The skin also get darker as it is toughening itself against the environment so this is a protective mechanism which the skin is doing. Nobody wants their face dark so it is very important that you use sunscreens regularly. When you use your sunscreens a dramatic change is not going to happen within few days or weeks, the pigmentation or the constitutional tone which went darker on your face is a cumulative exposure over years so the sunscreen which is a protective mechanism by itself cannot give you all the improvement. In the night you should start some good vitamin C creams which is good skin lightening agent and very safe, you can start using it every night or every alternative night which helps in lightening your skin. Other safe creams to use are glycholic acid creams which exfoliates skin gently use them once in a week. Kojic acid cream is a naturally occurring product. Kojic acid, which is a crystal derived from metabolisation of some fungi actually. It has been used in dermatology for lots of times for the treatment of mostly pigmentation as a skin lightening agent and especially for pigmentation after the acne scars are occurring and it is giving a lot of improvement to the skin. This is available as cream, lotion. It is available as a soap as well. The important thing to not about Kojic acid, it’s a mild skin lightening agent. It is not very harmful to the skin ad it is good to use the Kojic acid in a long term treatment t. there are lot of skin lightening creams in the market which have steroids as well as hydroquinone and both these products have significant side effects on the skin , therefore dermatologists like kojic acid, to prescribe kojic acid because it’s a safer alternative . With long term use there is no damage to the skin as such. But again kojic acid, you have to remember that it is an irritant to the skin. So you have to use it as recommended, preferably at bed time, at night time when you are not going out and make sure that you are using a sunscreen in addition in the morning. OS there is no sensitivity of the skin to the product and in the night use a small quantity of kojic acid on the pimple or over the pigmented area. It takes atleast 6 – 78 weeks for us to see improvement with kojic acid. So you cannot give up using the product in 2 weeks. That’s what happens to most of our patients. They use a product for 2 weeks and they say, this is not working for me. But usually in any skin regimen where we are looking at a topical cream we wait for a minimum of 6 weeks to see an improvement to be seen before we say that the treatment has been unsuccessful.

It is not normal to have periods twice in a month, periods are supposed to come once every month from between 21 to 35 days of cycle is considered normal. So if you had period twice in a month the reason he needs to be explored in, now if you have travelled you have had change of city, change of whether, any stress factors, any other medications for any other reason which is not related to period, be it your fever, urinary track infection, for any skin allergy these can have effect of the period. Consumption of emergency contraceptive commonly called I pill also has effect on the period and if none of these factors are present, it could be a plain simple hormonal imbalance. So these are the factors which need to be ruled out if it is only one episode that you had period twice in a month you can still wait and watch, but if it becomes like every 15-20 days every 5-10 days that you are bleeding and certainly not normal. Polycystic ovarian disease is a disease in women where there are changes in the hormonal levels leading to overweight and irregular cycles. And patient has hyperinsulinemia and the patient is on metformin. So we cannot put the patient on metformin lifelong. So the lifelong treatment for Polycystic ovarian disease is diet and exercise. BMI has to be maintained between 18 to 25. BMI is weight in kgs, to height in meter square. So it has to be maintained. So the girl has to be counseled with diet and exercise. So do one to one and a half hours exercise daily and like gymming, aerobics and swimming, walking, jumping, and climbing. So the mode of exercise the girl has to take high protein diet like egg white, milk products, all types of dals, sprouts, pulses, non-veg pieces. No gravies, almonds, acrot has high proteins. The fruit which has high protein is guava. So high protein diet and daily means of exercise depending upon the BMI is very necessary to bring down a proper BMI for girl with PCOS.

