What Types of Prostate Cancer Patients Should Choose Active Surveillance?

Active surveillance is generally for patients that have lower grade prostate cancer because those cancers tend to grow more slowly. But, there has been a debate for a number of years about the over- and under- treatment of prostate cancer, and whether some prostate cancers need to be treated at all. The short answer is that not all prostate cancers will benefit from treatment, but physicians have to make sure that they are not under treating prostate cancer because it is very difficult, or impossible, to go back in time and take several years back when that patient actually had localized prostate cancer. Urinary Incontinence After Prostate Cancer Surgery

Prostate surgery carries a higher risk of incontinence for men over 70, but younger men aren't immune from incontinence either. Due to more developed pelvic floor muscles in older men, surgery is typically less damaging in younger patients. Regardless of age, I advise all of my patients to meet with a physical therapist after the procedure. By continually practicing what we call Keegel exercises, most men significantly speed up the return of continence. Erectile dysfunction is another side effect of prostate surgery that concerns most patients. Erection nerves run alongside the prostate and research has shown that peeling these nerves off can result in temporary impotence. However, as long as the neurovascular bundles are preserved then the patient should experience returned potency. Men should be mindful of their erection strength prior to surgery to accurately judge the effects of prostate removal. The surgery will likely result in damage, but the effects aren't irreversible. In an ideal world, after radical prostatectomy, the pathologist would send a triumphant report to the surgeon: "I've looked at the prostate tissue you removed from Mr. Jones, and all of the edges are clear. Congratulations! You've removed all the cancer!" Most often, it happens that way. Sometimes, however, the pathologist's report is more ambiguous: Either the margins -- the edges of the removed tumor -- are "positive," meaning they show cancer cells, or they're "close," meaning cancer is just a hair's breadth away from the edge of the specimen. Prostate biopsy is used to find cancer in the prostate gland. Tissue samples are needed in order to do so. During a prostate biopsy needles are inserted through the rectum to remove a small amount of tissue. Dr. Mark Scholz says this procedure is not entirely safe. The rectum is a very dirty place filled with bacteria. Nearly 3% of prostate biopsies result in infection and one in one thousand cases can prove fatal. Prostate cancer hormone therapy is used quite often when treating prostate cancer patients. This therapy does not involve the patient receiving hormones. Rather, doctors try to suppress the hormone testosterone. Dr. Richard Bevan-Thomas tels us that prostate cancer cells actually use testosterone for fuel in its early stages. Patients are routinely given a shot which tells their brain to stop signalling the testicles to make testosterone. These shots may be administered from one to six months. This therapy works well, but it does have its side effects. Dr. Bevan-Thomas is the Medical Director for Prostate Cancer Live. A graduate of Duke University, Rich received both a master's degree in anatomy and his medical degree from St. Louis University School of Medicine. He completed residency training in urologic surgery at the University of Texas Houston Health Science Center and the renowned MD Anderson Cancer Center, also in Houston. In 2002, he received two top honors in his field; the Pfizer Scholar in Urology and the Gerald G. Murphy Scholar in Urology. Radiation therapy is a fairly common treatment for many prostate cancer patients. While the methods are fairly well known, many patents wonder how long the course of treatment will last. There is no cut and dried answer, though, because every case of prostate cancer is different in many ways. The length of treatment for radiation therapy for prostate cancer will first depend on whether the disease being treated has been defined as low or high risk. Higher and intermediate risk prostate cancers generally will have a longer course of treatment. Lower risk prostate cancers generally will have a shorter course of treatment. Dr. Lanceford Chong brings more than 20 years of experience to his role as a radiation oncologist at Cancer Treatment Centers of America (CTCA) at Western Regional Medical Center. Dr. Chong received a medical degree from the University of California, San Francisco School of Medicine, where he also completed a residency in radiation oncology and was appointed Chief Resident his senior year.
