Whenever there is a fatty liver disease, still now we used to neglect thinking that it is just an enlargement of the liver but fatty liver disease is the common reason for liver cirrhosis as well as liver cancer. That is why when we see fatty liver disease in the ultrasound, we should check for any sugar problem, any cholesterol problem or infections such as Hepatitis B or Hepatitis C. If fatty liver disease is there, their new method to find it out and that is called as fibroscan. We look at the liver by fibroscan and usually if there is any liver disease, then we usually treat them with vitamins, if they have sugar, we treat them with diabetes medicine. If they have cholesterol, we target them with cholesterol medicines. Most important message for fatty liver disease patients are that they have to reduce their weight and do exercise and they should cut down on their fats. This is the way to reduce fatty liver disease.

Some people who have type 2 diabetes can achieve their target blood sugar levels with diet and exercise alone, but many also need diabetes medications or insulin therapy. The decision about which medications are best depends on many factors, including your blood sugar level and any other health problems you have. Your doctor might even combine drugs from different classes to help you control your blood sugar in several different ways. 1. Metformin Generally, metformin is the first medication prescribed for type 2 diabetes. It works by improving the sensitivity of your body tissues to insulin so that your body uses insulin more effectively. Metformin also lowers glucose production in the liver. Metformin may not lower blood sugar enough on its own. Your doctor will also recommend lifestyle changes, such as losing weight and becoming more active. 2. Meglitinides These medications work like sulfonylureas by stimulating the pancreas to secrete more insulin, but they're faster acting, and the duration of their effect in the body is shorter. They also have a risk of causing low blood sugar, but this risk is lower than with sulfonylureas. Weight gain is a possibility with this class of medications as well. Examples include repaglinide (Prandin) and nateglinide (Starlix). 3. Thiazolidinediones Like metformin, these medications make the body's tissues more sensitive to insulin. This class of medications has been linked to weight gain and other more-serious side effects, such as an increased risk of heart failure and fractures. Because of these risks, these medications generally aren't a first-choice treatment. Rosiglitazone (Avandia) and pioglitazone (Actos) are examples of thiazolidinediones. 4. Sulfonylureas These medications help your body secrete more insulin. Examples of medications in this class include glyburide (DiaBeta, Glynase), glipizide (Glucotrol) and glimepiride (Amaryl). Possible side effects include low blood sugar and weight gain. 5. GLP-1 receptor agonists These medications slow digestion and help lower blood sugar levels, though not as much as sulfonylureas. Their use is often associated with some weight loss. This class of medications isn't recommended for use by itself. Exenatide (Byetta) and liraglutide (Victoza) are examples of GLP-1 receptor agonists. Possible side effects include nausea and an increased risk of pancreatitis. 6. SGLT2 inhibitors These are the newest diabetes drugs on the market. They work by preventing the kidneys from reabsorbing sugar into the blood. Instead, the sugar is excreted in the urine. Examples include canagliflozin (Invokana) and dapagliflozin (Farxiga). Side effects may include yeast infections and urinary tract infections, increased urination and hypotension. 7. DPP-4 inhibitors These medications help reduce blood sugar levels, but tend to have a modest effect. They don't cause weight gain. Examples of these medications are sitagliptin (Januvia), saxagliptin (Onglyza) and linagliptin (Tradjenta).

Doctors say fatty liver disease could be one of the first signs of diabetes, but there are usually no symptoms. Dr. Michael Bays, a gastroenterologist on staff at Lee Health, says everyone has a small amount of fat in their liver. “The liver is one of the places that’s responsible for metabolizing and creating fat. It’s where it creates insulin. You’re going to have some fat in the liver but the issue would be--are your liver enzymes abnormal?” Dr. Bays says when there’s too much fat in the liver it’s time to be concerned. “If you have fat in your liver it does indicate that you’re probably becoming somewhat insulin resistant, which may mean pre-diabetic.” Ninety percent of people who have abnormal liver enzymes have fatty liver disease. “When you see someone that has abnormal liver enzymes and has a slightly elevated liver on an ultrasound suggesting some fat in the liver, then you want to treat them with weight loss, make sure they’re not diabetic, and make sure they’re watching their sugar intakes, and checking their cholesterol levels,” said Dr. Bays. Doctors say most patients with fatty liver disease are overweight. If diagnosed patients need to make immediate diet and lifestyle changes to try to prevent the onset of diabetes. “The treatment is: try to keep your BMI low, make sure you avoid simple sugars, and stay away from the diabetic profile,” said Dr. Bays. It’s also important to be physically active and have regular visits with your doctor. Excess fat and calories can be reduced by removing all visible fat and skin from meat and poultry prior to cooking and using healthy methods to cook foods like grilling, steaming, broiling, baking, roasting and poaching. The healthiest dairy products include low-fat or non-fat milk, yogurt and cheese. Low-Cholesterol Foods Cholesterol is a type of fat found in animal products. A report in the August 7, 2011 issue of the "World Journal of Gastroenterology," recommends a diet that is low in saturated and trans fats. Foods high in omega fatty acids such as fatty fish, olive oil and nuts are recommended to help treat fatty liver. The report also recommends foods low in simple sugars and high-fructose corn syrup. Non-alcoholic Beverages Alcohol puts stress on the liver and is closely tied to liver disease. According to the California Pacific Medical Center as little as one glass of beer or wine a week can contribute to a fatty liver. The ALF encourages individuals with a fatty liver to avoid all alcohol. Examples of non-alcoholic beverages include water, juice, soda, tea, coffee, sports drinks and milk.