White spots on the face are something called as p alba or polymorphous light eruptions. When we say p alba, they are dry patches on the skin which are ill defined. They are usually dry and scaly, that means they seem as if some powder is coming off it. This is seen in people who do not use mild face wash or moisturizers on the skin and in some people who are prone to allergic tendency like dust allergy , they are prone to p alba. Usually they will say that they had these patches right from childhood. Another is the polymorphous light eruptions, where there is a mild sun allergy. There is a mild irritation and redness of the skin when they go out in the sun. Both these conditions use a gentle soap free face wash, use of face wash which is nice and water based called as hyaluronicacid an use a broad spectrum sunscreen, use it every 3 months, whether you are indoors or outdoors. This will help to bring the problem down. Misuse of over the counter creams or fairness creams could give you patchy whiteness of the skin. This will not get better with the treatment unless you stop the treatment. If with this your white patch does not get better, then it is better to consult your dermatologist because it could be a vitiligo or a leucoderma. Stop using any home care products. Use the gentle face wash, sunscreen and moisturizers. It will resolve in 2 – 3 weeks. If it does not get better, then consult your dermatologist.

But if a lady has undergone cyst of endometrioma which is endiometriosis of ovary then she should conceive as early as possible. The treatment modalysis to keep endometrioma in control is pregnancy where in she does'nt ovulate then endometrioma does'nt reoccur. The time frame for endometrioma is pregnancy as early as possible. Otherwise the couple can plan and get pregnant. After the removal of cyst it is not at all difficult to become pregnant because the ovary is not removed it is the cyst wall which is removed. The sperm leakage or a semen leakage occurs in all individuals whenever there is a sexual activity because the semen cannot enter into the cervical canal. The semen always leaks out but the sperms are actively motile, they move up through the uterus to reach the uterus and the fallopian tubes. Only precaution what the man has to do if she is planning a pregnancy is not to wash out the semen immediately after a sexual activity because water is toxic to the sperms and then lie down for sometime, say 10 – 15 minutes and then wipe out the secretions. So do not be bothered about sperm leakage, it can happen in all individuals.

FHS is Fetal Heart Rate. Fetal Heart Rate is absent at 6 weeks. There are many patients where we do an early pregnancy scan, which is 6 weeks and plus, we do not see a heart beat. There may be many reasons like thick abdominal wall, or obesity, but you may see a fetal pole and a sac. If you see a regular gestational sac with a fetal pole and yolk sac and if you do not get a heart beat, then nothing to worry, just repeat the scan after 1 week. But at 6 weeks, you see a gestational sac and you do not see a fetal pole and yolk sac, then probably there is little more to worry, because you haven’t seen the fetal pole at all because 6 weeks is the time when you should have seen. There are people with delayed cycles, delayed pregnancy, who would have ovulated a little long, who have been on infertility treatment. If a fetal pole and yolk sac is seen at weeks of pregnancy, then you can repeat to see the heart beat, there is nothing major to worry about.

Even a cervical problem can have pain radiating down to the chest ad down to the arm. Other issues are like hyperacidity like in Irritable Bowel Syndrome, Gastritis, Gastro Esophageal Reflux Disease, which is another reason why this symptoms can develop. When the pain radiates to the arm, rule out cardiac issues. If the pain is less than 5 minutes and it disappears after that, it could be angina pectora, which is relieved by sublingual tablets like sorbitrate. The other reason why you can have this kind of pain is the real heart attack. If you have severe pain with profuse sweating, vomiting and you have noticed that patients develop these symptoms after having food, heavy meal, then probably it is a heart attck.it does not come down after 15 minutes. It does not get relieved with any tablets.so then it requires medical intervention. Whereas pain related to the cervical, radiating down to the chest, or the arm, is a neurological pain, and it is usually caused by some bad posture like I had a patient who was regularly on the laptop, and a teacher who is looking up and developed this. So this way symptoms are there. There are also symptoms like Irritable Bowel Syndrome, then the gastritis can cause referred pain down to the arm.

