All three men (including Hitch) would be seen in the upcoming film, which was due for a theatrical release the next month. Both Marshall and Hitchcock decided on the same story to bring to the airwaves, which happened to be a favorite of both of them: Marie Belloc Lowndes' "The Lodger." Alfred Hitchcock had filmed this story for Gainsborough in 1926, and since then it had remained as one of his favorites. Herbert Marshall portrayed the mysterious lodger, and co-starring with him were Edmund Gwenn and character actress Lurene Tuttle as the rooming-house keepers who start to suspect that their new boarder might be the notorious Jack-the-Ripper. [Gwenn was actually repeating the role taken in the 1926 film by his brother, Arthur Chesney. And Tuttle would work again with Hitchcock nearly 20 years later, playing Mrs. Al Chambers, the sheriff's wife, in Psycho.] Character actor Joseph Kearns also had a small part in the drama, and Wilbur Hatch, head musician for CBS Radio at the time, composed and conducted the music specially for the program. Adapting the script to radio was not a great technical challenge for Hitchcock, and he cleverly decided to hold back the ending of the story from the listening audience in order to keep them in suspense themselves. This way, if the audience's curiosity got the better of them, they would write in to the network to find out whether the mysterious lodger was in fact Jack the Ripper. For the next few weeks, hundreds of letters came in from faithful listeners asking how the story ended.
