This video explains what treatments are available for prostate cancer and the factors that determine what treatment a person might have. Prostate cancer is the most common cancer in men in the UK so it's important to know the facts. Dr. Vincent Gnanapragasm from Cambridge University talks us through prostate cancer treatments: Over 80% of men live for 10 or more years after a diagnosis of Prostate Cancer. I’ve come to Cambridge university hospital to speak with Dr. Vincent Gnanapragasam about some of the treatments that are available for this type of Cancer. So Vincent what are the important factors when deciding what type of treatment a person should have? The treatment options for Prostate Cancer are very much dependent on what type of cancer an individual has and also the patient themselves. So if a cancer is confined to within the prostate gland then we might be able to treat it with surgery or radiotherapy, but equally there might be a very slow growing tumour which we can just keep an eye on that. But the kind of cancer is determined by the size of it or the stage of a cancer, the type of cancer it is or the grade and also the PSA test. Now we use those 3 put together to create an idea of the risk of a particular individual dying of prostate cancer. What treatments are available? In general there is surgery to remove the prostate gland, radiotherapy where you deliver high intensity radiation beams or surveillance where we can monitor or just check on someone and defer treatment until later. So a number of different options then Vincent but how do you decide what one is best? That decision is left to the patient in consultation with his consultant or his nurse specialist and comes down to whether the patient feels that a particular type of treatment is best suited for their lifestyle and also the risks and side effects which may come about from that. And what happens when treatment is finished? After treatment is finished the patient enters a phase of very close monitoring and where they’ll be seen regularly in hospital often with a PSA test being done on a regular interval. What about for treatment of cancers that have been spread? When a cancer has spread beyond a prostate then the most common treatment is hormonal therapy and that serves to keep the cancer under control and many men can live for many years with that and if that should fail there are actually other drugs that can be used, chemotherapies which again can be very effective. Dealing with cancer is never easy and this is especially so when deciding about treatments. But of course it helps if you know the facts. It has been around for quite some time and the side effects are well understood by doctors. While the side effects can be moderate, most patients are able to tolerate them well and are able to continue to have a decent quality of life. Radiation therapy side effects are generally urological and in nature but other symptoms, such as fatigue and diarrhea, may be experienced. I’ve come to Cambridge University hospital to meet Dr. Vincent Gnanapragasam to find out how best to cope. Well Vincent what advice do you have? Certainly hearing you’ve got Cancer is devastating news for yourself and for your family. But I do tell my patients there is every reason to be optimistic. Many men will live with their cancers and cures are eminently possible in many cases. But for those who are struggling to cope there are support services in your own hospital or in your GP practice or indeed nationally which they should go to. Now as a result for treatment for prostate cancer I understand there can be some urinary symptoms? Different treatments for prostate cancer can cause some trouble with urinary symptoms. With surgery you might get incontinence or leakage of urine. With radiotherapy you may get irritability of the bladder, making someone want to pee more often. And those kind of things can be managed with treatments and medication. The key thing is to seek help early when that happens. And what about other physical issues that may come up for some people? A lot depends on the kind of treatment, radiotherapy may for instance cause bowl symptoms and diarrhea transiently. And men who get hormone therapy may find that they become very tired, they put on weight and lose their vitality. And understandably one of the things most people want to know about is how prostate cancer effects their sex life? Unfortunately the prostate being where it is has the nerves for erections wrapped around it and there is no doubt that treatments will cause problems with erectile failure or disfunction. But there is things that can be done to help with this. Medication for example, artificial devices and all these things are available to men who need it after treatment. So can men have children after treatment? So if you have surgery the tubes as they were will be disconnected. However you can still retrieve sperm for fertility that was necessary. In radiotherapy in general that doesn’t affect fertility, the hormone therapy can suppress that for some time. Coping with prostate cancer isn’t easy but it’s good to hear that if you are having any problems there is lots of help available. So I’ve come to Cambridge University hospital to speak with Dr. Vincent Gnanapragasam to find out how they go about diagnosing this common cancer. Well Vincent, Prostate Cancer is notoriously difficult to diagnose isn’t it. Indeed, in fact Prostate Cancer most often has no symptoms. A man might go to his GP because he has notice a change in his urinary symptoms, getting up at night maybe or having difficulty passing water. And most often that’s due to a normal enlargement of the prostate as he gets older. In fact prostate cancer doesn’t cause any symptoms unless it is very advanced. In which case it might mimic some of those symptoms of an obstructed or large prostate. But if there are any concerns the first step is to go and see your GP who can then investigate further. What would your GP do? - Take a history - Do a physical examination - PSA blood test So what would your GP do? Most often they’ll take a history from you, do an examination of the prostate and do a PSA blood test. What is a PSA test? So PSA stand for Prostate Specific Antigene. It’s produced by the prostate gland and it’s a marker for the presence of Prostate Cancer. However it can also be raised by other conditions so it’s not a very specific marker but it’s the first test that we can do to see whether we should proceed to further investigations. What is a PSA test? A blood test that measures the amount of prostate specific antigen (PSA) in your blood. So what happens if you get sent to the hospital for an examination? The specialist will see you, again take a history and an examination and then most often what they will do is arrange a prostate biopsy. Increasingly they might recommend a scan to be done first, an MRI scan and then discuss with you what the best way to do a biopsy is. What is a Biopsy? The removal of a small sample of tissue for examination to determine the presence or extent of a disease. And if it is cancer tell us what happens then, tell us about some of the tests. Well, a discussion then starts. The first step is to make sure we know exactly what kind of Cancer it is and that might involve other types of scans, for example a CT scan or a bone scan. The kind of Cancer is determined by the size of it or the stage of a cancer. And once the specialist puts all that information together with what the biopsy has shown then they start to discuss with you what the treatments are."