PCOD or Polycystic Ovarian disease. 5 – 10 % in the reproductive age is suffering from thus problem. PCOD”S is because of hormonal imbalance. Heel pain is a very common problem mostly in females compared to me. It will be early morning pain, like you sleep in the night and wake up in the morning , you have a pain and you walk for few steps, 8 – 10 steps, then it gets better. The Same thing may happen when you sit for half an hour and get up. You may get, you may get pain. Again you take few steps, 8 – 10 steps, then your pain will go off. This condition is grouped into a condition called plantar fasciitis, also called as heel pain. Most of the times, there are no causes for this, it just happens like that. Over time, there is a heel pad fat, between the bone and the skin, which will get flattened or worn off. When it gets worn off, the bone will start taking more load. This is the commonest cause. The other one is, there is a tissue called fascia, which connects the heel one to the toes. The plantar fascia from the heel to the toe, it can get tightened, also giving rise to this pain. It happens due to multiple reasons, like rheumatoid arthritis, uric acid problems leading to gout and gout related or gout type of arthritis. Here are more types of arthritis which can lead to pain, but most times the reason is unknown.it can be an old injury like a stone or a thorn which has pricked you. The stone would have come out, but the fat part is injured, leading to long term pain. Sometimes the infection can also lead to pain. How to manage this? An easier way is by exercises, stretching of the plantar fascia, that is stretching of the foot. Secondly wearing soft footwear. Microcellular rubber, microcellular polymer slippers are available. There is silicone heel pad. For​ people wearing shoes can use the silicon cup inside the shoes and that will also help. Sometimes with all this pain doesn’t come down, give pain killers and do physical therapies in the form of ultrasonic massages. If this doesn’t work, give local injections with steroids. Gynaecomastia is basically an enlargement of the breast tissue in men. So how do you differentiate if it is a real gynaecomastia or something which is called pseudo gynaecomastia. Real gynaecomastia or the classical gynaecomastia as that is mentioned in our medical literature occurs only in teenagers and it is usually there is a hormonal background to it and usually occurs during the adolescence when the hormonal changes are occurring and the children are growing, in the teenagers. So because of the hormonal imbalance, they have the glands which grow very prominently and they grow as they are growing and that is what causes embarrassment to the kids because they are going to school, they can’t wear t-shirts, they can’t go swimming, they can’t go out into public without the shirts on and so that is painful. This is true gynaecomastia. But true gynaecomastia, the incidene is very low, it is only about 2%.But we get something called as pseudo gynaecomastia which most of the people have, most of the men have and that is the gynaecomastia that is most commonly people come to get the treatment done. That is basically during puberty, your glands are enlarging because of the hormonal changes which occur and usually about 23 – 24 years, the gland should involute and go back to normal, which doesn’t happen. Once that stays on into adulthood, that is when they come in for the treatment and usually what happens is there is some fat collection around the glands and both of them add to it and that is what causes gynaecomastia, which is the pseudo gynaecomastia or the false gynaecomastia. How do we treat it? Usually because this has happened over during your hormonal changes which are happened in adolescence, it just tends to stay there. If it has to go, it will go. If not it just stays there.it doesn’t cause any problems to you. It is just a cosmetic problem. It is just a source of embarrassment when you go out into public or when you go to the beach or when you go to the swimming pool or the gym, where you have to take off your shirt and they show up. The treatment, lot of people go to the gym and they try to work out, I suggest them not to do any chest exercises because the gland and the fat around the gland is sitting on top of the muscle. The more chest exercises you do, the muscle becomes warmer and it pushes the gland and makes it even more obvious. So if anybody is asking you to do exercise for gynaecomastia, just do aerobics, exercises to burn the fat. Don’t do chest exercises which will to help and they will make it more prominent. When you come to the treatment of gynaecomastia, the treatment sadly is only surgical. You can either leave it alone or you have to remove the gland as well as the fat around it. Nowadays we do the minimally invasive treatment which is basically the liposuction from the same we give a half centimeter cut from where the whole gland is removed and the fat is removed and is given a smooth appearance so that it merges with your muscle and it doesn’t give you the odd look like a breast or a pointed look, where you wear a t-shirt, it is seen. Some people try and remove only the gland. If you remove only the gland, what you end up with is something called a saucered deformity where there is a depression where the gland was and the surrounding area there is a fat collection. So best is you have to deal with the fat as well as the gland and both have to be done at the same sitting so that to have a smooth uniform look and immediately after the surgery within a week you can start with your exercises so that you can build up your gland and the excess skin which is taken care of by the, as you build up your muscle, the skin is taken care of and it takes care of your problem once and for all and it is a permanent solution. Once you do the surgery, it doesn’t come back because you are not going through the old phase and the gland is also removed.

Follow up ultrasound should rule out subchorionic hematoma, check the size, and growing embryo.Routine blood examinations. So, now this patient who is on pills for 3 months have got a regular cycle, following which patient is on metformin, so the chances of pregnancy coming up quite early is definitely there, but, no fix dates can be told 1, 2, 3 months that metformin can cause. But, Metformin definitely increases chances of ovulation and pregnancy.

If you start seeing blood in the mucus there is not much a thing to be panicked. A lot of times we induce this blood by our own actions. Either by clearing the throat forcefully, clearing mucus all the time and spitting it out or using instruments like tongue cleaner or brush to scrape the tongue and injuring the tissues. If you go on clearing the throat and spitting it out, looking at the mucus whether it is blood stained or not, there is no stopping of the blood. Basically after getting injured the blood stops forming clots and if you remove those clots it will again start bleeding and you will continue to see blood in the mucus. Mucus whatever colour, thickness is not abnormal and it is as important as saliva in your mouth. Even if there is lot of mucus in the throat it is not going to hurt you anyways. So leave the mucus alone, let the clot be around so that the blood will stop and the healing will happen. Stop forcefully clearing and spitting out the mucus, stop using tongue cleaners or brush over the tongue. These are not really required. Body has its own mechanisms to take care of whatever deposits are happening around the tongue. Even in spite of all this if you still feel that the mucus is still there and containing blood then consult a specialist, an ENT doctor or pulmonologist and clear the doubt. Let them examine, see if something is abnormal, may be get the mucus testing also and we will come to a conclusion.

I often get asked in my OPD practise that what is the best time to get pregnant after periods. The best time to get pregnant after periods depends upon your period cycle length. The best time is 1 or 2 days before you ovulation and one or two days after your ovulation. This is because the egg is fertilisable only for about 12 to 24 hours after it is getting released. Whereas the sperm can be staying in the female genital tract for about 7 days once it is ejaculated and usually the 28 days cycle the ovulation happens at around the 14th day. So around 12, 13 and 14 the day are the most fertile days. But the ovulation can happen as early as 9th day, 10th day also and also can happen as later as 19th day, 20th day also depending upon the cycle length of the patient. So an average to tell the patient, the most fertile days of the patient is from the day 8 to day 21 of the cycle and sex atleast every alternate days in this time would lead to more chances of pregnancy.

Ptosis is nothing but the dropping of the eyelid. When the 2 lids are not symmetrical and one of the eye is much lower than the other eye, we call it as ptosis. In colloquial terms it is called as drooping. It can be due to a mechanical cause. Be a swelling in the lid and there can be a drooping. It can be due to allergy, and we tend to keep rubbing the eyes and this causes a swelling and hence drooping. These are due to the mechanical causes. There are also conditions due to the nerve conditions which causes drooping. The nerve of the eye can get paralyzed and can cause drooping. This is called as neurogenic ptosis. In addition to this older people can develop a condition called as myasthenia gravis. Myasthenia gravis is a condition which affects the muscles of the body and cause weakness. It affects the eyes and causes drooping of the eyes. This needs to be evaluated. Diabetes can also cause drooping of the eyes. This comes under the category of neurogenic ptosis where the nerve controlling the eyelid become less perfused with blood and this can cause drooping of the eyes.