Hormone Therapy for Prostate Cancer

Prostate cancer hormone therapy is used quite often when treating prostate cancer patients. This therapy does not involve the patient receiving hormones. Rather, doctors try to suppress the hormone testosterone. Dr. Richard Bevan-Thomas tels us that prostate cancer cells actually use testosterone for fuel in its early stages. Patients are routinely given a shot which tells their brain to stop signalling the testicles to make testosterone. These shots may be administered from one to six months. This therapy works well, but it does have its side effects. Dr. Bevan-Thomas is the Medical Director for Prostate Cancer Live. A graduate of Duke University, Rich received both a master's degree in anatomy and his medical degree from St. Louis University School of Medicine. He completed residency training in urologic surgery at the University of Texas Houston Health Science Center and the renowned MD Anderson Cancer Center, also in Houston. In 2002, he received two top honors in his field; the Pfizer Scholar in Urology and the Gerald G. Murphy Scholar in Urology. read more on the link : Researchers from Canada have found that having a diet of red meat, organ meats, soft drink and bottled water increased the risk of getting prostate cancer. They compared the diets of 80 men with prostate cancer, and 334 healthy men, and the diet just described more than doubled the risk of developing prostate cancer. So what prostate diet can reduce the risk of prostate cancer? Swedish researchers examined the diet of 1499 men with prostate cancer and 1130 men who were healthy, and they published their findings in the Cancer Causes and Control journal. They found that eating foods rich in phytoestrogens decreased the risk of prostate cancer. Phytoestrogens are plant molecules that have a weak estrogenic effect. They are made converted in the intestines from plant precursors by bacteria as they digest food. They are taken up by the same parts of the body that process the hormone estrogen. Interestingly, one of the treatments for prostate cancer by doctors involves using estrogens to reduce the level of testosterone in men with advanced prostate cancer. It does this indirectly, through its effect on the hypothalamus, as less luteinizing hormone releasing hormone is produced by the hypothalamus. This means not as much luteinizing hormone is released by the body, which suppresses the production of testosterone. High doses of estrogens for men can lead to cardiovascular complications. But phytoestrogens obtained from diet, in less quantities than a drug, and with a milder effect to start with, should not pose a problem. And researchers don't yet know what exactly causes phytoestrogens to have such a protective effect, whether it is similar to that of the estrogen treatment for advanced prostate cancers. And they don't know which phytoestrogens are the most active. So what phytoestrogen foods could you include in a diet for a healthy prostate? Soy beans and nuts are a good source. Soy and nuts contain a type of phytoestrogen called isoflavones, whilst berries and seeds contain lignans. Foods could include peanuts, sunflower seeds, peanuts, and berries. The only possible concerns with soy is that a study in mice found that male mice that had a type of heart disease that people also have, suffered heart failure when fed a soy based diet. Whether this extrapolates to the human population is yet to be established. But it may be wise for those me who have this particular genetic heart condition, dilated cardiomyopathy, to be cautious about soy products in their diet. But for other men, soy products could be good news. A meta analysis, which is a study of related studies, published in the International Journal of Cancer, found that diets high in soy lowered men's risk of prostate cancer by 30%. Another important mineral to consider for men is zinc. Zinc is used by men's bodies to make some of the male hormones, and for general prostate health. A deficiency in zinc can lead to problems with the testicles, the prostate, and the health of the sperm. Foods that are great sources of zinc include seeds like pumpkin, sesame, and sunflower; nuts like almonds, brazil, cashews and walnuts; and foods like lettuce, oats and onions. Raw onion is particularly suggested by Paul Bedson, a natural therapist. He also suggests vegetable juice made of equal parts of beetroot, carrot, and cucumber in cases where the prostate gland is enlarged. There was no dispensary in the area, but a friend made me cannabis butter, so I took that, up to tolerance. In three months the primary cancer was gone, only minor metastatic lesions were left. At that point I found a supplier for Rick Simpson oil and killed off the metastases in the next three months. Now I just take a maintenance dose of locally produced hash oil that is 1:1 THC:CBD with about a 30% potency. This will certainly keep me clear of cancer, anywhere, for ever. My point in telling this story is the fact that in the face of advanced aggressive cancer, all I had was very weak cannabutter, but it was enough to eliminate the primary tumor. Now there are strains of 95% THC. But is this necessary? If you have cancer and want to pursue the cannabis treatment, any at all will be good. More important than extreme potency, is balance between THC and CBD. If you can get high potency, great. If not, common potencies will work perfectly. Finally, if you choose cannabinoid treatment, start small, then increase dosage as rapidly as tolerable. To kill cancer you have to hit it hard, be conscientious about your treatment. Cannabis does no harm to the body, it is a metabolic support for the immune system." Radiation therapy is a fairly common treatment for many