Patellar dislocation is dislocation of the patella. It is knee cap in simple language and it is present in front of the knee. The injury can happen when you fall or when you are playing and you twist your knee and you fall. Those are the injuries s where you get damage the ligament of the knee cap ad that causes dislocation of the knee cap. Most of the times in the playground itself the patie1nts have a tendency to reduce the knee cap or if they straighten the knee cap then it reces an ies back to its place. This is how it is reduced back in place. Very rarely they do come back to the hospital with a dislocated patella and that time we reduce it under anaesthesia. The recovery of this injury takes up to couple of months upto 3 months. Initially it consists of putting a compression bandage and ice packs and taking x-rays, sometimes MRI scan to check for any loose fragment of bone that may have come off from the knee cap or the patella and is lodged in the joint. If that happens, they may need surgery to get rid of that piece. Otherwise it is mainly a non-operative treatment in the form of compression bandage, ice packs, using crutches to put limited weigh ton the leg till the ligaments heal, which can take upto 3 to 4 weeks. Further 6 weeks is required for physical therapy in the form of strengthening the quadriceps muscle, especially the one which is on the inside of your knee which is attached to the knee cap is called as VMP or vastus medialis obliquus. This muscle needs particularly more strengthening exercises and also to mobilise early and to recover quickly.

"Amniotic fluid index or AFI is measurement of Amniotic fluid during pregnancy in the womb. It is a measurement which is taken by sinologist and it is given usually in millimetres or Centimetres. The normal range of amniotic fluid index or AFI is 8 to 18 centimetre . It is measure by sinologist when you go for your check-ups and it is very good Indicator of fatal indicates wellbeing and healthy pregnancy. The AFI is measured by dividing the uterus into 4 quadrants, in each quadrant a clear pocket of amniotic fluid is seen and measured vertically. Clear amniotic fluid pocket means there should not be any fatal parts or umbilical card in that area so we get whatever is the fluid as a correct measurement. Then all the four major means are added on and the sum is given to you as amniotic fluid index. There are two ways of denoting these index, one is we can just tell the measurement in centimetres saying that it is 5, 6, 10 or 12 or we give some charts In which the percentile is mentioned. In between the percentile of amniotic fluid and index fall below 5 percentile or 5 centimetre is not good, between 8 and 18 is good which is called as normal AFI. Below 8 centimetres we call it as oligoamnios and above 18 Centimetres. When the fluid index is at around about 22 to 21 we call it has polyhydramnios. Reasons for hydro, polyhydromnios and oligoamnios are very different. In oligoamnios there are some factors which are related to mother as improper nutrition, Lifestyle, hypertension during pregnancy, some are placenta indication which are responsible for oligoamnios like poor functioning of placenta and some of it indicates which can be a improper formation of urine in fatal body because of lack of one kidney or poor function of one kidney or some block in the urinary bladder of the fittest so these are the reasons what we normally get when we see oligoamnios. Another biggest reason is PROM premature rupture of membranes, normally gestational diabetes in mother is one of the major reasons for polyamines . So when you go for your routine check up with glaciologist they seen] to AFI and feel very comfortable when it fall in the normal range otherwise and advise you what is to be done to keep it under normal range and keep it in safe limit. It is a personal choice whether you want to get a circumcision or not, sometimes it is based on the beliefs. But, medically, when it is surgically indicated, when you’re not able to retract the skin back enough to clean the area. Most often what happens in uncircumcised penis is if you can’t retract there would be smegma and there is secretions which collect around the head, and they can cause irritations and infections. If you’re not able to pull the skin back and clean that area then you’re a candidate to undergo a circumcision or frenuloplasty depending on the condition. Normally, if you have a normal penis with a normal prepuce you should be able to pull the skin back fully so that it exposes the head of the penis. If that is not possible then you need circumcision, might not necessarily. If you are able expose the head, then you need not do anything. It is basically for hygiene purposes and rule out any other problems. I’m also often asked that they don’t want to undergo a circumcision or a surgery, what are the help that can be done. When the frenulum or prepuce is tight, simple solution is use a moisturizing lotion or when you’re showering gently start pulling the skin back onto the head or the shaft of the penis as you keep doing it daily with some lubrication or with local anaesthetic cream, what happens is your skin is stretching. If the skin is too tight, not able to retract even a little bit, then of course, surgery is the only option. If you have a partial retraction, then these exercises might help stretch the skin, and you may end up not undergoing a surgery at all. But, it has to be gradual, if you try too hard and too fast, then you will end up tears, and sometimes if the tear happens at the frenulum then it might lead to bleeding also, so you’ve to be careful and gentle.