prostate cancer patients. While the methods are fairly well known, many patents wonder how long the course of treatment will last. There is no cut and dried answer, though, because every case of prostate cancer is different in many ways. The length of treatment for radiation therapy for prostate cancer will first depend on whether the disease being treated has been defined as low or high risk. Higher and intermediate risk prostate cancers generally will have a longer course of treatment. Lower risk prostate cancers generally will have a shorter course of treatment. One of the most common conditions that we deal with as urologists is prostate cancer. Prostate cancer is the most common cancer in men in America. Because it's so common, we screen for it. By "screen," I mean we're checking men every year to try to find prostate cancer when it's in its earliest stages and when it's curable. There are two screening exams that we perform to screen men for prostate cancer, we do a PSA blood test, and we do a digital rectal exam to actually feel the prostate with our finger. In general for men across the boards, we're screening for prostate cancer starting at age 50 and we recommend screening yearly. If a patient is at an increased risk for prostate cancer, meaning if they have a family history of prostate cancer or if they are of African descent, we recommend to initiate screening at age 40. So the treatments for prostate cancer depend on the stage of the prostate cancer. When we look at prostate cancer we can really break it up into early prostate cancer, or advanced prostate cancer. When we diagnose early prostate cancer our goal is to cure the cancer and there are really two main treatment options that we have in America, one being surgery, the other being radiation, and truth be told surgery and radiation are equally effective at curing early prostate cancer. When we diagnose advanced prostate cancer, and when I say "advanced," I mean metastatic prostate cancer, this is not a cancer that is curable, but it is a cancer that we can control through hormonal treatments and or chemo therapy. It's important to note that less than 15% of men with prostate cancer die of prostate cancer. So the vast majority of men are either being cured of their prostate cancer or living with their cancer for many years before they pass of another condition Well, the way that we perform prostate cancer surgery today is very different than from say ten or fifteen years ago. Today we're able to perform laparoscopic surgery using the assistance of robotic technology. The vast majority of patients, I'd say over 90% of patients are able to leave the hospital one day after surgery. Patients after prostate cancer surgery will have a catheter in place while the urinary tract is healing- a urinary catheter in place, and this typically stays in for about a week or so. Once that catheter comes out then the real recovery begins and it's usually another week or two until patients are back to themselves. Some men may experience some urinary incontinence after surgery but typically after the first few months of surgery that incontinence improves. I'd say the vast majority of men, within 3 to 4 weeks are back to work, or back to doing most things like they were doing prior to surgery.

Dr. Mack Roach III of the University of California San Francisco presents his lecture "Treatment of Prostate Cancer in Developing Countries: the Use of 3D as Opposed to IMRT for Locally Advanced Prostate Cancer" at the Greater Horn Oncology Symposium (GHOS) on March 25, 2014. GHOS was a multidisciplinary oncology symposium designed to improve access to cancer care in the Greater Horn of Africa and as a forum for exchange of ideas between oncologists from around the world. The event was hosted by Kilimanjaro Christian Medical Center (KCMC) in Moshi, Tanzania on March 24 and 25, 2014. The event was organized by Radiating Hope, a US-based non-profit organization founded by radiation oncologists Brandon Fisher and Larry Daugherty, with the leadership of Dr Mira Shah, a radiation oncologist with the Henry Ford Hospital in Detroit, and support of Charlie Wittmack, a humanitarian-attorney and the President of Charity-Launch. During the symposium, lecturers discussed common local malignancies, early detection, and treatment. Specific topics included telemedicine, cervical cancer screening, sarcoma brachytherapy and breast brachytherapy, among other topics focusing on cancer care, prevention, early detection as well as treatment. The event also included industry panels, which were assembled to discuss the future of cancer treatment in the region as well as what physical facilities should be built to best serve Tanzania. How Do You Know Radiation Treatment for Prostate Cancer Worked?

After radiation therapy for prostate cancer patients may be wondering how well it worked. Dr. Lanceford Chong offers some insight on how doctors measure these outcomes. Post-radiation followup is a series of procedures used to help doctors determine how well your treatment worked. Followup will include a patient history, physical examination and lab tests including the PSA study. Patients will continue to have PSA evaluations for several years after undergoing prostate cancer treatment. It can take one and a half to two years to determine what the lowest point of a patient's PSA score is. PSA is a protein made in the prostate that is present in men without prostate cancer in very low levels in the bloodstream. When a patient has prostate cancer PSA levels can be detected with lab tests. Dr. Lanceford Chong brings more than 20 years of experience to his role as a radiation oncologist at Cancer Treatment Centers of America (CTCA) at Western Regional Medical Center.
